Abstract
本发明属于医疗器械技术领域,具体是一种多功能鼻胆引流管。其包括引流管段Ⅰ,引流管段Ⅱ,引流管段Ⅲ,引流管段Ⅳ,引流管段Ⅴ,引流管段Ⅵ;引流管段Ⅰ呈直线型,引流管段Ⅱ呈螺旋形,引流管段Ⅱ内的通道一与引流管段Ⅰ相通;引流管段Ⅲ中央完弯曲,所成角度α为30~45°,引流管段Ⅲ内通道三与通道一相通,通道四与通道二相通;引流管段Ⅳ中央完弯曲,所成角度β为60~90°,引流管段Ⅳ与通道三相通;引流管段Ⅴ中央完弯曲,所成角度γ为60~120°,引流管段Ⅴ的通道五与通道三相通;引流管段Ⅵ呈直线型,引流管段Ⅵ内通道七与通道五相通,通道八与通道六相通。本发明可在肝胆胰疾病的内镜治疗等领域广泛推广及应用。
Description
技术领域
本发明属于医疗器械技术领域,具体是一种多功能鼻胆引流管。
背景技术
随着内镜技术的不断发展及其附件的不断完善,目前十二指肠镜技术(ERCP)已然成为治疗肝胆胰疾病的重要措施,包括胆总管结石、急性梗阻性化脓性胆管炎、胆管损伤、胆管狭窄、肝移植术后胆道并发症等。该技术与传统开腹手术相比,具有创伤小、安全系数高、恢复快等优点。ERCP治疗术后常规放置鼻胆外引流管,用于引流感染性胆汁、胆泥、碎石等,可降低术后胆管炎发生率及胆总管结石复发率。
目前临床应用的鼻胆引流管均为单通道引流管,引流管身呈直线型,头端弯曲成襻,引流管头端管壁上设有圆形侧孔,在胆道内-体外的压力差的作用下,胆汁可经管腔引出。该引流管存在以下不足:1.引流管头端自然呈襻,其固定力较小,容易发生引流管脱出;2.拔管过程中,襻式引流管头端可引起胆管及肠道粘膜的损伤,甚至能诱发急性胆管炎、急性胰腺炎、上消化道出血等并发症;3.只具备胆汁外引流的作用,胆汁的持续丢失导致胆汁的中和综合胃酸、杀灭细菌、消化脂肪、刺激肠蠕动等功能减弱,出现代谢性酸中毒、营养不良等并发症;4.经鼻胆引流管造影时,由于引流管管体堵塞于十二指肠乳头,影响造影剂排泄入十二指肠,导致造影剂逆流入肝内胆管,引起胆血反流,导致急性胆管炎的发生,与此同时胆总管中下段显影效果较差。
发明内容
为解决现有技术中存在的缺陷,本发明提供一种多功能鼻胆引流管。
为实现上述发明目的,本发明采用如下技术方案:
多功能鼻胆引流管,包括引流管段Ⅰ,引流管段Ⅱ,引流管段Ⅲ,引流管段Ⅳ,引流管段Ⅴ,引流管段Ⅵ;
引流管段Ⅰ为单通道引流管,呈直线型,头端呈圆锥状,头端1/2管壁上设有侧孔;
引流管段Ⅱ为双通道引流管,呈螺旋形,有2~3个螺旋状环,引流管段Ⅱ内设通道一和通道二,通道一与引流管段Ⅰ相通,通道二头端为盲端,通道二侧壁上设有侧孔;
引流管段Ⅲ为双通道引流管,中央完弯曲,所成角度α为30~45°,引流管段Ⅲ内设通道三和通道四,通道三与通道一相通,通道四与通道二相通,通道四末端为盲端,通道四管壁上设有侧孔;
引流管段Ⅳ为单通道引流管,中央完弯曲,所成角度β为60~90°,引流管段Ⅳ与通道三相通;
引流管段Ⅴ为双通道引流管,中央完弯曲,所成角度γ为60~120°,引流管段Ⅴ内设通道五及通道六,通道五与通道三相通,通道六末端为盲端,通道六管壁上设有侧孔;
引流管段Ⅵ为双通道引流管,呈直线型,引流管段Ⅵ内设通道七及通道八,通道七与通道五相通,通道八与通道六相通。
具体地,引流管段Ⅰ、引流管段Ⅱ、引流管段Ⅲ、引流管段Ⅳ、引流管段Ⅴ和引流管段Ⅵ的管腔外径均为7~10Fr。
具体地,引流管段Ⅰ长度为5~7cm。
具体地,引流管段Ⅱ在自然状态下长1~2cm,伸直状态下长约3~6cm。
具体地,引流管段Ⅱ螺旋状环的外径为1cm。
与现有技术相比,本发明具有以下优点:
1.本发明多功能鼻胆引流管的引流管段Ⅱ具有防滑脱的作用;
2.本发明多功能鼻胆引流管具有胆汁内引流的功能,胆管内胆汁能经引流管段Ⅱ内通道二和引流管段Ⅲ内通道四进入十二指肠,起到中和胃酸、消化脂肪、杀灭细菌及刺激肠蠕动的作用,并能减少胆汁中胆汁酸盐的丢失;
3.进行胆道冲洗或胆道造影时,冲洗液及造影剂能快速灌注整个肝外胆管,冲洗及造影效果更好,并经引流管段Ⅱ内通道二和引流管段Ⅲ内通道四排入十二指肠,降低因胆道压力升高所引起的急性胆管炎的发生率;
4. 本发明多功能鼻胆引流管的引流管段Ⅴ内通道六具有引流胃内胃液及气体的作用,能改善术后腹胀症状,减轻胆汁及胃酸对胃黏膜的损伤,预防胆汁反流性胃炎、急性胃黏膜病变,并能降低术后胰腺炎的发生率。

以下结合具体实施方式对本发明做进一步说明。
一种多功能鼻胆引流管,包括引流管段Ⅰ1,引流管段Ⅱ2,引流管段Ⅲ3,引流管段Ⅳ4,引流管段Ⅴ5,引流管段Ⅵ6;
引流管段Ⅰ为单通道引流管,呈直线型,头端呈圆锥状,头端1/2管壁上设有侧孔;引流管段Ⅰ的长度为5~7cm;
引流管段Ⅱ为双通道引流管,呈螺旋形,有2~3个螺旋状环,螺旋状环的外径为1cm;引流管段Ⅱ内设通道一2-1和通道二2-2,通道一与引流管段Ⅰ相通,通道二头端为盲端,通道二侧壁上设有侧孔;引流管段Ⅱ在自然状态下长1~2cm,伸直状态下长约3~6cm;
引流管段Ⅲ为双通道引流管,中央完弯曲,所成角度α为30~45°,引流管段Ⅲ内设通道三3-3和通道四3-4,通道三与通道一相通,通道四与通道二相通,通道四末端为盲端,通道四管壁上设有侧孔;
引流管段Ⅳ为单通道引流管,中央完弯曲,所成角度β为60~90°,引流管段Ⅳ与通道三相通;
引流管段Ⅴ为双通道引流管,中央完弯曲,所成角度γ为60~120°,引流管段Ⅴ内设通道五5-5及通道六5-6,通道五与通道三相通,通道六末端为盲端,通道六管壁上设有侧孔;
引流管段Ⅵ为双通道引流管,呈直线型,引流管段Ⅵ内设通道七6-7及通道八6-8,通道七与通道五相通,通道八与通道六相通。
引流管段Ⅰ、引流管段Ⅱ、引流管段Ⅲ、引流管段Ⅳ、引流管段Ⅴ和引流管段Ⅵ的管腔外径均为7~10Fr。
使用时,将斑马导丝穿过本发明引流管,在导丝引导下到达指定位置后,将斑马导丝抽出,则引流管呈自然状态。引流管段Ⅱ有2~3个螺旋状环,可以防止其滑脱;胆管内的胆汁经引流管段Ⅱ内通道二和引流管段Ⅲ内通道四进入十二指肠;进行胆道冲洗或胆道造影时,冲洗液及造影剂能快速灌注整个肝外胆管,并经引流管段Ⅱ内通道二和引流管段Ⅲ内通道四排入十二指肠;引流管段Ⅴ内的通道六可以引流胃内胃液及气体。
本发明可在肝胆胰疾病的内镜治疗等领域广泛推广及应用。

1.一种多功能鼻胆引流管,其特征在于,包括引流管段Ⅰ,引流管段Ⅱ,引流管段Ⅲ,引流管段Ⅳ,引流管段Ⅴ,引流管段Ⅵ;
引流管段Ⅰ为单通道引流管,呈直线型,头端呈圆锥状,头端1/2管壁上设有侧孔;
引流管段Ⅱ为双通道引流管,呈螺旋形,有2~3个螺旋状环,引流管段Ⅱ内设通道一和通道二,通道一与引流管段Ⅰ相通,通道二头端为盲端,通道二侧壁上设有侧孔;
引流管段Ⅲ为双通道引流管,中央完全弯曲,所成角度α为30~45°,引流管段Ⅲ内设通道三和通道四,通道三与通道一相通,通道四与通道二相通,通道四末端为盲端,通道四管壁上设有侧孔;
引流管段Ⅳ为单通道引流管,中央完全弯曲,所成角度β为60~90°,引流管段Ⅳ与通道三相通;
