Abstract

一种能消除CT扫描胃内液面伪影的造影剂及其使用方法,是先服用枸橼酸25~35ml,然后服用由1.7~2.5g的NaHCO3与8~12g的羧甲基纤维素钠加水300ml形成的混合液,能够从根本上消除胃内液面的伪影,从而利于CT诊断。

Description

能消除CT扫描胃内液面伪影的造影剂
本发明涉及CT腹部检查消除胃内液面的造影剂及使用方法。
在上腹部CT扫描中,来自胃的伪影给CT诊断带来较大影响,其原因主要有二,一是胃的蠕动,二是胃内气液面。对于前者,大都采用解痉药物,其效果已经得到公认,对于后者,目前国内外尚无好的解决办法,不管是采用稀碘液、稀钡液、水剂、油剂,因口服这些造影剂与胃内气体存在一液平面,二者总是存在较大密度差,常常出现条状伪影,给CT诊断带来影响。
本发明的目的是提供一种能消除CT扫描胃内液面伪影的造影剂,从而利于CT诊断。
本发明消除胃内液面伪影的造影剂由两种组分组成,一种是医用酸性泡腾剂溶液,一种是碱性泡腾剂和固泡剂的混合液。
酸性泡腾剂如枸橼酸、乳酸、苯胺酸,碱性泡腾剂如NaHCO3,固泡剂如羧甲基纤维素钠。
本发明消除伪影的关键是消除胃内气液面,选用枸橼酸、NaHCO3等作泡腾剂,羧甲基纤维素钠等为固泡剂,并且选择适宜的使用方法,使之在胃内产生气泡,从胃下部向上缓慢上升,由于羧甲基纤维素钠粘稠度大,上升之气泡不迅速破裂,形成从下到上逐渐递减的密度差,不形成液平面,从根本上达到消除伪影的目的。
枸橼酸、碳酸氢钠、羧甲基纤维素钠在临床及制药业广为应用,均无毒副作用,未曾应用于CT检查,常用于胃肠低张双重造影检查,与钡剂联合应用,可产生双重对比效果。在这方面的应用要求产生较多气体,且不希望有气泡,而本发明是希望气泡不断产生,且能较长时间存留。本发明主要是通过服用方法及增加羧甲基纤维素钠等固泡剂的用量得到了解决。各组分服用量为:7.5%的枸橼酸25~35ml,1.7~2.5g的NaHCO3与8~12g的羧甲基纤维素钠加水300ml形成混合液。使用方法为:先服用酸性泡腾剂,然后服用碱性泡腾剂和固泡剂的混合液。
本发明通过如下试验说明:
先把7.5%的枸橼酸30ml倒入玻璃杯内,然后再倒入搅匀的2g碳酸氢钠与10g羧甲基纤维素钠混合液300ml。见到由于羧甲基纤维素钠液粘度较大,气泡从杯底产生后缓慢上升,上升至表面的气泡不迅速破裂,形成一气泡带,越往上泡壁越薄,不形成液面。对试验物扫描观察,密度由下向上逐渐减低,无液面形成。达到根本消除液面的目的。对上述试验观察气泡产生及消失时间的结果如下。两种溶液接触后即刻有气泡产生,但数量较少,液面仍存在;30秒至5分钟,气泡大量产生、从底部至表面显示出由高到低的密度差,液面消失。6~8分钟虽上层尚有气泡,但数量明显减少,液面逐渐形成,10分钟以后可出现液面。不难看出30″~5′产泡最多,为消除液面最佳时间。
以下为对比试验:
随机抽查120例上腹部CT扫描病人做为对照组,扫描前口服低密度造影剂(水300~500ml)者86例,服高密度造影剂(300~500ml水加60%泛影葡胺5~6ml)者34例。试验组120例,80例扫描前按本发明服用(亦称为低密度造影剂,接上述试验中所采取的组分和量),另40例服高密度造影剂(本发明再加60%泛影葡胺5~6ml)。
观察:(1)两组服低、高密度造影剂出现伪影情况。(2)观察胃内液面消失情况。根据两组病人CT检查照片进行评定,不存在液面、无伪影或出现轻度伪影但不影响诊断为优片。出现液面和伪影,影响诊断为差片。对照组及试验组病例均使用西德SOMATOM CR全身CT机检查,扫描前15~20分钟均注射654-2,20mg,病人仰卧,扫描时屏气,扫描时间5秒,层厚8mm,窗宽250Hu,窗位30Hu。
结果:
1、对照组服低密度造影剂86例,出现伪影者35例,服高密度造影剂者34例,出现伪影者18例。统计学处理,P>0.05,二者相差不显著。2、试验组80例服低密度造影剂者1例出现伪影,40例服高密度造影者1例出现伪影,经统计学处理,P>0.05。3、胃内液面消除情况比较,按上述评片标准,对照组优片67,差片53,试验组优片118,差片2,经统计学处理,二者有非常显著性差异P<0.01。
虽然试验组有两例有伪影,但经观察,胃内不存在液面,伪影亦不是从气液交界处产生的放射状,而是在有较多气体胃壁旁,较局限,考虑由于个体差异,解痉效果欠佳,胃壁出现小的蠕动而致。这样,就胃内液面消除情况,120例病人均达到满意效果,因为所选用药物,使用方法能从根本上消除液面,通过试验已经得到证实。
试验组120例中,虽然有40例加入泛影葡胺5~6ml,观察所有照片,将二者进行比较,发现胃内液面全部消失,因此即使本发明的造影剂中加入泛影葡胺也不产生伪影。可根据病人不同情况选择使用。
本发明气泡破裂后产生较多气体,聚积于胃的前部,据观察,气体的多少对扫描质量无明显影响。
服用本发明的造影剂后30秒至5分钟气泡大量产生,为消除液面的最佳时间。
本发明的优点:经实验室研究及临床400余例应用验证,从根本上消除了胃内液平面,CT检查不出现伪影,有利于CT诊断。
再举几个实施侧:
实施例一:
枸橼酸25ml,加水300ml搅匀的NaHCO31.7g与羧甲基纤维素钠8g,按本发明使用方法,马上扫描,均达到消伪效果。
实施例二:
枸橼酸35ml,加水300ml搅匀的NaHCO32.5g与羧甲基纤维素钠12g。按本发明使用方法。
实施例三:
枸橼酸28ml,加水300ml搅匀的NaHCO31.8g与羧甲基纤维素钠11g,按本发明使用方法。

Claims (2)

1、一种能消除CT扫描胃内液面伪影的造影剂,其特征在于由两种组分组成,一种是医用酸性泡腾剂,一种是碱性泡腾剂和固泡剂的混合液,所述酸性泡腾剂为枸橼酸,碱性泡腾剂为NaHCO3,固泡剂为羧甲基纤维素钠。
2、根据权利要求1所述的造影剂,其特征在于需要7.5%的枸橼酸25~35ml,1.7~2.5g的NaHCO3与8~12g的羧甲基纤维素钠加水300ml形成混合液。
