Abstract

鼻空肠营养管置入引导装置是针对全麻病人使用气管导管、手术中进行鼻空肠营养管安置时而专门设计的,是鼻空肠营养管安置辅助装置。鼻空肠营养管置入引导装置由推杆、气管引导圈、鼻空肠营养管引导圈、观察通道组成。推杆选用空心管,空心管中心为观察通道,可放置光源与摄像头;推杆的前端为一斜切面,在最顶端为圆弧形,圆弧形中心设有一个定位标识,推杆一侧面安置二个鼻空肠营养管引导圈,对侧面前部安置一个气管引导圈;气管引导圈、鼻空肠营养管引导圈为“C”字型或“U”字形,并具有一定的弹性。

鼻空肠营养管置入引导装置
技术领域
本发明涉及医疗器械,更具体的说是一种鼻空肠营养管置入引导装置。
背景技术
随着现代医学的迅速发展,肠内营养用于支持治疗消化道肿瘤等疾病手术后患者已成为共识。这类手术因切除较多组织及器官,重建消化系统等,创伤较大,致使患者术后多出现胃肠功能恢复较慢,甚至出现胃瘫、胃肠功能恢复困难,直接影响到病人术后恢复。食管、贲门手术后引起的并发症,需患者在长时间内禁食。目前认为,机体禁食一周以上就开始发生肠黏膜萎缩和屏障功能破坏。术后的营养不良,增加了术后离子紊乱,维生素缺乏,伤口愈合不良等并发症的发生。
因此,只要消化道功能存在,应提倡早期经肠道营养支持。肠道营养支持必须通过管饲进行,肠手术中安置鼻空肠营养管是早期肠内营养的有效方法,手术中安全、准确地放置鼻空肠营养管对病人手术后的恢复有重要意义。
管饲途径的选择原则应包括以下几个方面:满足肠内营养需要;置管方式尽量简单、方便;尽量减少对患者损害;患者舒适和能利于长期带管。安置鼻空肠营养管的时间可分为手术前安置与手术中安置;在手术中安置鼻空肠营养管的方法又可分为无创式和有创式二大类别。按照管饲途径的选择原则,临床上常采用无创式的方法安置鼻空肠营养管。目前,在手术前安置鼻空肠营养管的方法是:在手术前将鼻空肠营养管插入胃管尖端,同时送入胃腔,在手术过程中再由胃残端拖出分离,待吻合完毕后再将胃管及鼻空肠营养管各自送至胃腔和空肠。这种方法的优点是置管位置准确,但是这种方法对于癌症患者来说,手术前安置胃管及鼻空肠营养管有引起肿瘤细胞脱落种植、转移的可能。
手术中安置鼻空肠营养管的方法是:手术患者在全身麻醉下切除肿瘤且吻合完毕之后,进行鼻空肠营养管的放置,既减少了患者手术前同时安置胃管及鼻空肠营养管的痛苦,又减少了胃管及鼻空肠营养管接触肿瘤细胞。所以目前多采用的手术中安置鼻空肠营养管的方法(属于无创式)。
采用手术中安置鼻空肠营养管的方法时,由于全麻病人处于无意识状态,无法要求患者完成吞咽等动作以配合置入鼻空肠营养管;同时全麻患者气管内由于插入气管导管,从而在患者口咽部及喉咽部占据一定的位置阻碍鼻空肠营养管经此通道下行入食道。因为喉咽部的上部有气管导管的存在,导致鼻空肠营养管下行至此处时从咽峡返入口腔,当鼻空肠营养管继续下行时,鼻空肠营养管则盘踞在口腔内,鼻空肠营养管不能进入食道,造成鼻空肠营养管插管失败。由于气管导管在咽喉部的阻挡,导致手术中插入鼻空肠营营养管存在一定的难度,鼻空肠营养管易盘在口腔内,护理人员无法顺利插进营养管入食道,这也能称其为盲插。插营养管时间过长,准确率低,无明确方向性致使误插胃管、营养管发生率高,反复盲插导致咽喉部组织水肿加重插管难度;鼻腔内有丰富的海绵状静脉组织,盲插空肠营养管如果不能一次成功,反复多次重插,也易造成鼻黏膜出血。这是采用手术中安置鼻空肠营养管的方法的不足之处,是需要进行改进的。
发明内容
气管导管的插入途径:经口→咽峡→口咽部→喉咽部的上部→喉口→入喉→入气管。
鼻空肠营养管插入途径:经鼻→鼻咽部→口咽部→喉咽部全部→食管→胃→空肠。
由此可知,气管导管与鼻空肠营养管在口中共用通道是口咽部和喉咽部,而在手术中气管插管的病人在全麻状态下置入鼻空肠营养管的障碍也就在于此处。鼻空肠营养管下行被受阻的部位是口咽部和喉咽部,当鼻空肠营养管下行通过喉咽部后就能顺利进入食道。如何置入鼻空肠营养管时,当鼻空肠营养管下行至口腔时,鼻空肠营养管不在盘踞于口腔内,继续下行进入食道,是所要解决的问题。也就是说将“盲插”变为有目的、有引导的“明插”。
人体咽喉部局部解剖显示:气管软骨环在后方缺如,由软组织充填,是气管环中活动度最大处,而食管系肌性结构,紧邻气管后方偏左。根据人体咽喉部局部特点,在行鼻空肠营养管插管时,依托气管导管就能顺利找到进入食道入口的通道。鼻空肠营养管置入引导装置就是以气管导管为依托与导引鼻空肠营养管下行,找到食道入口,再导入鼻空肠营养管进入食道之内。
针对手术中“盲插”置入鼻空肠营养管的弊端,克服手术中“盲插”鼻空肠营养管的缺陷,鼻空肠营养管置入引导装置是人为的在口咽与喉咽的上部建立一个通道,在气管插管的状态下引导鼻空肠营养管的行走方向,避免“盲插”,提高鼻空肠营养管插管成功率,显著减少了其并发症,节约了插管时间,减少了手术医生手术中等待置管成功的时间,从而缩短了手术时间,提高了医生的满意度。
鼻空肠营养管置入引导装置的主要任务是:引导与置入。引导是利用在气管插管的状态下的气管导管为引导物,延着气管导管的路径到达食管入口的唯一通道开口;置入是将鼻空肠营养管在设定通道内顺利行走,并由此将鼻空肠营养管送入食管内。
鼻空肠营养管置入引导装置由推杆、气管引导圈、鼻空肠营养管引导圈、观察通道组成。推杆选用空心管,空心管中心为观察通道;推杆的前端为一斜切面,在最顶端为圆弧形,圆弧形中心设有一个定位标识。推杆前部有定位标识的侧面安置二个鼻空肠营养管引导圈,对侧面安置一个气管引导圈。气管引导圈、鼻空肠营养管引导圈为“C”字型或“U”字形;气管引导圈开口较大;鼻空肠营养管引导圈开口较小。观察通道内可放置光源与摄像头,以便照明与摄像。观察方式可分为直接观察与间接观察,直接观察是操作者用肉眼通过观察通道(空心管的空心处)来直接观看,进行相关的操作,间接观察是在观察通道内放置光源及摄像头,并通过显示器来观看,进行有关操作及教学。利用定位标识确定食道位置,引导鼻空肠营养管进入食道。

具体实施方式
鼻空肠营养管置入引导装置是针对全麻病人使用气管导管、手术中进行鼻空肠营养管安置时而专门设计的,是鼻空肠营养管安置辅助装置。
鼻空肠营养管置入引导装置属于医疗产品,选用的材质应符合相关的规定;鼻空肠营养管置入引导装置可采用一次性成型的加工工艺,也可采用组配加工工艺;鼻空肠营养管置入引导装置所选用的材质应有一定的硬度,同时又要有一定的可塑性。鼻空肠营养管置入引导装置每条边、角均加工成圆弧形,不能为直角,以免在操作过程中造成患者的意外损伤。
鼻空肠营养管置入引导装置由推杆(1)、气管引导圈(2)、鼻空肠营养管引导圈(3)、观察通道(4)组成:推杆(1)选用空心管,空心管中心就是观察通道(4),空心管作为观察通道(4),其中可放置光源与摄像头,也可用肉眼直接观看。这将以往的“盲插”改为“明插”及可扩大观看人群,进行相关的教学。推杆(1)的前端为一斜切面,在推杆(1)最前顶端,设为圆弧形,并在其中心设一个定位标识。定位标识是用来对准、定位食道,引导鼻空肠营养管进入食道。推杆(1)前部一个(有定位标识)侧面安置二个鼻空肠营养管引导圈(3),对侧面安置一个气管引导圈(2)。气管引导圈(2)和鼻空肠营养管引导圈(3)均为“C”字型或“U”字形,并具有一定的弹性;气管引导圈(2)的圈内径与气管外径相匹配,气管引导圈(2)可轻松的卡住气管导管,并能延气管导管上下自由移动,故气管引导管(2)“C”字型或“U”字形的开口较大,以方便卡住气管导管与从其中取下,另一方面气管导管的外径比较大,气管引导圈(2)在延气管导管移动时一般不会掉下。鼻空肠营养管引导圈(3)是用来安置鼻空肠营养管,鼻空肠营养管引导圈(3)的圈内径与外径相鼻空肠营养管匹配,鼻空肠营养管放入鼻空肠营养管引导圈(3)内,能在其内移动,鼻空肠营养管管径比较细,并且比较软,如“C”字型或“U”字形的开口较大将会从其开口处脱落,故鼻空肠营养管引导圈(3)选择开口较小。
推杆(1)选用空心管。采用空心管有二个好处:1、能扩大其功能,利用中心空管,可插入光源和摄像头,来进行可视操作或直接观看;2、减轻总重,便于操作。为了便于定位,在推杆(1)的前端设为一斜切面,并将最顶端改为圆弧形。选取一根150~200MM的非金属软管(具有一定的硬度与可塑性)作为推杆(1)材料,在软管的一端(前端)加工一个45°的斜切面,在斜切面最前端加工成圆弧形,在圆弧形的中心设一个定位标识,此定位标识用来引导对准食道。在圆弧形的中心(定位标识)直线上安置二个鼻空肠营养管引导圈(3),一个鼻空肠营养管引导圈(3)在前端,另一个鼻空肠营养管引导圈(3)在中部;在其对侧面、近斜切面端安置气管引导圈(2)。斜切面前端圆弧中定位标识是用来作为定位之用,用来引导鼻空肠营养管下行入食道。
气管引导圈(2)、鼻空肠营养管引导圈(3)选为“C”字型或“U”字形,是为了方便卡住气管、鼻空肠营养管,并能容易取出。气管引导圈(2)直径大于鼻空肠营养管直径;气管引导圈(2)内径略大于气管导管外径,气管引导圈(2)可从“C”字型或“U”字形的开口处进入、环抱气管导管,并能以气管导管为依托可进行上下移动。为移动方便、减少阻力,气管引导圈(2)的“C”字型或“U”字形的开口可略大些。鼻空肠营养管引圈(3)的内径略大于鼻空肠营养管的外径,鼻空肠营养管可从鼻空肠营养管引导圈(3)“C”字型或“U”字形的开口处进入其中,并能在其中上下推动,为防止鼻空肠营养管在推动中从鼻空肠营养管引导圈(3)“C”字型或“U”字形的开口处滑出,故鼻空肠营养管引圈(3)“C”字型或“U”字形的开口较小。
采用鼻空肠营养管导入装置安置鼻空肠营养管的过程:手术医生在手术中切除消化道肿瘤且吻合完毕之后,下医嘱放置鼻空肠营养管;巡回护士将鼻空肠营养管导入装置的气管引导圈(2)卡在气管导管上;鼻空肠营养管从鼻腔插入,当进入口腔后,用插管钳夹取鼻空肠营养管头端引入(安置)进二个鼻空肠营养管引导圈(3)之中。此时,推杆(1)的二侧分别安置:鼻空肠营养管与气管导管。
抓住鼻空肠营养管置入引导装置的推杆(1)后端,将推杆(1)延着气管导管下行,即向口腔内移动,下行过程中鼻空肠营养管在二个鼻空肠营养管引导圈(3)“C”字型或“U”字形内,增加鼻空肠营养管对推杆(1)的依附性。在操作过程中,可通过观察通道(4)直接观看或在其内放置摄像头,通过显示器观看。当越过悬雍垂处,到达口咽部,根据解剖位置此处空间扩大,调整推杆(1)方向(以气管导管为中心转动),使推杆(1)前端圆弧中心在推杆(1)下方,定位标识正对食道,即鼻空肠营养管引导圈(3)在推杆(1)下方,气管引导圈(2)在推杆(1)上方。从而鼻空肠营养管导入装置依立于气管导管,推杆(1)使气管导管与咽后壁形成一个通道,该通道能方便鼻空肠营养管下行,顺利到达至食管入口处。巡回护士此时将鼻空肠营养管经食管入口处向下推送鼻空肠营养管进入食道,直至与手术医生会合,手术医生配合将鼻空肠营养管送至空肠,手术医生固定住空肠内的鼻空肠营养管;巡回护士将鼻空肠营养管导入装置沿着气管导管向后退至口腔,从推杆(1)两侧鼻空肠营养管引导圈(3)和气管引导圈(2)的“C”字型或“U”字形开口处分离气管导管和鼻空肠营养管;从口中取出鼻空肠营养管置入引导装置,将鼻空肠营养管固定在鼻翼。

1.一种鼻空肠营养管置入引导入装置,其特征是:由推杆(1)、气管引导圈(2)、鼻空肠营养管引导圈(3)、观察通道(4)组成;推杆(1)选用空心管,空心管中心为观察通道(4);观察通道内可放置光源与摄像头,用来照明与摄像,观察可分为直接观察与间接观察;推杆(1)的前端为一斜切面,在最顶端为圆弧形,圆弧形中心设有一个定位标识,推杆(1)前部与中部有定位标识的侧面安置二个鼻空肠营养管引导圈(3),对侧面前部安置一个气管引导圈(2);气管引导圈(2)、鼻空肠营养管引导圈(3)为“C”字型或“U”字形,并具有一定的弹性;气管引导圈(2)的圈内径与气管外径相匹配,;气管引导圈(2)内可安置气管导管,并能延气管导管移动,为减少阻力与方便卡住与取下,气管引导圈(2)开口较大;鼻空肠营养管引导圈(3)的圈内径与外径相鼻空肠营养管匹配,鼻空肠营养管引导圈(3)内可安置鼻空肠营养管,为防止在移动过程中,鼻空肠营养管从鼻空肠营养管引导圈(3)脱落,鼻空肠营养管引导圈(3)开口较小;利用定位标识确定食道位置,引导鼻空肠营养管进入食道。
