Abstract
本发明涉及一种治疗疮疡的药膏及其制备方法,治疗疮疡的药膏包括以下重量份的原料:白芷15‑17份、甘草30‑35份、当归30‑35份、紫草6‑8份、血竭3‑4份、轻粉3‑4份、香油500份、蜂蜡32‑35份。其制备方法为:将白芷、甘草、当归、紫草加入到香油中充分浸泡,然后进行加热直至有黑色烟雾冒出且白芷表面发黑时,停止加热滤除药渣,得药液;趁热向药液中加入蜂蜡进行溶解;冷却后加入血竭粉末和轻粉粉末混合均匀,即得。本发明的药膏能有效抑制创面的有害菌的繁殖;快速促进创面肉芽生长,不仅可用于烧伤、烫伤、各种外伤皮肤感染,还可用于长期不愈的深部皮肤组织溃疡,治愈率高,可以长期储存。
Description
技术领域
本发明属于医药技术领域,具体涉及一种用于治疗各种难以愈合创面(如压疮、下肢静脉溃疡、糖尿病足等)的药膏及其制备方法。
背景技术
创面是正常皮肤(组织)在外界致伤因子如外科手术、外力、热、电流、化学物质、低温以及机体内在因素如局部血液供应障碍等作用下所导致的损害。常伴有皮肤完整性的破坏以及一定量正常组织的丢失,同时,皮肤的正常功能受损,也称为伤口或者创伤。根据创面的愈合周期,分为急性创面和慢性创面。常见的急性创面有:手术切口(Surgicalincision)、皮肤擦伤(Abrasion)、烧伤(Burns)、供皮区(Donor site)等;常见的慢性创面有:褥疮(Pressure sores)、下肢血管性(动脉性/静脉性)溃疡(Leg Ulcer)、糖尿病性足溃疡(Diabetic foot)以及其他难愈合创面(Hard-to-healing)。
创面愈合是指由于致伤因子的作用造成组织缺失后,局部组织通过再生(Regeneration)、修复(Repair)、重建(Reconstruction),进行修补的一系列病理生理过程。本质上是机体对各种有害因素作用所致的组织细胞损伤的一种固有的防御性适应性反应。目前治疗皮肤创伤的药物种类繁多,其在基底细胞自身的分裂、增生和分化的基础上协助基底细胞来实现创面的愈合,但多数药物治疗范围有一定的局限性,功能比较单一,只能选择性应用;且这些药物多数只有抑菌作用,能起到预防的目的,却不具杀菌作用,针对慢性创面治疗周期长,伤口愈合速度慢、治愈率低。
发明内容
有鉴于此,本发明的目的在于克服现有技术的不足,提供一种治疗疮疡的药膏及其制备方法,该药膏能有效抑制创面有害菌的繁殖,不仅对于烧伤、烫伤,各种外伤皮肤感染有较好的疗效,而且对于长期不愈的深部皮肤组织溃疡可快速促进创面肉芽的生长,缩短30%以上的创面治愈周期。
为实现以上目的,本发明采用如下技术方案:
一种治疗疮疡的药膏,包括以下重量份的原料:
白芷15-17份、甘草30-35份、当归30-35份、紫草6-8份、血竭3-4份、轻粉3-4份、香油500份、蜂蜡32-35份。
慢性伤口是由于创面存在异常病理因素或微生物,伤口愈合过程被打断,伤口无法愈合而形成的。常见的病理因素包括:静脉功能不全,导致静脉系统内高静水压,血管内液体渗出,纤维素收缩使小血管闭合,局部组织缺血缺氧;糖尿病患者持续高血糖,许多重要蛋白质的非酶糖化反应增强,导致功能异常;动脉血循环障碍则导致组织缺血和缺氧,甚至坏死。
本发明中的轻粉攻毒敛疮,有杀菌作用;当归具有抗血栓形成作用,对多种致炎剂引起的急性毛细血管通透性增高、组织水肿及慢性损伤均有显著抑制作用,且能抑制炎症后期肉芽组织增生,对体外痢疾、伤寒、副伤寒、大肠杆菌、白喉杆菌、霍乱弧菌及a、B溶血性链球菌等均有抗菌作用;紫草有凉血、活血、清热、解毒之功效,其具有抗菌、抗炎的作用,对金黄色葡萄球菌、灵杆菌、化脓菌、大肠杆菌有抑制作用,并能加速上皮生长以及治疗烧伤,抑制毛细血管通透性,促进创伤愈合;三者的结合可有效破坏由大量细菌定植于伤口床形成生物膜,使得在生物膜保护作用下的细菌等微生物与巨噬细胞和抗生素充分接触,避免巨噬细胞由于发挥“失败”而释放大量的蛋白酶和毒性氧自由基,造成组织细胞、细胞外基质和生长因子等的损伤,阻碍肉芽组织的发育和表皮细胞的迁移;
同时白芷对大肠杆菌、痢疾杆菌、变形杆菌、伤寒杆菌、副伤寒杆菌、绿脓杆菌、霍乱弧菌、人型结核杆菌等均有抑制作用,并可明显抑制二甲苯所致小鼠耳部的炎症;甘草抗炎、抗过敏,有清热解毒之功;白芷和甘草的添加进一步增强了本发明药膏的抑菌作用,避免生物膜的形成;最后血竭活血定痛、化瘀止血、敛疮生肌;当归既能补血,又能活血,既可通经,又能活络,还能显著促进机体造血功能,升高红细胞、白细胞和血红蛋白含量;蜂蜡有抑菌和防腐作用,解毒、生肌、止痢、止血、定痛的作用;香油作为药膏基质具有滋润皮肤、促进新陈代谢和细胞分裂以及延缓衰老作用,血竭、当归、蜂蜡、香油的结合可以促进组织细胞的再生、修复以及创面肉芽的生长,加快创面的愈合,除此之外,蜂蜡的添加使得本发明的药膏可以长期储存而不会变质。
优选地:一种治疗疮疡的药膏,包括以下重量份的原料:
白芷15份、甘草30份、当归30份、紫草6份、血竭3份、轻粉3份、香油500份、蜂蜡32份。
一种制备治疗疮疡的药膏的方法,包括以下步骤:
(1)将上述重量份的白芷、甘草、当归、紫草加入到上述重量份的香油中进行充分浸泡后,得第一混合物料,将上述重量份的血竭和轻粉分别进行粉碎,得血竭粉末和轻粉粉末以备用;
(2)对所述第一混合物料进行加热,直至所述第一混合物料中有黑色烟雾冒出且所述白芷表面发黑时,停止加热并对所述第一混合物料进行固液分离,得药液;
(3)趁热向所述药液中加入上述重量份的蜂蜡并进行搅拌溶解,得第二混合物料;待所述第二混合物料冷却后,向所述第二混合物料中加入步骤(1)中所述的轻粉粉末和血竭粉末进行搅拌混合,混合均匀后即可得到治疗疮疡的药膏。
优选地:步骤(1)中所述充分浸泡的时间为8-10h。其中充分浸泡的判断标准为所述白芷、甘草、当归、紫草均完全沉入所述香油底部。
优选地:步骤(1)中所述血竭粉末和所述轻粉粉末的颗粒细度均为20-300目。
优选地:步骤(2)中所述加热为用武火加热至所述第一混合物料的温度为800-850℃。
本发明中武火指火力大而急的火,大火煮沸水时,水泡大且凸起快,沸腾水泡大小为3-4cm,液面水泡密集沸腾,布满整个液面,水泡起伏周期较短,约为0.5s。
优选地:步骤(3)中所述冷却后的温度为300-350℃。
白芷(拉丁文名:Angelica dahurica(Fisch.ex Hoffm.)Benth.et Hook.f.exFranch.et Sav),又名川白芷、芳香,以根入药,有祛病除湿、排脓生肌、活血止痛等功能。主治风寒感冒、头痛、鼻炎、牙痛。赤白带下、痛疖肿毒等症,亦可作香料。
甘草(学名:Glycyrrhiza uralensis Fisch),别名:国老、甜草根、红甘草、粉甘草、甜草、乌拉尔甘草、甜根子。药用部位是根及根茎,气微,味甜而特殊。功能主治清热解毒、祛痰止咳、脘腹等。
当归,(学名:Angelica sinensis,)别名干归、秦哪、西当归、岷当归、金当归、当归身、涵归尾、当归曲、土当归,其根可入药,具有补血和血,调经止痛,润燥滑肠、抗癌、抗老防老、免疫之功效。
紫草(学名:Lithospermum erythrorhizon),别名山紫草、紫丹、紫芙、藐、地血、鸦衔草、紫草根,是紫草科紫草属植物,有凉血、活血、清热、解毒之功效。治温热斑疹,湿热黄疸,紫癜,吐、衄、尿血,淋浊,热结便秘,烧伤,湿疹,丹毒,痈疡。
血竭,中药名,为棕榈科植物麒麟竭果实渗出的树脂经加工制成,又名麒麟血、骐竭,具有活血定痛,化瘀止血,生肌敛疮的功效。用于跌打损伤,心腹瘀痛,外伤出血,疮疡不敛。
轻粉,中药名。为氯化亚汞(Hg2Cl2)。外用具有杀虫,攻毒,敛疮之功效;内服具有祛痰消积,逐水通便之功效。外治用于疥疮,顽癣,臁疮,梅毒,疮疡,湿疹;内服用于痰涎积滞,水肿臌胀,二便不利。
香油是芝香油,是从胡科植物芝麻种子榨取的脂肪油,亦称胡麻、油麻、巨胜、脂麻、香油等。内服可润肠、润肺;外用作为软膏及硬膏基质;外敷用,如烫伤、烧伤、疮等,用香油和(拌)药(粉状),敷在患处,不干不裂,好的快。
蜂蜡,又称黄蜡、蜜蜡。蜂蜡是由蜂群内适龄工蜂腹部的4对蜡腺分泌出来的一种脂肪性物质。其主要成分有:酸类、游离脂肪酸、游离脂肪醇和碳水化合物。此外,还有类胡萝卜素、维生素A、芳香物质等。具有收涩、敛疮、生肌、止痛、调理之功效。外用于溃疡不敛,臁疮糜烂,创伤、烧、烫伤。常作成药赋型剂及油膏基质。
本发明的使用方法:将创面充分消毒后,使用无菌剪刀等锐器清除创面坏死组织,再次用消毒剂对创面进行消毒,,然后用洁净纱布擦拭干净,再将本发明的药膏涂抹于创面上,并用与本发明的药膏充分接触、搓揉后的纱布或棉垫覆盖于创面上进行固定即可。
本发明的有益效果:
本发明的药膏能有效抑制创面的有害菌(绿脓杆菌、大肠埃希菌等)的繁殖;能快速促进创面肉芽生长,可将创面治愈周期缩短30%以上;本发明的药膏不仅可用于烧伤、烫伤,各种外伤皮肤感染,而且对于长期不愈的深部皮肤组织溃疡也有较好的疗效,治愈率高,可以长期储存,不会变质。
具体实施方式
下面通过3个具体实施例,对本发明的技术方案做进一步的详细描述,在本发明中,若非特指,所有的份、百分比均为重量单位,所有的设备和原料等均可从市场购得或是本行业常用的。下述实施例中的方法,如无特别说明,均为本领域的常规方法。
实施例1
一种治疗疮疡的药膏,包括以下重量份的原料:
白芷15g;甘草30g;当归30g;紫草6g;血竭3g;轻粉3g;香油500g;蜂蜡32g。
制备上述治疗疮疡的药膏的方法,包括以下步骤:
(1)将上述重量份的白芷、甘草、当归、紫草加入到上述重量份的香油中充分浸泡8h,直至所述白芷、甘草、当归、紫草均完全沉入所述香油底部得第一混合物料备用,将上述重量份的血竭粉碎为20目的血竭粉末以备用,将上述重量份的轻粉粉碎为20目的轻粉粉末以备用;
(2)用武火对所述第一混合物料进行加热至第一混合物料的温度为800℃,当所述第一混合物料中有黑色烟雾冒出且所述白芷表面发黑时,停止加热并对所述第一混合物料进行固液分离,得药液;
(3)趁热向所述药液中加入上述重量份的蜂蜡并进行搅拌溶解,得第二混合物料;当冷却至温度为300℃时向所述第二混合物料中加入上述重量份的血竭粉末和轻粉粉末进行搅拌混合,混合均匀后即可得到治疗疮疡的药膏。
实施例2
一种治疗疮疡的药膏,包括以下重量份的原料:
白芷16g;甘草33g;当归33g;紫草7g;血竭3.5g;轻粉3.5g;香油500g;蜂蜡33g。
制备上述治疗疮疡的药膏的方法,包括以下步骤:
(1)将上述重量份的白芷、甘草、当归、紫草加入到上述重量份的香油中充分浸泡9h,直至所述白芷、甘草、当归、紫草均完全沉入所述香油底部得第一混合物料备用,将上述重量份的血竭粉碎为200目的血竭粉末以备用,将上述重量份的轻粉粉碎为200目的轻粉粉末以备用;
(2)用武火对所述第一混合物料进行加热至第一混合物料的温度为830℃,当所述第一混合物料中有少许黑色烟雾冒出且所述白芷表面发黑时,停止加热并对所述第一混合物料进行固液分离,得药液;
(3)趁热向所述药液中加入上述重量份的蜂蜡并进行搅拌溶解,得第二混合物料;当冷却至温度为320℃时向所述第二混合物料中加入上述重量份的血竭粉末和轻粉粉末进行搅拌混合,混合均匀后即可得到治疗疮疡的药膏。
实施例3
一种治疗疮疡的药膏,包括以下重量份的原料:
白芷17g;甘草35g;当归35g;紫草8g;血竭4g;轻粉4g;香油500g;蜂蜡35g。
制备上述治疗疮疡的药膏的方法,包括以下步骤:
(1)将上述重量份的白芷、甘草、当归、紫草加入到上述重量份的香油中充分浸泡10h,直至所述白芷、甘草、当归、紫草均完全沉入所述香油底部得第一混合物料备用,将上述重量份的血竭粉碎为300目的血竭粉末以备用,将上述重量份的轻粉粉碎为300目的轻粉粉末以备用;
(2)用武火对所述第一混合物料进行加热至第一混合物料的温度为850℃,当所述第一混合物料中有少许黑色烟雾冒出且所述白芷表面发黑时,停止加热并对所述第一混合物料进行固液分离,得药液;
(3)趁热向所述药液中加入上述重量份的蜂蜡并进行搅拌溶解,得第二混合物料;当冷却至温度为350℃时向所述第二混合物料中加入上述重量份的血竭粉末和轻粉粉末进行搅拌混合,混合均匀后即可得到治疗疮疡的药膏。
典型病例:
病例1:患者,女,84岁,浙江绍兴人,教师,因长期站立导致左下肢静脉曲张病史10年余,1月余前无明显诱因下出现左小腿外侧皮肤破溃,后创面逐渐扩大,渗液明显,伴臭味。于2016年8月24日入住我院。入院诊断为:静脉溃疡。入院后体格检查创面大小3x5cm2,立即消毒清创换药,完善下肢血管检查。结果显示下肢侧枝静脉循环丰富。创面处理:第一日给予消毒清创换药,锐器清除坏死组织,第二日创面予以涂抹本药膏于创面,再将本药膏与纱布充分揉合均匀放置于创面上,外放洁净干纱布包扎,胶布固定,每日2次,6天后患者创面愈合出院,出院日期为2016年8月29日。
病例2:患者,男,45岁,浙江省杭州市人,患者10年前外伤致胸11、12骨折伴脊髓损伤,当时在浙医二院行骨折内固定术,后遗双下肢活动障碍,5年前因护理不当开始出现骶尾部、双髋部皮肤溃烂,创面逐渐增大,半流脓。患者于2016年6月1日入住我院,入院诊断为:压疮。入院首日完善患者相关检查,创面体格检查为骶尾部0.5x1.5cm2大小不规则创面,左坐骨结节3x2cm2大小创面,右坐骨结节2x2cm2创面。入院后创面处理:第一日给予消毒清创换药,锐器清除坏死组织,第二日创面予以涂抹本药膏于创面,再将本药膏与纱布充分揉合均匀放置于创面上,外放洁净干纱布包扎,胶布固定,每日两次。患者创面于2016年7月19日愈合出院,半年后随访未见复发。
病例3:患者,男,57岁,农民,浙江省金华人。糖尿病史10余年,半月前下地干活时足底被铁钉刺破,继发红肿,于2016年7月11日来我院治疗。入院诊断:糖尿病足伴溃疡。处理:第一日给予消毒清创换药,锐器清除坏死组织,第二日创面予以涂抹本药膏于创面,再将本药膏与纱布充分揉合均匀放置于创面上,外放洁净干纱布包扎,胶布固定,每日两次,患者创面于2016年7月25日愈合。15天后随访溃疡已愈合。
本发明的疮疡膏共治疗静脉溃疡、糖尿病足、压疮、烧烫伤、外伤感染者960例,治愈时间最短3天,最长62天,未发现过敏及其他毒副作用。治愈率100%。

白芷15-17份、甘草30-35份、当归30-35份、紫草6-8份、血竭3-4份、轻粉3-4份、香油500份、蜂蜡32-35份。

白芷15份、甘草30份、当归30份、紫草6份、血竭3份、轻粉3份、香油500份、蜂蜡32份。
3.一种制备如权利要求1或2所述治疗疮疡的药膏的方法,其特征在于,包括以下步骤:
(1)将上述重量份的白芷、甘草、当归、紫草加入到上述重量份的香油中进行充分浸泡后,得第一混合物料,将上述重量份的血竭和轻粉分别进行粉碎,得血竭粉末和轻粉粉末以备用;
(2)对所述第一混合物料进行加热,直至所述第一混合物料中有黑色烟雾冒出且所述白芷表面发黑时,停止加热并对所述第一混合物料进行固液分离,得药液;

4.根据权利要求3所述的方法,其特征在于,步骤(1)中所述充分浸泡的时间为8-10h。
5.根据权利要求3所述的方法,其特征在于,步骤(1)中所述血竭粉末和所述轻粉粉末的颗粒细度均为20-300目。
6.根据权利要求3所述的方法,其特征在于,步骤(2)中所述加热为用武火加热至所述第一混合物料的温度为800-850℃。
7.根据权利要求3所述的方法,其特征在于,步骤(3)中所述冷却后的温度为300-350℃。
