Abstract
本发明提供一种清热除湿、保肝利胆的佩兰口服液,包括以下重量份数的原料药:佩兰10~50份,茵陈10~20份,茯苓10~20份,金钱草10~20份,陈皮10~20份,赤芍10~20份,贯众10~20份,玉米须10~20份,白茅根10~20份,桑寄生10~20份,黄芩10~20份,鸡骨草10~20份,川楝子10~20份,小米草10~20份,薏仁10~20份,垂盆草10~20份,半枝莲10~20份,车前草10~20份,马齿苋10~20份,板蓝根10~20份,女贞子10~20份,菟丝子10~20份,泽泻10~20份。本发明有选择性地降低SGOT作用,大剂量使肝脏指数减少,也说明本药有较好的保肝利胆,除湿减肥排毒的作用。

本发明涉及含有来源于植物、动物或矿物原料的医用保健品,特别涉及一种清热除湿、保肝利胆的佩兰口服液及其制备方法。
背景技术
看中医时,我们常会听医生说“湿热”。那么,什么是湿热,湿热有哪些表现,应注意什么问题呢?要明白湿热,先应了解什么叫湿,什么叫热。所谓湿,即通常所说的水湿,它有外湿和内湿的区分。外湿是由于气候潮湿或涉水淋雨或居室潮湿,使外来水湿入侵人体而引起;内湿是一种病理产物,常与消化功能有关。中医认为脾有“运化水湿”的功能,若体虚消化不良或暴饮暴食,吃过多油腻、甜食,则脾就不能正常运化而使“水湿内停”;且脾虚的人也易招来外湿的入侵,外湿也常因阻脾胃使湿从内生,所以两者是既独立又关联的。所谓热,则是一种热象。而湿热中的热是与湿同时存在的,或因夏秋季节天热湿重,湿与热合并入侵人体,或因湿久留不除而化热,或因“阳热体质”而使湿“从阳化热”,因此,湿与热同时存在是很常见的。湿热体质面部和鼻尖总是油光发亮,易生粉刺、疮疖,常感到口苦、口臭或嘴里有异味,经常大便粘滞不爽,小便有发热感,尿色发黄,女性常带下色黄,男性阴囊总是潮湿多汗。发病倾向:湿热体质的人易患疮疖、黄疸、火热病等征。
湿热的一般表现为:肢体沉重,发热多在午后明显,并不因出汗而减轻;舌苔黄腻,脉数。具体表现因湿热所在不同的部位而有差别:在皮肉则为湿疹或疔疱;在关节筋脉则局部肿痛。但通常所说的湿热多指湿热深入脏腑,特别是脾胃的湿热,可见脘闷腹满,恶心厌食,便溏稀,尿短赤,脉濡数;其它如肝胆湿热表现为肝区胀痛,口苦食欲差,或身目发黄,或发热怕冷交替,脉弦数;膀胱湿热见尿频、尿急,涩少而痛,色黄浊;大肠湿热见腹痛腹泻,甚至里急后重,泻下脓血便,肛门灼热、口渴。
湿热的治疗,一般要分湿重还是热重。湿重的化湿为主,可选用六一散、三仁汤、平胃散;热重以清热为主,可选用连朴饮、茵陈蒿汤、葛根芩连汤。在这一原则下,再根据某些特殊表现选择相应的药,如湿疹、疔疱,加野菊花、地丁草、苦参、白藓皮;关节肿痛加桂枝、忍冬藤、桑枝等;腹泻甚至痢疾加白头翁汤、地榆、车前子等;阴囊湿疹、睾丸胀痛、白带黄臭可加龙胆草、苦参等;血尿可加小蓟草、茅根、石苇、扁蓄等。此外,因热往往依附湿而存在,所以,应注意起居环境的改善和饮食调理,不宜暴饮暴食、酗酒,少吃甜食。
肝胆疾病是严重危害人类健康的常见病多发病,目前无特效药物促进肝细胞再生和防止肝细胞坏死。
佩兰(EupatoriumfortuneiTurcz.)为菊科植物佩兰(兰草)的地上部分,多年生草本,高40-100cm。又名鸡骨香、水香(《本经》)。以全草入药。有解热清暑、化湿健胃、止呕的作用。感受暑湿;寒热头痛;湿润内蕴;脘痞不饥;恶心呕吐;口中甜腻;消洵。用于湿浊中阻,脘痞呕恶,口中甜腻,口臭,多涎,暑湿表症,头胀胸闷。《本经》:主利水道,杀蛊毒。《别录》:除胸中痰癖。《开宝本草》:煮水以浴,疗风。《中药志》:发表祛湿,和中化浊。治伤暑头痛,无汗发热,胸闷腹满,口中甜腻,口臭。《纲目》:消痈肿,调月经。《现代实用中药》:为芳香性健胃、发汗、利尿药。用于冒寒性头痛,鼻塞,神经性头痛,传染性热病,腹痛,腰肾痛,结石等。
佩兰的临床应用:
1、用于湿阻脾胃、脘腹胀满、湿温初起,以及口中甜腻等症。佩兰气味芳香,善于化湿醒脾,功效与藿香相似,治疗湿阻脾胃症候,两药往往相须为用。本品气味清香,性平不温,故又为治疗湿温病症要药,常与藿香、黄芩、苡仁等药配合应用。此外,又适用于湿热内阻、口中甜腻多涎、口气腐臭之症。
2、用于暑湿症。佩兰能醒暑化湿,用于内蕴、畏寒、发热、头胀、胸闷、胃呆等症,,常配合藿香、厚朴、荷叶同用。
3、农历五月以后,天气渐渐炎热,百虫孳生,易发瘟疫(传染病),民间流传有“端午节,天气热;五毒醒,不安宁”的谚语,故防疫禳灾成为端午节民俗的主要内容。
4、佩兰气味芳香,辛能发散,香能去秽,故其有化湿解暑的功效。可用于治疗外感暑湿或湿温初起,症状表现为无汗发热、胸闷腹满。脾经湿热(口中甜腻、多涎、口气腐臭)者也适用。
5、夏季防暑佩兰10克,洗净切碎,冲入开水泡10分钟。有解暑避浊、化湿和中的功效。适用于流行性感冒、头痛鼻塞、恶心呕吐、食欲不振等症。佩兰、金银花、野菊花、绿豆衣各10克,加水煎汤,代茶饮用,可加白糖调味,适用于痱子初起时。
6、疰夏该病主要表现为夏季长期发热、食欲不振、消瘦、口渴、皮肤干燥等。患者多为小儿,多由排汗机能发生障碍引起。方用西瓜翠衣15克,乌梅、藿香、佩兰各10克,煎汤,加冰糖少许。
7、治狐臭:茵陈、香薷各30克,芦根45克,藿香、佩兰各10克,茉莉花5克,研为粗末,水煎代茶饮。每日1剂,能清热利湿、芳香化浊。
8、佩兰药酒治夏天蚊虫叮咬,用佩兰叶搓出水后搽于被咬处,可立止蚊虫叮咬之瘙痒,如加配藿香泡酒外搽则疗效更佳。
9、药浴佩兰煎水沐浴,古时即有此风俗。因其含有挥发油,可抑菌杀菌,能预防和治疗多种夏季皮肤病的发生,还可起到开窍提神、祛风止痛、舒筋活络等医疗保健作用。
10、药枕佩兰古时又称为“醒头草”。将佩兰置于枕芯做内枕,可起到芳香行散、开窍提神之功效,有助于治疗鼻塞、神经性头痛、感冒性头痛。
11、香囊中国民间端午节佩香囊的习俗极为古老,马王堆汉墓出土的文物中就有内装佩兰的香囊。将佩兰放入香囊内佩带,具有芳香化浊辟秽的功效,可以预防多种呼吸道疾病。
佩兰全草含挥发汕1.5%-2.0%,其中主成分为:对-聚伞花素(p-cymene),乙酸橙醇酯(nerylacetate),百里香酚甲醚(methylthymylether)。花及叶中含蒲公英甾醇(taraxasterol),蒲公英甾醇乙酸酯(taraxasterylacetate),蒲公英甾醇棕榈酸酯(taraxasterylpalmi-tate),β-香树脂醇乙醇(β-香树脂醇棕榈酸酯(β-amyrinpalmitate),豆甾醇(stigmasterol),β-谷甾醇(β-sitosterol),二十八醇(octacosanol),棕榈酸(palmiticacid)。茎、叶含延胡索酸(fumaricacid),琥珀酸(succinicacid),甘露醇(mannitol)。地上部分含宁德洛菲碱(lindelofine)。根含宁德络菲碱,仰卧天芥菜碱(lindelofin).

为解决上述技术问题,本发明还提一种所述清热除湿、保肝利胆的佩兰口服液的制备方法,其所述口服液的制备步骤包括:
a.佩兰的制备萃取,作为组分1;
b.将所述其余原料药放入乙醇中加热回流提取2次,作为组分2;
c.将上述两种提取液组分1和组分2合并,浓缩后加蜂蜜或糊精调和,消毒杀菌后制备成口服液装瓶。
所述步骤a中,可以收获佩兰后,除去杂质,切段,干燥,置于有盖不锈钢桶内,加70%乙醇液,质量为粗粉的0.8-1倍,搅拌均匀,湿润密闭放置1小时以上,使充分膨胀;将渗漉筒底部滤板用纱布袋包裹铺平后再将湿润膨胀后的药物样品拌松弄散,然后用不锈钢勺盛粉,均匀的装入渗漉筒,装10-12厘米厚,用T型棒压匀,再按上述操作,一层一层的装入,适当加压,药粉填装不得超过渗漉筒的2/3高处;药粉面上盖不锈钢孔板压牢,打开渗漉筒下面的放料阀,并放一容器,然后缓缓加入70%乙醇液;待排出药粉粉粒之间的空气,并有乙醇流出约20L左右,关闭放料阀,盖上漉筒、浸渍24小时,然后开放料阀进行渗漉,控制渗漉速度一般为1000g药材每分钟流出2~3ml,滤液放入贮液缸内,并将排空时的乙醇液倒入贮液缸;将渗滤液合并静置,加热浓缩至膏状,静置备用,成组分1。
所述步骤b中,可以将所述其余原料泡入10倍量乙醇中,加热提取2次,每次1-2小时,去上清液,合并提取液,100-120目滤过,再经截流分子量为5000-10000的超滤柱超滤,超滤液减压浓缩相对密度为80℃时1.06的浸膏,加热浓缩至膏状,静置备用,成组分2。
所述步骤c中,可以将组分1和组分2合并后,置入双效真空浓缩器中,
浓缩至90℃时相对密度为1.36的浓缩液,置0~5℃低温冷藏24小时;将冷藏液加0.3%的助滤剂硅藻土,过滤,滤液再置入双效真空浓缩器中,浓缩至每1ml含0.1g生药量;浓缩后的膏剂加蜂蜜或糊精调和,紫外线消毒杀菌后装瓶。
为解决上述技术问题,本发明也提一种所述清热除湿、保肝利胆的佩兰口服液的制备方法,制备步骤还可以为:
a.佩兰蒸馏汽化,得精油,作为组分1;
b.将蒸馏后残渣与所述其余原料药一起加水浸泡、提取2次作为组分2;
c.将组分2浓缩后加蜂蜜与组分1调和,紫外线消毒杀菌后装瓶。
所述步骤a中,可以收获佩兰后,除去杂质,洗净,稍润,切段,干燥,放入蒸馏器中汽化提取精油,静置备用,成组分1;所述步骤b中,将蒸馏提取的佩兰渣滓与所述其余原料一起放入占其质量5-10倍量水中,浸泡1-2小时,加热提取2次,每次1-2小时,去上清液,合并提取液,100-120目滤过,将2次提取液合并静置,减压并浓缩至药液浓度为0.6g生药/mL,抽滤后,滤液的相对密度约为20℃时1.08;减压至0.03-0.08MPa,温度保持在60-80℃,浓缩至相对密度为1.20,温度至60℃-70℃的浸膏,作为组分2。
所述步骤c中,可以将组分2放入减压浓缩罐内,减压回收乙醇,浓缩至药液浓度为0.1g生药/mL,抽滤至滤液的相对密度为20℃时1.06;上述滤液经体积为10L的大孔吸附树脂柱吸附后,用10倍树脂柱体积的去离子水或蒸馏水洗脱,再用5倍树脂柱体积的95%乙醇洗脱,收集乙醇洗脱液,去除溶剂,调和组分1,再调和蜂蜜,紫外线消毒杀菌后装瓶。

具体实施方式
有一类胖子,古人称为“肉人”,我们称为结实型的肥胖,与虚胖相反。他们性格急躁,动不动就发火。其实,这类胖子的急躁易怒并不是天生的,而是因为他们的体内有湿热。湿就是身体内的死水,死水与痰结合,就成了痰湿,死水与热结合,就成了湿热。农村的人都知道,麦收以后,农民会将麦秆砌成一垛一垛的,一阵雷雨过后,麦秆被淋湿了,这时你将手伸进麦垛内,就会感觉到里面又潮、又湿、又热,这种湿乎乎、热乎乎的感觉就是湿热。湿热蕴藏在麦垛内,时间一长,麦垛就会腐烂、发酵,农民用它来做肥料;湿热蕴藏在人体内,排不出去,就会造成胆气上溢,口苦口干,急躁易怒。湿热内阻,阳气被遏,因此人总感觉身重困倦。感觉有一层油似的。除此之外,脸上还经常会长出一些痤疮、粉刺,痤疮很多情况是因为体内湿热郁积,排不出去,最后在脸上激发出来,这也是我们平时说的痘痘。看一个胖子有没有湿热,还应看他的饭量。湿热型的人都食欲旺盛,很能吃,但能吃并不表示脾胃功能正常,相反是处于“胃强脾弱”的病理状态。体内有湿热会影响到胃,胃有湿热,中医称为“胃热湿阻”,胃受热之后,其功能就会亢进,这时人的饭量就会大增,动不动就会感到饥饿。然而,胃纳过旺,就势必加重脾运化的负担,脾有“运化水湿”的作用,脾的负担过重容易造成“水湿内停”。所以,中医又将这类胖子称为“胃热湿阻型肥胖”。分辨湿热型胖子,还要看舌。湿热型胖子舌质偏红舌,苔黄腻,体内的热越盛,舌苔就越黄,就好像煮饭的时候火太大,饭就煮焦了,成了焦黄色的锅巴,如果再不关火,锅巴就会烧成了黑色。所以舌苔的颜色越深,证明你体内的湿热越严重。我们还可以通过观察大小便来判断自己是否有湿热,大便太干燥或者太湿,都是体内湿热的表现。如果身体里热重于湿,则大便燥结;湿重于热,则大便黏滞,小便短赤。湿热体质的人,怕湿怕热,对湿环境或气温偏高,尤其是夏末秋初湿热交蒸气候难以适应。看一个胖子体内是否有湿热,还应看他的眼睛,湿热内蕴,热灼血络,这时人的两眼内就会有红赤的血丝,不痛不痒,西医说这是免疫系统的反应。其实,这就是体内有湿热的表现。总的来说,面垢油光、长痘、舌苔黄腻、脾气急躁、眼睛红赤、大便太干燥或者太湿都是湿热型胖子的表现。湿热的治疗,一般要分湿重还是热重。湿重的化湿为主,可选用六一散、三仁汤、平胃散;热重以清热为主,可选用连朴饮、茵陈蒿汤、葛根芩连汤。在这一原则下,再根据某些特殊表现选择相应的药,如湿疹、疔疱,加野菊花、地丁草、苦参、白藓皮;关节肿痛加桂枝、忍冬藤、桑枝等;腹泻甚至痢疾加白头翁汤、地榆、车前子等;阴囊湿疹、睾丸胀痛、白带黄臭可加龙胆草、苦参等;血尿可加小蓟草、茅根、石苇、扁蓄等。此外,因热往往依附湿而存在,所以,应注意起居环境的改善和饮食调理,不宜暴饮暴食、酗酒,少吃甜食。
佩兰(EupatoriumfortuneiTurcz.)为菊科植物佩兰(兰草)的地上部分,多年生草本,高40-100cm。又名鸡骨香、水香(《本经》)。以全草入药。有解热清暑、化湿健胃、止呕的作用。感受暑湿;寒热头痛;湿润内蕴;脘痞不饥;恶心呕吐;口中甜腻;消洵。用于湿浊中阻,脘痞呕恶,口中甜腻,口臭,多涎,暑湿表症,头胀胸闷。《本经》:主利水道,杀蛊毒。《别录》:除胸中痰癖。《开宝本草》:煮水以浴,疗风。《中药志》:发表祛湿,和中化浊。治伤暑头痛,无汗发热,胸闷腹满,口中甜腻,口臭。《纲目》:消痈肿,调月经。《现代实用中药》:为芳香性健胃、发汗、利尿药。用于冒寒性头痛,鼻塞,神经性头痛,传染性热病,腹痛,腰肾痛,结石等。
佩兰的临床应用:
1、用于湿阻脾胃、脘腹胀满、湿温初起,以及口中甜腻等症。佩兰气味芳香,善于化湿醒脾,功效与藿香相似,治疗湿阻脾胃症候,两药往往相须为用。本品气味清香,性平不温,故又为治疗湿温病症要药,常与藿香、黄芩、苡仁等药配合应用。此外,又适用于湿热内阻、口中甜腻多涎、口气腐臭之症。
2、用于暑湿症。佩兰能醒暑化湿,用于内蕴、畏寒、发热、头胀、胸闷、胃呆等症,,常配合藿香、厚朴、荷叶同用。
3、农历五月以后,天气渐渐炎热,百虫孳生,易发瘟疫(传染病),民间流传有“端午节,天气热;五毒醒,不安宁”的谚语,故防疫禳灾成为端午节民俗的主要内容。
4、佩兰气味芳香,辛能发散,香能去秽,故其有化湿解暑的功效。可用于治疗外感暑湿或湿温初起,症状表现为无汗发热、胸闷腹满。脾经湿热(口中甜腻、多涎、口气腐臭)者也适用。
5、夏季防暑佩兰10克,洗净切碎,冲入开水泡10分钟。有解暑避浊、化湿和中的功效。适用于流行性感冒、头痛鼻塞、恶心呕吐、食欲不振等症。佩兰、金银花、野菊花、绿豆衣各10克,加水煎汤,代茶饮用,可加白糖调味,适用于痱子初起时。
6、疰夏该病主要表现为夏季长期发热、食欲不振、消瘦、口渴、皮肤干燥等。患者多为小儿,多由排汗机能发生障碍引起。方用西瓜翠衣15克,乌梅、藿香、佩兰各10克,煎汤,加冰糖少许。
7、治狐臭:茵陈、香薷各30克,芦根45克,藿香、佩兰各10克,茉莉花5克,研为粗末,水煎代茶饮。每日1剂,能清热利湿、芳香化浊。
8、佩兰药酒治夏天蚊虫叮咬,用佩兰叶搓出水后搽于被咬处,可立止蚊虫叮咬之瘙痒,如加配藿香泡酒外搽则疗效更佳。
9、药浴佩兰煎水沐浴,古时即有此风俗。因其含有挥发油,可抑菌杀菌,能预防和治疗多种夏季皮肤病的发生,还可起到开窍提神、祛风止痛、舒筋活络等医疗保健作用。
10、药枕佩兰古时又称为“醒头草”。将佩兰置于枕芯做内枕,可起到芳香行散、开窍提神之功效,有助于治疗鼻塞、神经性头痛、感冒性头痛。
11、香囊中国民间端午节佩香囊的习俗极为古老,马王堆汉墓出土的文物中就有内装佩兰的香囊。将佩兰放入香囊内佩带,具有芳香化浊辟秽的功效,可以预防多种呼吸道疾病。
佩兰全草含挥发汕1.5%-2.0%,其中主成分为:对-聚伞花素(p-cymene),乙酸橙醇酯(nerylacetate),百里香酚甲醚(methylthymylether)。花及叶中含蒲公英甾醇(taraxasterol),蒲公英甾醇乙酸酯(taraxasterylacetate),蒲公英甾醇棕榈酸酯(taraxasterylpalmi-tate),β-香树脂醇乙醇(β-香树脂醇棕榈酸酯(β-amyrinpalmitate),豆甾醇(stigmasterol),β-谷甾醇(β-sitosterol),二十八醇(octacosanol),棕榈酸(palmiticacid)。茎、叶含延胡索酸(fumaricacid),琥珀酸(succinicacid),甘露醇(mannitol)。地上部分含宁德洛菲碱(lindelofine)。根含宁德络菲碱,仰卧天芥菜碱(lindelofin).
茯苓味甘、淡、性平,入药具有利水渗湿、益脾和胃、宁心安神之功用。现代医学研究:茯苓能增强机体免疫功能,茯苓多糖有明显的抗肿瘤及保肝脏作用。茯苓性味甘淡平,入心、肺、脾经。具有渗湿利水,健脾和胃,宁心安神的功效。可治小便不利,水肿胀满,痰饮咳逆,呕逆,恶阻,泄泻,遗精,淋浊,惊悸,健忘等症。茯苓之利水,是通过健运脾肺功能而达到的,与其它直接利水的中药不同。用于脾虚泄泻,带下茯苓既能健脾,又能渗湿,对于脾虚运化失常所致泄泻、带下,应用茯苓有标本兼顾之效,常与党参、白术、山药等配伍。有可用为补肺脾,治气虚之辅佐药。
黄芩其以根入药,味苦、性寒,有清热燥湿、泻火解毒、止血、安胎等功效。主治温热病、上呼吸道感染、肺热咳嗽、湿热黄胆、肺炎、痢疾、咳血、目赤、胎动不安、高血压、痈肿疖疮等症。黄芩的临床抗菌性比黄连好,而且不产生抗药性。《本草纲目》:治风热湿热头疼,奔豚热痛,火咳肺痿喉腥,诸失血。《别录》:疗痰热胃中热,小腹绞痛,消谷,利小肠,女子血闭,淋露下血,小儿腹痛。
茵陈清热利湿;退黄。主治:黄疸、小便不利、湿疮瘙痒、传染性黄疸型肝炎。有利胆,保护肝功能,解热,抗炎,降血脂,降压,扩冠等作用.茵陈有显著的保肝作用,对甲,乙型肝炎,黄疸型肝炎,有显著的疗效。有利胆,促进胆汁分泌,增加胆汁中胆酸和胆红素排出的作用.能增加心脏冠脉血流量,改善微循环,并有降血压,降血脂,抗凝血,利尿解热平喘,驱除蛔虫及抑制多种致病性皮肤真菌与细菌的作用。
金钱草是一种常见植物,而且是一种中药材,他主要功效就是可以预防结石和利胆排石可以说对结石有非常好治疗和预防作用,利水通淋;清热解毒;散瘀消肿,清利湿热,沙淋,尿涩作痛,黄疸尿赤,痈肿疔疮,主治肝胆及泌尿系结石;热淋;肾炎水肿;湿热黄疸;疮毒痈肿;毒蛇咬伤;跌打损伤。
赤芍苦,微寒。归肝经。主治功效:清热凉血;活血祛瘀。主温毒发斑;吐血衄血;肠风下血;目赤肿痛;痈肿疮疡;闭经;痛经;崩带淋浊;瘀滞胁痛;疝瘕积聚;跌扑损伤。用于温毒发斑,吐血衄血,目赤肿痛,肝郁胁痛,经闭痛经,症瘕腹痛,跌扑损伤,痈肿疮疡。《本经》:主邪气腹痛,除血痹,破坚积,寒热疝瘕,止痛,利小便,益气。《别录》:通顺血脉,缓中,散恶血,逐贼血,去水气,利膀胱大小肠,消痈肿,时行寒热,中恶腹痛,腰痛。《药性论》:治肺邪气,腹中疞痛,血气积聚,通宣脏腑拥气,治邪痛败血,主时疾骨热,强五脏,补肾气,治心腹坚胀,妇人血闭不通,消瘀血,能蚀脓。
玉米须味甘、淡,性平。有利尿消肿;清肝利胆的功效。主水肿,小便淋沥,黄疸,胆囊炎,胆结石,高血压,糖尿病,乳汁不通。《滇南本草》:宽肠下气。治妇人乳结,乳汁不通,红肿疼痛,怕冷发热,头痛体困。《岭南采药录》:和猪肉煎汤治糖尿病。又治小便淋沥砂石,苦痛不可忍,煎汤频服。
白茅根凉血止血,清热利尿。用于血热吐血,衄血,尿血,热病烦渴,肺热咳嗽,胃热呕吐,湿热黄疸,水肿尿少,热淋涩痛。《本经》:劳伤虚羸,补中益气,除瘀血、血闭寒热,利小便。《别录》:下五淋,除客热在肠胃,止渴坚筋,妇人崩中。久服利人。
女贞子用于肝肾阴虚的目暗不明,视力减退,须发早白,腰酸耳鸣及阴虚发热等。本品能补肝肾阴,但药力平和,须缓慢取效。治目暗不明,常配熟地,枸杞等同用,治须发早白,常配桑葚同用,阴虚发热,常配地骨皮。肝肾阴虚,头昏目眩、遗精耳鸣、腰膝酸软,须发早白,冠心病、高脂血症、高血压、慢性肝炎。对肝脏的保护作用研究认为,女贞子含有齐墩果酸,给大鼠皮下注射,可降低血清谷丙转氨酶,对四氯化碳诱发的肝损伤有明显保护作用,并能促进肝细胞再生.齐墩果酸亦可使肝内甘油三酯蓄积减少、肝细胞变性坏死明显减轻、糖原蓄积增加、血清γ球蛋白下降;超微结构可见肝细胞内线粒体肿胀与内质网囊泡变性减轻、肝组织间质炎症反应减弱。
川楝子性寒,味苦,是行气药的一种。主要入肝经,疏泄肝热,行气止痛,有除湿热、清肝火、止痛、杀虫的功能。除湿热,清肝火,止痛,杀虫。治热厥心痛,胁痛,疝痛,虫积腹痛。《本经》:″主温疾、伤寒太热烦狂,杀三虫疥窃,利小便水道。″
车前草,中药名。为车前科植物车前PlantagoasiaticaL.或平车前PlantagodepressaWilld.的干燥或新鲜全草。清热,利尿,祛痰,凉血,解毒。主治用于水肿尿少,热淋涩痛,暑湿泻痢,痰热咳嗽,吐血衄血,痈肿疮毒。《别录》:金疮,止血衄鼻,瘀血血瘕,下血,小便赤,止烦下气,除小虫。
垂盆草为景天科景天属多肉植物(拉丁学名:SedumsarmentosumBunge),别名:狗牙半支、狗牙瓣、鼠牙半支、垂盆草是一味民间流传极广的常用药草,全草入药,有清热解毒、消肿利尿、排脓生肌等的功效。生于山坡岩石上或栽培。分布中国南北。清热利湿;解毒消肿。主湿热黄疸;淋病;泻痢;肺痈;肠痈;疮疖肿毒;蛇虫咬伤;水火烫伤;咽喉肿痛;口腔溃疡及湿疹;带状疱疹。
半枝莲(学名:ScutellariabarbataD.Don)(江苏省植物药材志),别称:赶山鞭(四川成都)、瘦黄芩(四川普格),牙刷草(江苏苏州),田基草(江苏宜兴),水黄芩(江苏南京),狭叶韩信草(广州植物志)。是马唇形科黄芩属多年生草本植物。半支莲全草入药,具有清热解毒、活血祛瘀、消肿止痛、抗癌等功能。性寒味酸,全草含多种维生素、微量元素及氨基酸等成分。有凉血解毒,散瘀止痛,消肿和清热利湿之功效。
贯众苦、涩,寒。归肝、胃经。功效杀虫;清热;解毒;凉血止血。主治用于风热感冒;温热癍疹;吐血;咳血;衄血;便血;崩漏;血痢;带下及钩、蛔、绦虫等肠寄生虫病。《本经》:腹中邪热气,诸毒,杀三虫。《别录》:去寸白,破症瘕,除头风,止金疮。《本草纲目》:治下血崩中带下,产后血气胀痛,斑疹毒,漆毒,骨哽。解猪病。
马齿苋(拉丁学名:PortulacaoleraceaL.)为马齿苋科一年生草本植物。该种为药食两用植物。全草供药用,有清热利湿、解毒消肿、消炎、止渴、利尿作用;种子明目。现代研究,马齿苋还含有丰富的SL3脂肪酸及维生素A样物质。SL3脂肪酸是形成细胞膜,尤其是脑细胞膜与眼细胞膜所必需的物质;维生素A样物质能维持上皮组织如皮肤、角膜及结合膜的正常机能,参与视紫质的合成,增强视网膜感光性能,也参与体内许多氧化过程。

菟丝子为双子叶植物药旋花科植物菟丝子、南方菟丝子、金灯藤等的种子,能补肾益精、养肝明目、固胎止泄之功效。全国大部分地区有分布,以北方地区较多。性温,味甘。归肝、肾、脾经。功能主治滋补肝肾、固精缩尿,安胎,明目,止泻。用于阳痿遗精、尿有余沥、遗尿尿频、腰膝酸软、目昏耳鸣、肾虚胎漏、胎动不安、脾肾胎漏、胎动不安、脾肾虚泻;外治白癜风。将原药筛去泥屑,拣去杂质,晒干。有“续绝伤、补不足、益健人”之功。《名医别录》谓其有“养肌强阴、坚筋骨”的作用。祖国医学认为,菟丝子味辛甘,性平,入肝、肾二经。具有补肾益精、养肝明目、安胎的功效。可用于腰膝酸痛、阳痿、早泄、遗精、遗尿、尿频余沥、耳鸣、头眩眼花、视力减退、先兆流产、带下等症。
薏仁味甘、淡,性微寒。其中以蕲春四流山村为原产地的最为出名,有健脾利湿、清热排脓、美容养颜功能。薏苡种仁是中国传统的食品资源之一,可做成粥、饭、各种面食供人们食用。尤其对老弱病者更为适宜。薏苡仁:利湿健脾;舒筋除痹;清热排脓。主水肿;脚气;小便淋沥;湿温病;泄泻带下;风湿痹痛;筋脉拘挛;肺痈;肠痈;扁平疣。滇南本草》:清利小便。治热淋疼痛,尿血,止血淋、玉茎疼痛,消水肿。《本草蒙筌》:治肺痈。《纲目》:捣汁和酒服,治黄疸。《草木便方》:能消积聚症瘕,通利二便,行气血。治胸痞满,劳力内伤。
小米草为玄参科植物小米草、高枝小米草或短腺水米草的全草。学名:EuphrasiapectinataTen,为一年生草本植物,全草具药用价值。味苦;性微寒。归膀胱经,功能主治:清热解毒;利尿。主热病口渴;头痛;肺热咳嗽;咽喉肿痛;热淋;小便不利;口疮;痈肿。贯众
陈皮理气健脾,调中,燥湿,化痰。主治脾胃气滞之脘腹胀满或疼痛、消化不良。湿浊阻中之胸闷腹胀、纳呆便溏。痰湿壅肺之咳嗽气喘。故陈皮有三大类作用,一是导胸中寒邪,二破滞气,三益脾胃。这三大作用中,主要作用是行脾胃之气。脾胃主运化水湿,故脾胃之气行则能去湿、健脾、化痰,故又可以说,陈皮温能养脾,辛能醒脾,苦能健脾。由于陈皮主行脾胃之气,脾胃地处中焦,中焦之气通行,使三焦之气也随之涌动。三焦为决渎之官,通行水液,与湿相伴;又为藏府之外府,上及心、肺,下及肝、肾。所以陈皮的作用可宽及所有藏府,遍及全身之湿。从肺而言,则辛散肺气,苦泄肺气,温化寒气,能治痰多咳喘,气壅食停;从心而言,则辛开心气,苦泄心火,温化湿浊,能治胸中烦热,口气哕臭;从肝而言,则辛散肝郁,苦降肝火,温化寒湿,所以它能治肝郁有热,饮停食滞;从肾而言,则辛润肾燥,苦泄肾湿,温和肾气,所以它能治命火不足,饮食不化。当然,种种解释都与湿和脾有关,所以它的药性作用发挥余地比较大,一般理气化痰方中均可使用。如李时珍说:“橘皮苦能泄能燥,辛能散能和,其治百病,总是取其理气燥湿之功。同补药则补,同泻药则泻,同升药则升,同降药则降。”如和白术同用,则有补脾胃的作用,与人参、甘草同用则有补肺气的作用,与杏仁同用则有通降大肠气秘的作用,与桃仁同用则有通润大肠血秘的作用,与半夏同用则有燥湿的作用,与茯苓同用则有祛湿的作用,与陈皮同用则有降气止呕的作用,与干姜同用能温化寒痰,与黄连同用能清除热痰或者直接服用胃肠饮治疗胃肠疾病疗效更好
桑寄生苦甘,平。入肝、肾经。功能主治补肝肾,强筋骨,除风湿,通经络,益血,安胎。治腰膝酸痛,筋骨痿弱,偏枯,脚气,风寒湿痹,胎漏血崩,产后乳汁不下。治久咳,舌纵眩晕。《本经》:主腰痛,小儿背强,痈肿,安胎,充肌肤,坚发、齿,长须眉。《别录》:主金疮,去痹,女子崩中,内伤不足,产后余疾,下乳汁。
鸡骨草,中药名。为豆科植物广州相思子的干燥全株。甘、微苦,凉。有利湿退黄,清热解毒,疏肝止痛之功效。常用于湿热黄疽,胁肋不舒,胃脘胀痛,乳痈肿痛。《中国药植图鉴》:“治风湿骨痛,跌打瘀血内伤;并作清凉解热药。”

板蓝根(常用别名:靛青根、蓝靛根、大青根)是一种中药材。其性寒,味先微甜后苦涩,具有清热解毒、预防感冒、利咽之功效。主要用于治疗温毒发斑、舌绛紫暗、烂喉丹痧等疾病。研究表明,板蓝根的根茎提取物通过限制病毒分裂而起到对甲型禽流感病毒,乙型禽流感病毒的抑制杀灭作用,其提取物的水溶液对普通感冒病毒有抑制作用。对流行性乙型脑膜炎病毒有抑制和杀灭作用。菘蓝根对多种细菌也有抑制作用.水浸液对枯草杆菌、金黄色葡萄球菌、八联球菌、大肠杆菌、伤寒杆菌、副伤寒杆菌、痢疾(志贺、弗氏)杆菌、肠炎杆菌等都有不同程度的抑制作用;丙酮浸出液也有类似作用,且对溶血性链球菌有效(皆用琼脂小孔平板法).对A型脑膜炎球菌之抑菌作用与大蒜、金银花相似.对流感病毒PR8株和京科68-1株有明显抑制作用.在试管内板蓝根有杀灭钩端螺旋体的作用。
本发明具体实施方式
具体实施例1:
收获佩兰后,除去杂质,干燥切段,切碎后将其泡入5-10倍量的乙醇中,将药粉置有盖不锈钢桶内,加70%乙醇液,质量为粗粉的0.8-1倍,搅拌均匀,湿润密闭放置1小时以上,使充分膨胀;将渗漉筒底部滤板用纱布袋包裹铺平后再将湿润膨胀后的药物样品拌松弄散,然后用不锈钢勺盛粉,均匀的装入渗漉筒,装10-12厘米厚,用T型棒压匀,再按上述操作,一层一层的装入,适当加压,药粉填装不得超过渗漉筒的2/3高处;药粉面上盖不锈钢孔板压牢,打开渗漉筒下面的放料阀,并放一容器,然后缓缓加入70%乙醇液;待排出药粉粉粒之间的空气,并有乙醇流出约20L左右,关闭放料阀,盖上漉筒、浸渍24小时,然后开放料阀进行渗漉,控制渗漉速度一般为1000g药材每分钟流出2~3ml,滤液放入贮液缸内,并将排空时的乙醇液倒入贮液缸;将渗滤液合并静置成组分1。
将其余原料药茵陈,茯苓,金钱草,陈皮,赤芍,贯众,玉米须,白茅根,桑寄生,黄芩,鸡骨草,川楝子,小米草,薏仁,垂盆草,半枝莲,车前草,马齿苋,板蓝根,女贞子,菟丝子,泽泻,放入10倍量乙醇中,浸泡1-2小时,加热提取2次,每次1-2小时,合并提取液,静置后去上清液,然后放入减压浓缩罐内,减压回收乙醇,浓缩至药液浓度为0.6g生药/mL,抽滤至滤液的相对密度为80℃时1.06的浸膏,成为组分2。
将组分1和组分2合并后置入双效真空浓缩器中,浓缩至90℃时相对密度为1.36的浓缩液,置0~5℃低温冷藏24小时;将冷藏液加0.3%的助滤剂硅藻土,过滤,滤液再置入双效真空浓缩器中,浓缩至每1ml含0.1g生药量;浓缩后的膏剂加糊精调和,紫外线消毒杀菌后装瓶。
具体实施例2:

将茵陈1500g,茯苓1500g,金钱草1500g,陈皮1500g,赤芍1500g,贯众1500g,玉米须1500g,白茅根1500g,桑寄生1500g,黄芩1500g,鸡骨草1500g,川楝子1500g,小米草1500g,薏仁1500g,垂盆草1500g,半枝莲1500g,车前草1500g,马齿苋1500g,板蓝根1500g,女贞子1500g,菟丝子1500g,泽泻1500g,将其余原料药的药放入10倍量乙醇中,浸泡1-2小时,加热提取2次,每次1-2小时,合并提取液,静置后去上清液,然后放入减压浓缩罐内,减压回收乙醇,浓缩至药液浓度为0.6g生药/mL,抽滤至滤液的相对密度为20℃时1.06的浸膏,成为组分2。将组分1和组分2合并后置入双效真空浓缩器中,浓缩至90℃时相对密度为1.35的浓缩液,置0~5℃低温冷藏24小时;将冷藏液加0.3%的助滤剂硅藻土,过滤,滤液再置入双效真空浓缩器中,浓缩至每1ml含0.1g生药量;浓缩后的膏剂加糊精调和,紫外线消毒杀菌后装瓶。
具体实施例3:将收获佩兰4500g后,除去杂质,洗净,稍润,切段,干燥,用蒸馏法将其汽化蒸馏后提取精油,成组分1,将所述其余原料茵陈
1200g,茯苓1100g,金钱草1100g,陈皮1200g,赤芍1100g,贯众1200g,玉米须1100g,白茅根1100g,桑寄生1200g,黄芩1100g,鸡骨草1200g,川楝子1100g,小米草1200g,薏仁1100g,垂盆草1200g,半枝莲1100g,车前草1200g,马齿苋1200g,板蓝根1200g,女贞子1200g,菟丝子1200g,泽泻1200g,放入5-10倍量乙醇中,浸泡1-2小时,加热提取2次,每次1-2小时,去上清液,合并提取液,100-120目滤过,将2次提取液合并静置,减压回收乙醇,并浓缩至药液浓度为0.6g生药/mL,抽滤后,滤液的相对密度约为20℃时1.08;减压至0.03-0.08MPa,温度保持在60-80℃,浓缩至相对密度为1.20,温度至60℃-70℃的浸膏,作为组分2。将组分2置入双效真空浓缩器中,浓缩至90℃时相对密度为1.05的浓缩液,置0~5℃低温冷藏24小时;将冷藏液加0.3%的助滤剂硅藻土,过滤,滤液再置入双效真空浓缩器中,浓缩至每1ml含0.1g生药量;浓缩后的膏剂加组分1的精油然后加蜂胶调和,紫外线消毒杀菌后装瓶。
具体实施例4:将收获佩兰4500g后,除去杂质,洗净,稍润,切段,干燥,泡入10倍量乙醇中,加热提取2次,每次1-2小时,去上清液,合并提取液,100-120目滤过,再经截流分子量为5000-10000的超滤柱超滤,超滤液减压浓缩相对密度为80℃时1.06的浸膏,加热浓缩至膏状,静置备用,成组分1。将茵陈1200g,茯苓1100g,金钱草1100g,陈皮1200g,赤芍1200g,贯众1200g,玉米须1100g,白茅根1100g,桑寄生1200g,黄芩1100g,鸡骨草1100g,川楝子1100g,小米草1200g,薏仁1100g,垂盆草1200g,半枝莲1100g,车前草1200g,马齿苋1200g,板蓝根1200g,女贞子1100g,菟丝子1200g,泽泻1200g,,将所述原料药放入粉碎机粉碎后,将颗粒物放入乙醇中加热回流提取2次,每次1~2小时,将2次提取液合并静置;将上述乙醇提取过的药渣加10倍量水加热回流提取2次,每次1~2小时,将2次提取液合并静置,将上述两种提取液合并,作为组分2;合并组分1组分2减压浓缩相对密度为80℃时1.36的滤液,回收乙醇,调入蜂胶,紫外线杀菌后装瓶。
试验例1:本发明急性毒性试验
一、试验材料:
动物:昆明种小鼠,体重21-24g,雌雄各半,山东大学生物试验室育种。药物:本发明口服液(按实施方法2制备)。
二、方法:
1、LD50计算:采用改良寇氏法,将小鼠随机分成5组,每组10只,雌雄各半,将栓剂溶解,配成最大浓度,按小鼠最大允许容量给药,所给剂量按生药量依次为18,14.4,11.5,9.2,7.4(g.kg-1),在动物禁食(不禁水)18小时后,一日内分两次给药(间隔半小时),每次0.5ml,观察动物死亡情况。
2、最大耐受剂量测定(MTD值):取小鼠20只,雌雄各10只。将栓剂加蒸馏水溶解,配成最高浓度,按动物的最大耐受量,以注射灌喂器能抽动为准。在动物禁食(不禁水)18小时后,一日内分两次给药(间隔半小时),每次0.5ml(每ml含生药0.36g),总药量为18g生药/kg.d,相当临床成人50Kg体重用量的300倍。给药后连续观察7天。
三、试验结果:
在LD50计算中当用最大允许浓度和最大允许容量给予小鼠时(18g/Kg.d),未见小鼠死亡,即未测出LD50,只可求最大耐受剂量,在7天观察期中,动物其食欲、活动、毛色、精神状态等皆正常,发育正常,未见有死亡。即选用相当于临床剂量的300倍药量,并无不良反应发生,表明急性毒性极小,MTD>18g/Kg.d。
试验例2:本发明急性皮肤刺激试验报告
1、实验目的:检测本发明对实验动物皮肤的刺激/腐蚀作用和强度
2、材料和方法:
2.1.受试物:本发明口服液(按实施例方法2制得)
2.2.实验动物:壹级大耳白种家兔,10只,雌性,体重2.1-2.3kg;由青岛大学医学院生物系实验动物中心提供;
2.3.试验方法:
选用皮肤完好的健康家兔10只,于试验前24小时将其背部脊柱两侧被毛剪掉,去毛面积左、右各约4cm×4cm。试验时取受试物0.2ml涂于2.5cm×2.5cm的二层纱布上,敷贴于一侧去毛皮肤表面,油纸覆盖,胶布固定。另一侧皮肤作为空白对照。封闭4小时后,去除覆盖物,并用温水清洗去残留受试物,于去除受试物后的1、24和48小时观察涂抹部位皮肤反应,并评分。
3、实验结果:实验动物皮肤未出现任何红斑,红肿,发炎现象。
4、小结:根据《消毒技术规范》(1999)中皮肤刺激反应评分标准判定,本发明对动物皮肤刺激强度属无刺激性。
实施例3:长期毒性实验资料
1、实验方法
将豚鼠随机份成4组,每组15只。在豚鼠背部脊柱两侧将脱毛剂均匀涂上,使其脱毛范围约40平方厘米。洗净脱毛剂,观察24小时后每组豚鼠分别涂本发明口服液溶液0.2、0.4和0.8ml,分别含生药92、184和368mg,另一组涂溶媒0.8ml每日二次,连续30天,观察豚鼠的一般情况,实验结束后处死动物进行血液学、血液生化及病理学检查。
2、结果
上述三组用药豚鼠躯干去毛区,未见局部皮肤有水肿、充血、红斑、出血点及溃疡。用药组与对照组动物毛发色泽、摄食、四肢活动等对照组无明显差异,血液生化检查,用药组与对照组均在正常范围。病理组织学检查,实验各组心、肝、肾及局部皮肤均未见明显病变。提示,本发明中药口服液长期用药对局部皮肤及全身重要脏器均未见明显的毒性作用。
试验结果表明:本发明口服液实验安全。该药对乙型链球菌、金黄色葡萄球菌、大肠杆菌的最小抑制浓度为1∶150,淋球菌为1∶150,白色念珠菌为1∶300,对1∶20稀释度时的杀灭时间为5分钟,1∶50时为10分钟,1∶100时为20分钟,1∶200时为40分钟,1∶400时为8小时。
药理学实验:
佩兰对正常小鼠由四氯化碳引起的肝损伤有明显保护作用,表现为SGOT显著降低,使肝体比降低(大剂量)。对正常大鼠有显著的利胆作用,胆汁流量增加持续2h。熊去氧胆酸流量增加持续1小时。胆汁中胆固醇、胆红素给药前和给药后组间比较未见显著差异性。为此我们进行了实验研究。
1、材料与方法
药物:本发明提醇沉液的制备:取一定量本发明原料药,水煎两次,水浴上浓缩至一定体积后加95乙醇沉淀杂质,过液,滤液回收乙醇。配成100%浓度。
云芝肝泰系吉林省集安制药厂生产,批号为吉卫药准字:831430011。熊去氧胆酸为日本三菱化成生产。
动物ICR小鼠,SD大鼠均由本院动物研究室提供。
1.1对四氯化碳引起的小鼠肝损伤的保护作用:雄性小鼠78只,体重18-23g,按体重随机分层分成六组,每组13只。本发明3个剂量组2.5g/kg、5.0g/kg、10.0g/kg,阳性对照药云芝肝泰10g/kg,模型组,水对照组每天灌胃给药一次,连续6d,于第六天给药后1h除水对照组外其余各组均皮下注射0.15的四氯化碳菜油5ml/kg。16h后摘眼球取血用北京化工厂产的试剂盒测定血清SGPT、SGOT。解剖出肝脏称重,根据处死时体重计算每只小鼠的肝体比、数据以均值士标准差表示,组间进行t检验。
1.2对正常大鼠胆汁流量的影响1.2.1
胆汁流量的测定:取大鼠60只,雄性,体重250~350g,称重随机分成五组,每组12只,实验前禁食12h,不禁水。实验以10%乌拉坦1.0g/kg腹腔注射麻醉,仰位固定于手术台,沿腹正中线切开腹壁,找出十二指肠,在降部肠系膜中找到白色有韧性的胆管,在其下穿二根丝线,结扎乳头部,向肝脏方向作“V”形切口,插入硬质塑料管,结扎固定,即可见有淡黄绿色胆汗流出,试管收集胆汁,术后以盐水纱布覆盖腹腔,待稳定1h后,先收集30min的胆汁,作为给药前的基础值,然后分组给药(经十二指肠)。本发明3个剂量组2.5g/kg、5.0g/kg、10.0g/kg,水对照组,阳性药熊去氧胆酸0.5g/kg。给药后每隔30min收集测量1次胆汁。共4h。
1.2.2胆汁成分的测定:将给药前,给药后4h收集的胆汁测定胆红素(伊利康生物技术有限公司生产,咖啡因法)。胆固醇含量测定用东欧生物工程公司生产的试剂盒,酶法测定。
2实验结果
2.1对四氯化碳引起的小鼠肝损伤的保护作用:四氯化碳引起的SGPT、SGOT显著升高P<0.01,说明造型成功。本发明使SGOT显著降低三个剂量组均P<O.05,本发明对SGPT无明显影响。云芝肝泰使SGPT、SGOT显著降低(P-~O.05)。泽兰10g/kg组使肝体比显著低于四氯化碳模型组。详见表1。

2.2对正常大鼠胆汁流量的影响:本发明2.5g/kg组使正常大鼠胆汁流量增加,持续2h,以给药后的30min增加最显著<0.05)。5.0g/kg组给药2h各时相都显著增加<0.05.10.0g/kg组给药后90min内各时相都比对照组显著升高(P<0.01,P<0.05)。阳性药熊去氧胆酸也使流量增加,持续1h。P<0.05详见表2。

注t以给药前流量为100%,组问进行t检验,与对照组比*P<0.05,**P<0.01.本发明给药前、给药后涓定胆汁中胆红素和胆同醇含量与对照组相比未见显著差异。详见表3。

讨论:
肝胆疾病是严重危害人类健康的常见病多发病,目前无特效药物促进肝细胞再生和防止肝细胞坏死。我们的实验已经证明本发明有良好抗实验性肝硬变作用。本实验表明本发明对CCL4引起的急性肝损伤有明显的保护作用,有使SGOT显著降低作用。血清转氨酶升高主要反映肝细胞变性、坏死,细胞膜通透性升高,使肝细胞内GPT、GOT释放入血所致。有文献报道由CCL4中毒引起的动物SGPT、SGOT升高与肝细胞损伤程度呈粗略平行关系。本发明有选择性地降低SGOT作用,大剂量使肝脏指数减少,也说明本药有较好的保肝作用。
临床试验
自2013年2月至2015年8月,采用自拟清肝利湿降脂汤治疗脂肪肝80例,取得显著疗效,临床资料本组80例中为体检发现者和本单位门诊患者,年龄32-50岁,平均42岁;疗程1~6年不等。两组资料经Ridit检验,年龄、病程等方面均无显著差异性。具有可比性。诊断依据临床表现和影像学及实验室检查,均符合脂肪肝特征。按就诊顺序随机分为治疗组和对照组各40例。诊断标准:国家中医药管理局中医肝病重点专科写作组主编《中医肝病诊疗常规》2005年10月第1版。
治疗方法
治疗组:本发明口服液,方药组成佩兰30g,茵陈12g,茯苓11g,金钱草13g,陈皮12g,赤芍11g,贯众12g,玉米须11g,白茅根11g,桑寄生12g,黄芩11g,鸡骨草12g,川楝子12g,小米草12g,薏仁11g,垂盆草12g,半枝莲11g,车前草12g,马齿苋12g,板蓝根13g,女贞子12g,菟丝子11g,泽泻12g,按实施例方法2制备,每日3瓶,连服2个月后,停服1周,再服1个月为1疗程。
对照组:强肝胶囊(石家庄东方药业有限公司生产)国药准字:ZL0980012,1次5粒,ld3次。两个月1疗程。治疗期间停服任何药物。
疗效标准:临床治愈:症状、体征完全消失,肝功能正常,血脂基本正常,B超提示肝脏声像恢复正常。有效:症状体征消失,谷丙转氨酶及血脂升高部分下降50%以上,B超提示脂肪肝声像图减轻。无效:未达以上标准者。
治疗结果见表1。

*临床治愈与有效视为有效例数,经统计学处理两组比较有明显差异(P<0.05),治疗组明显优于对照组。
讨论:脂肪肝是近年来才引起医学界高度重视的肝脏脂肪代谢紊乱性疾病。现代医学认为本病主要由脂类摄人过多、肥胖、或毒物(乙醇),或遗传与代谢因素引起过量的脂肪在肝内堆积致病。查阅古典,祖国医学虽没有脂肪肝之病名,但在历代医籍中、有一些类似本病的记载。《灵枢.卫气失常》即已指出人体内有“脂、”有“膏”有“肉”、并根据人体形体的不同而分为“脂人”、“膏人、”“肉人”。其云:“膏者,多气而皮纵缓,故能纵腹垂腴。肉者,身体容大。脂者,其身收小。”张志聪《灵枢集注》云:“中焦之气,蒸津液化.其津微溢于外则皮肉膏肥。余于肉,则膏脂丰满。”张景岳《类经》亦云:“精液和合而为膏,以填补骨空之中,则为脑为髓,为精为血。”由此可见,上文描述之膏脂与现代医学之脂类物质相类似。同时也说明膏脂实乃人体的生理组成成分之一,属精液之范畴,并可与津液其他成分相互转化,津从浊化为膏,凝则为脂。正常脂膏随血液的运行营养五脏六腑,四肢白骸以及脑髓。若禀赋不足,饮食不节,脾胃失调,情志内伤,肝胆失调,年老体弱,身虚不足等原因,而致摄食过多或传输、利用、排泄异常,皆可是血中脂膏堆积,过多的脂膏浊化。而成为湿浊、痰饮、侵淫隧道,使气血运行障碍,血瘀的本质具体表现在微循环障碍,血流的动力学异常和血液流变学异常。而痰浊与血瘀同是机体脏腑功能失调的病理产物,最终形成肝经痰凝瘀滞,肝内脂肪瘀积的病理变化,而出现“痰证”、“瘀证”、“脉痹”等证。
综合看来当属肝痞、胁痛、症瘕、积聚等范畴基于上述认识,遵循疏肝而降脂、利湿兼化痰、活血而化瘀之大法,采用疏肝降脂、化痰祛湿、活血散结、软坚消积之中药。

1.一种清热除湿、保肝利胆的佩兰口服液,其特征在于,所述口服液包括以下重量份数的原料药:佩兰10~50份,茵陈10~20份,茯苓10~20份,金钱草10~20份,陈皮10~20份,赤芍10~20份,贯众10~20份,玉米须10~20份,白茅根10~20份,桑寄生10~20份,黄芩10~20份,鸡骨草10~20份,川楝子10~20份,小米草10~20份,薏仁10~20份,垂盆草10~20份,半枝莲10~20份,车前草10~20份,马齿苋10~20份,板蓝根10~20份,女贞子10~20份,菟丝子10~20份,泽泻10~20份。

4.一种如权利要求1~3中任一项所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,所述口服液的制备步骤包括:
a.佩兰的制备萃取,作为组分1;
b.将所述其余原料药放入乙醇中加热回流提取2次,作为组分2;
c.将上述两种提取液组分1和组分2合并,浓缩后加蜂蜜或糊精调和,消毒杀菌后制备成口服液装瓶。
5.根据权利要求4所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,所述步骤a中,收获佩兰后,除去杂质,切段,干燥,置于有盖不锈钢桶内,加70%乙醇液,质量为粗粉的0.8-1倍,搅拌均匀,湿润密闭放置1小时以上,使充分膨胀;将渗漉筒底部滤板用纱布袋包裹铺平后再将湿润膨胀后的药物样品拌松弄散,然后用不锈钢勺盛粉,均匀的装入渗漉筒,装10-12厘米厚,用T型棒压匀,再按上述操作,一层一层的装入,适当加压,药粉填装不得超过渗漉筒的2/3高处;药粉面上盖不锈钢孔板压牢,打开渗漉筒下面的放料阀,并放一容器,然后缓缓加入70%乙醇液;待排出药粉粉粒之间的空气,并有乙醇流出约20L左右,关闭放料阀,盖上漉筒、浸渍24小时,然后开放料阀进行渗漉,控制渗漉速度一般为1000g药材每分钟流出2~3ml,滤液放入贮液缸内,并将排空时的乙醇液倒入贮液缸;将渗滤液合并静置,加热浓缩至膏状,静置备用,成组分1。
6.根据权利要求4所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,所述步骤b中,将所述其余原料泡入10倍量乙醇中,加热提取2次,每次1-2小时,去上清液,合并提取液,100-120目滤过,再经截流分子量为5000-10000的超滤柱超滤,超滤液减压浓缩相对密度为80℃时1.06的浸膏,加热浓缩至膏状,静置备用,成组分2。
7.根据权利要求4所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,所述步骤c中,将组分1和组分2合并后,置入双效真空浓缩器中,浓缩至90℃时相对密度为1.36的浓缩液,置0~5℃低温冷藏24小时;将冷藏液加0.3%的助滤剂硅藻土,过滤,滤液再置入双效真空浓缩器中,浓缩至每1ml含0.1g生药量;浓缩后的膏剂加蜂蜜或糊精调和,紫外线消毒杀菌后装瓶。
8.一种如权利要求1~3中任一项所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,制备步骤还可以为:
a.佩兰蒸馏汽化,得精油,作为组分1;
b.将蒸馏后残渣与所述其余原料药一起加水浸泡、提取2次作为组分2;
c.将组分2浓缩后加蜂蜜与组分1调和,紫外线消毒杀菌后装瓶。
9.根据权利要求8所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,所述步骤a中,收获佩兰后,除去杂质,洗净,稍润,切段,干燥,放入蒸馏器中汽化提取精油,静置备用,成组分1;所述步骤b中,将蒸馏提取的佩兰渣滓与所述其余原料一起放入占其质量5-10倍量水中,浸泡1-2小时,加热提取2次,每次1-2小时,去上清液,合并提取液,100-120目滤过,将2次提取液合并静置,减压并浓缩至药液浓度为0.6g生药/mL,抽滤后,滤液的相对密度约为20℃时1.08;减压至0.03-0.08MPa,温度保持在60-80℃,浓缩至相对密度为1.20,温度至60℃-70℃的浸膏,作为组分2。
10.根据权利要求8所述清热除湿、保肝利胆的佩兰口服液的制备方法,其特征在于,所述步骤c中,将组分2放入减压浓缩罐内,减压回收乙醇,浓缩至药液浓度为0.1g生药/mL,抽滤至滤液的相对密度为20℃时1.06;上述滤液经体积为10L的大孔吸附树脂柱吸附后,用10倍树脂柱体积的去离子水或蒸馏水洗脱,再用5倍树脂柱体积的95%乙醇洗脱,收集乙醇洗脱液,去除溶剂,调和组分1,再调和蜂蜜,紫外线消毒杀菌后装瓶。
