背景技术
席汉氏综合征也称垂体前叶机能减退症(西蒙-席汉综合征),是多种病因所致腺垂体激素分泌不足,继发性腺、甲状腺、肾上腺皮质功能低下所呈现的临床征群。病因: 垂体瘤,下丘脑病变,垂体缺血性坏死,蝶鞍区手术放疗和创伤,感染和炎症,糖皮质激素长期治疗,垂体卒中等。 由于产后大出血,尤其是伴有长时间的失血性休克,使垂体前叶组织缺氧、变性坏死,继而纤维化,最终导致垂体前叶功能减退的综合征,其发生率占产后出血及失血性休克患者的25%左右。近几年研究显示席汉综合征的发生,并非仅与垂体前叶功能减退有关,有报道部分患者垂体前叶功能有减退征象,其中50%显示垂体后叶功能亦有不同程度的异常。席汉综合征不仅可以发生于阴道分娩者,亦可发生于剖宫产术之后,在现代剖宫产率上升的今天,应引起产科医师的高度重视。
席汉氏综合征的治疗,根据甲状腺、肾上腺皮质、性腺等功能低下的具体情况,分别予以长期的激素替代疗法。1.一般治疗:加强营养,适当运动,补充维生素、钙剂,治疗贫血等。2.药物治疗:(1)肾上腺皮质激素 口服可的松或氢化可的松,有水肿者,改用泼尼松或地塞米松。当有感染、发热、创伤、手术时,剂量应适当增加。(2)甲状腺素片 一般在服用肾上腺皮质激素几天之后开始服用。(3)性激素 可采用人工周期疗法:中年以上者可以不用,青年患者口服己烯雌酚,最后5天加用黄体酮,停药3~7天后如月经来潮,可在出血后5天重复使用。有生育要求者,为促排卵可联合应用HMG或HCG,效果良好。上述西药治疗副作用较大,患者还可结合使用中医中药治疗,效果更好。

下面逐一分析席汉氏综合征及其治疗药物中各中药的药性功效。党参补中益气,生津,养血。主治脾胃虚弱,气血两亏,体倦无力,食少,口渴,久泻,脱肛。木瓜有促进腺体分泌的功效,特别是促进乳腺分泌奶液更是效果明显,民间常用食用作妇女产后催奶之药膳食品。枸杞子可调节机体免疫功能、能有效抑制肿瘤生长和细胞突变、具有延缓衰老、抗脂肪肝、调节血脂和血糖、促进造血功能等方面的作用,并应用于临床。红枣的功效:用于健脾益胃、脾胃虚弱,腹泻,倦怠无力等症,能补血益气、健脾胃,达到增加食欲、止泻的功效。菟丝子性温,味甘,入肝、肾经,能滋补肝肾,增强性欲,固精缩尿,安胎,明目,止泻,用于阳痿遗精、尿有余沥、遗尿尿频、腰膝酸软、目昏耳鸣、肾虚胎漏、胎动不安、脾肾虚泻;外治白癜风。桂圆肉也叫元肉,元肉性温味甘,益心脾、补气血,具有良好有滋养补益,补气血、益智宁心、安神定志的功效,可用于心脾虚损、气血不足。黄芪补气固表,托毒排脓,利尿,生肌,用于气虚乏力、久泻脱肛、自汗、水肿、子官脱垂、慢性肾炎蛋白尿、糖尿病、疮口久不愈合。杜仲补肝肾,强筋骨,安胎,治腰脊酸疼,足膝痿弱,小便余沥,阴下湿痒,胎漏欲堕,胎动不安,高血压。桑寄生用于补肝肾,强筋骨,祛风湿,安胎元,用于风湿痹痛,腰膝酸软,筋骨无力,崩漏经多,妊娠漏血,胎动不安,高血压。生地清热、凉血、养阴、生津。牛膝散瘀血,消痈肿,治淋病,尿血,经闭,癥瘕,难产,胞衣不下,产后瘀血腹痛,喉痹,痈肿,跌打损伤,熟用牛膝补肝肾,强筋骨,治腰膝骨痛,四肢拘挛,痿痹。海带富含多糖类成分藻胶酸和昆布素、甘露醇、无机盐,干品中20~35%是无机物、碘含量0.27~0.72%,海带软坚,行水,治瘰疬,瘿瘤,噎膈,水肿,睾丸肿痛,带下。 黄精主治:阴虚劳嗽,肺燥咳嗽,脾虚乏力,食少口干,消渴,肾亏腰膝酸软,阳痿遗精,耳鸣目暗,须发早白。甘草片为镇咳祛痰药品,作用颇为广泛,在中药汤剂及中成药配伍中,常用其补气益脾、和中缓急、调和诸药之功效。

席汉氏综合征及其治疗药物适合于一个席汉氏综合征患者服用的一个剂量药量是按下面各中药质量组合而成:党参15克、木瓜50克、枸杞子10克、红枣15克、菟丝子12克、元肉12克、黄芪15克、杜仲12克、桑寄生12克、生地15克、牛膝12克、海带15克、黄精10克、甘草7克。使用方法:煎汤服,日服一剂量,也可两日服用一剂量,服用一个剂量即可见效:小便次数减少,精神有所振作,身体各种机能增强。
