延胡索:性味:辛、苦,温。归肝、脾经。功能主治:活血,利气,止痛。用于胸胁、脘腹疼痛,经闭痛经,产后瘀阻,跌扑肿痛。《雷公炮炙论》:“治心痛欲死。”《日华子本草》:“除风,治气,暖腰膝,破症癖,扑损瘀血,落胎,及暴腰痛。”《开宝本草》:“主破血,产后诸病,因血所为者。妇人月经不调,腹中结块,崩中淋露,产后血运,暴血冲上,因损下血,或酒摩及煮服。”《医学启源》:“治脾胃气结滞不散,主虚劳冷泻,心腹痛,下气消食。”《纲目》:“活血,利气,止痛,通小便。”
丹参:性味:苦,微寒。归心、肝经。功能主治:祛瘀止痛,活血通经,清心除烦。用于月经不调,经闭痛经,症瘕积聚,胸腹刺痛,热痹疼痛,疮疡肿痛,心烦不眠;肝脾肿大,心绞痛。《本经》:“主心腹邪气,肠鸣幽幽如走水,寒热积聚;破症除瘕,止烦满,益气。”《吴普本草》:“治心腹痛。”《别录》:“养血,去心腹痼疾结气,腰脊强,脚痹;除风邪留热,久服利人。”《滇南本草》:“补心定志,安神宁心。治健忘怔冲,惊悸不寐。”《纲目》:“活血,通心包络。治疝痛。”《云南中草药选》:“活血散瘀,镇静止痛。治月经不调,痛经,风湿痹痛,子宫出血,吐血,乳腺炎,痈肿。”
红花:性味:辛,温。归心、肝经。功能主治:活血通经,散瘀止痛。用于经闭,痛经,恶露不行,症瘕痞块,跌扑损伤,疮疡肿痛。《唐本草》:“治口噤不语,血结,产后诸疾。”《开宝本草》:“主产后血运口噤,腹内恶血不尽、绞痛,胎死腹中,并酒煮服。亦主蛊毒下血。”《本草蒙筌》:“喉痹噎塞不通,捣汁咽。”《纲目》:“活血,润燥,止痛,散肿,通经。”
水蛭:性味:咸、苦,平;有小毒。归肝经。功能主治:破血,逐瘀,通经。用于症瘕痞块,血瘀经闭,跌扑损伤。《本经》:“主逐恶血、瘀血、月闭,破血瘕积聚,无子,利水道。”
甘草:性味:甘,平。归心、肺、脾、胃经。功能主治:补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药。用于脾胃虚弱,倦怠乏力,心悸气短,咳嗽痰多,脘腹、四肢挛急疼痛,痈肿疮毒,缓解药物毒性、烈性。《本经》:“主五脏六府寒热邪气,坚筋骨,长肌肉,倍力,金疮肿,解毒。”《别录》:"温中下气,烦满短气,伤脏咳嗽,止渴,通经脉,利血气,解百药毒。”《药性论》:“主腹中冷痛,治惊痫,除腹胀满;补益五脏;制诸药毒;养肾气内伤,令人阴(不)痿;主妇人血沥腰痛;虚而多热;加而用之。”《日华子本草》:“安魂定魄。补五劳七伤,一切虚损、惊悸、烦闷、健忘。通九窍,利百脉,益精养气,壮筋骨,解冷热。”
本发明中药药物组方机理如下:
君药:人参、党参、太子参、白术共为君药,其中人参功以大补元气,补脾益肺,生津,安神;党参功以补中益气,健脾益肺;太子参功以益气健脾;白术功以补气健脾;四药共奏益气健脾之效。
臣药:苍术、半夏、川芎、丹参、红花共为臣药,其中苍术功以燥湿健脾;半夏功以燥湿化痰;天南星功以燥湿化痰;竹沥功以清热豁痰;瓜蒌功以清热涤痰,宽胸散结;浙贝母功以清热化痰散结;川芎功以活血行气;延胡索功以活血,利气,止痛;丹参功以祛瘀止痛,活血通经,清心除烦。红花功以活血通经;共奏燥湿化痰、活血通经之效。
佐药:水蛭功以破血逐瘀痛经,加强活血化瘀功效;
甘草调和诸药为使药。
冠心病稳定型心绞痛合并高血压病及高脂血症在临床非常常见,中医学认为其与情志及饮食失调有关。现代社会关系复杂,生活压力大,长期恼怒、忧郁、精神紧张易造成肝之疏泄功能异常,气机郁滞,不能助心行血,而致心脉不畅,发为冠心病,因气机上逆,肝阳上亢,临床常可发现合并高血压病。另外,随着生活水平的提高,我国人民的膳食结构发生很大变化,肥甘厚腻之品在饮食中的比重不断增加,不健康的饮食生活方式等可导致脾胃受损,健运失司;而肝郁易乘脾土,也可使脾病健运失司。脾胃虚损一则使气血津液生化乏源,心失所养,同时中气亏虚则心气亦不足,气病则致血病,行血无力致脉道迟缓不畅,气虚不能自守,心下空虚而内动为悸。二则使痰湿内生,湿浊弥漫,上蒙胸阳而致胸阳不展,正如仲景《金匮要略》“阳微阴弦”之病机,胸闷、气短乃作。另外,血脂亦由脾运化水谷而生成,并依赖脾的转输功能布散周身,脾失健运则膏脂转输异常故临床检测可发现高脂血症。
综上所述,冠心病稳定型心绞痛患者多合并高血压及高脂血症,其病机与肝郁、脾虚、痰浊、血瘀有关。本研究采用本发明的治疗冠心病稳定型心绞痛合并高血压高脂血症的中药治疗冠心病稳定型心绞痛合并高血压病、高脂血症患者,方中柴胡、陈皮、青皮、佛手、香橼、薤白功以疏肝解郁、行气止痛;人参、党参、太子参、白术功以益气健脾。苍术、半夏、天南星、竹沥、瓜蒌、浙贝母功以燥湿化痰;川芎、延胡索、丹参、红花功以活血化瘀、通经止痛。水蛭功以破血逐瘀通络,加强活血化瘀功效;刺蒺藜功以平肝疏肝、罗布麻功以平抑肝阳,加强降血压作用;全方配伍合理,诸药合用,共奏疏肝解郁、益气健脾、祛湿化痰、活血化瘀之功效,与冠心病稳定型心绞痛合并高血压病及高脂血症的病机完全契合,经过临床试验验证,临床疗效显著。

下面结合具体实施例对本发明作更进一步的说明,以便本领域的技术人员更了解本发明,但并不因此限制本发明。
实施例1
一种治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,各原料药重量配比如下:
柴胡12g、陈皮10g、青皮10g、佛手10g、香橼10g、薤白10g、刺蒺藜9g、罗布麻9g、人参10g、党参15g、太子参15g、白术12g、苍术9g、半夏9g、天南星9g、竹沥20g、瓜蒌10g、浙贝母9g、川芎9g、延胡索9g、丹参9g、红花9g、水蛭3g、甘草6g。
实施例2
一种治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,各原料药重量配比如下:
柴胡10g、陈皮12g、青皮8g、佛手12g、香橼8g、薤白12g、刺蒺藜7g、罗布麻11g、人参8g、党参17g、太子参13g、白术14g、苍术7g、半夏11g、天南星7g、竹沥22g、瓜蒌8g、浙贝母11g、川芎7g、延胡索11g、丹参7g、红花11g、水蛭2g、甘草7g。
实施例3
一种治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,各原料药重量配比如下:
柴胡14g、陈皮8g、青皮12g、佛手8g、香橼12g、薤白8g、刺蒺藜11g、罗布麻7g、人参12g、党参13g、太子参17g、白术10g、苍术11g、半夏7g、天南星11g、竹沥18g、瓜蒌12g、浙贝母7g、川芎11g、延胡索7g、丹参11g、红花7g、水蛭4g、甘草5g。
实施例4
一种治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,各原料药重量配比如下:
柴胡8g、陈皮14g、青皮6g、佛手14g、香橼6g、薤白14g、刺蒺藜5g、罗布麻13g、人参6g、党参19g、太子参11g、白术16g、苍术5g、半夏13g、天南星5g、竹沥24g、瓜蒌6g、浙贝母13g、川芎5g、延胡索13g、丹参5g、红花13g、水蛭1g、甘草8g。
实施例5
一种治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,各原料药重量配比如下:
柴胡16g、陈皮6g、青皮14g、佛手6g、香橼14g、薤白6g、刺蒺藜13g、罗布麻5g、人参14g、党参11g、太子参19g、白术8g、苍术13g、半夏5g、天南星13g、竹沥16g、瓜蒌14g、浙贝母5g、川芎13g、延胡索5g、丹参13g、红花5g、水蛭5g、甘草4g。
对本发明的实施例l的中药进行了临床药效学试验,详情如下:
1.1 研究对象
选取2013年2月-2015年2月我院收治的冠心病稳定型心绞痛合并高血压、高脂血症患者。纳入标准:(1)西医诊断标准符合中华医学会心血管病分会制定的《慢性稳定型心绞痛诊断与治疗指南》;稳定型心绞痛CCS分级:Ⅰ~Ⅲ级;高血压病及高脂血症诊断符合《诊断学》中相关诊断标准;中医诊断符合《中药新药临床研究指导原则》中辩证属肝郁脾虚痰瘀阻络证者:临床主要表现为:胸痛、胸闷、胁肋胀痛、气短、疲倦乏力、纳呆便溏、心悸、自汗、口唇面色或指甲紫暗、苔白腻、脉涩或结、代、促;(2)30岁<年龄<70岁;(3)能完成运动平板试验者;(4)自愿加入本研究,并签订知情同意书者;排除标准:(1)入院前3个月内发生不稳定型心绞痛或心肌梗死或心血管手术者;(2)合并心脏其他病变、心力衰竭、需进行治疗的心律失常;(3)心电图示:完全性左束支传导阻滞、ST段压低>1.0mv、左心室肥厚、预计综合症或心室起搏心律;心脏彩超示左室射血分数≤40%;(4)合并严重肝肾功能不全、血液系统、内分泌系统疾病及恶性肿瘤者;(5)妊娠或哺乳期妇女;(6)精神疾病等依从性差不能配合治疗及观察者;共有100例患者入选,按照随机数字表法分为观察组和对照组各100例。两组患者的一般资料比较差异无统计学意义(P>0.05),具有可比性。
1.2 方 法
1.2.1 治疗方法
本发明的治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,组方如下:柴胡12g、陈皮10g、青皮10g、佛手10g、香橼10g、薤白10g、刺蒺藜9g、罗布麻9g、人参10g、党参15g、太子参15g、白术12g、苍术9g、半夏9g、天南星9g、竹沥20g(冲服)、瓜蒌10g、浙贝母9g、川芎9g、延胡索9g、丹参9g、红花9g、水蛭3g、甘草6g;水煎服,每日一剂,早晚分服。
两组均以2个月为一个疗程。
1.2.2 观察指标
疗效指标:
中医症候积分:主症:胸痛、胸闷,由轻到重分别为0、2、4、6分;次症:气胁肋胀痛、气短、疲倦乏力、纳呆便溏、心悸、自汗、口唇面色或指甲紫暗、苔白腻、脉涩或结、代、促,由轻到重分别为0、1、2、3分。
1.2.3 疗效评价标准:
参照《中药新药临床研究指导原则》中关于冠心病中医症候疗效评价标准:
治愈:治疗后临床症状体征明显改善,中医症候积分较治疗前改善>90%
显效:治疗后临床症状体征明显改善,中医症候积分较治疗前改善>70%;
有效:治疗后临床正传体征好转,中医症候积分较治疗改善30%~70%。
无效:治疗后临床正传体征无改善甚至加重,中医症候积分较治疗改善<30%。
治愈率%=治愈例数/总例数x100%;总有效率%=(治愈+显效+有效)例数/总例数x100%。
2.结果
2.1 冠心病稳定型心绞痛改善情况
治疗后患者的运动总时间、最大负荷量、诱发ST段下移0.1mv时间、诱发ST段下移0.1mv运动量、最大ST段下降值、达最大负荷量时心律、ST段下移持续时间及达目标心率时间均较治疗前明显改善,差异具有统计学意义(P<0.05);见表1。
治疗后患者在心绞痛发作次数、发作持续时间及硝酸甘油消耗量方面均较治疗前明显改善,差异具有统计学意义(P<0.05),见表2。
2.2 血压改善情况
治疗后患者的平均收缩压和舒张压均较治疗前明显降低,差异具有统计学意义(P<0.05),见表3。
2.3 血脂改善情况
治疗后患者的血脂指标较治疗前改善,差异具有统计学意义(P<0.05);见表4。
2.4中医症候疗效
100例冠心病稳定型心绞痛合并高血压、高脂血症患者治愈90例、显效8例、有效1例、无效1例,治愈率90.0%,总有效率分别为99.0% 。
2.5 生活质量改善情况
治疗后患者的各项生活质量评分均较治疗前明显改善,差异具有统计学意义(P<0.05),见表5。
Claims (3)
1.一种治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,由以下重量份的中药原料药制成:
柴胡8-16份、陈皮6-14份、青皮6-14份、佛手6-14份、香橼6-14份、薤白6-14份、刺蒺藜5-13份、罗布麻5-13份、人参6-14份、党参11-19份、太子参11-19份、白术8-16份、苍术5-13份、半夏5-13份、天南星5-13份、竹沥16-24份、瓜蒌6-14份、浙贝母5-13份、川芎5-13份、延胡索5-13份、丹参5-13份、红花5-13份、水蛭1-5份、甘草4-8份。
2.根据权利要求1所述的治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,其特征在于,由以下重量份的中药原料药制成:
柴胡10-14份、陈皮8-12份、青皮8-12份、佛手8-12份、香橼8-12份、薤白8-12份、刺蒺藜7-11份、罗布麻7-11份、人参8-12份、党参13-17份、太子参13-17份、白术10-14份、苍术7-11份、半夏7-11份、天南星7-11份、竹沥18-22份、瓜蒌8-12份、浙贝母7-11份、川芎7-11份、延胡索7-11份、丹参7-11份、红花7-11份、水蛭2-4份、甘草5-7份。
3.根据权利要求2所述的治疗冠心病稳定型心绞痛合并高血压高脂血症的中药,其特征在于,由以下重量份的中药原料药制成:
柴胡12份、陈皮10份、青皮10份、佛手10份、香橼10份、薤白10份、刺蒺藜9份、罗布麻9份、人参10份、党参15份、太子参15份、白术12份、苍术9份、半夏9份、天南星9份、竹沥20份、瓜蒌10份、浙贝母9份、川芎9份、延胡索9份、丹参9份、红花9份、水蛭3份、甘草6份。
