Description
技术领域
本发明属于中药领域,具体涉及一种治疗慢性心衰的中药制剂及其制备方法。
背景技术
慢性心力衰竭(CHF)是各种心脏疾病的严重或终末阶段的一种临床综合征。导致CHF的常见原因为冠心病、高血压病、糖尿病和各种心肌病等,其中冠心病引起的心衰占首位。
CHF的发病率和死亡率居高不下,与CHF的临床过程有密切的关系。2007中国《慢性心力衰竭诊断和治疗指南》和2005美国的心衰指南均指出CHF是一种进行性的病变,一旦起始,即使没有新的心肌损害,临床亦处于稳定阶段,仍可自身不断发展。因此,美国心衰编写委员会率先发展了心衰分类的新方法,将心衰按其严重程度分为A、B、C、D四个阶段。阶段A为“前心衰阶段”(Pre-Heart Failure),此阶段无心脏的结构或功能异常,也无心衰的症状和(或)体征。多见于心衰的高发危险人群(主要指高血压病、冠心病、糖尿病等),这一阶段的心衰是可以预防的。阶段B属“前临床心衰阶段”(Pre-Clinical HeartFailure),患者从无心衰的症状和(或)体征已发展成结构性心脏病,例如:左室肥厚、无症状瓣膜性心脏病、以往有MI(心肌梗死)史等。这一阶段相当于无症状性心衰,或NYHA心功能Ⅰ级。由于心衰是一种进行性的病变,心肌重构可自身不断地发展,因此,这一阶段病人的积极治疗极其重要,而治疗的关键是阻断或延缓心肌重构。阶段C为临床心衰阶段。患者已有基础的结构性心脏病,以往或目前有心衰的症状和(或)体征;或目前虽无心衰的症状和(或)体征,但以往曾因此治疗过。这一阶段包括NYHAⅡ、Ⅲ级和部分Ⅳ级心功能患者。阶段D为难治性终末期心衰阶段。患者有进行性结构性心脏病,虽经积极的内科治疗,休息时仍有症状,且需要特殊干预。
由此可见,针对心衰阶段B,在心肌梗死或左室肥厚患者的心衰症状出现之前早期采取治疗措施,可降低心衰的发病率和死亡率。
中医药在长期的治病过程中,积累了丰富的经验。中医传统文献中无“心衰”之名,根据其临床特征,涉及中医“喘证”、“水肿”、“心悸”、“怔忡”、“痰饮”、“心痹”等范畴。大多数研究认为,本病属本虚标实、虚实夹杂之证,以心气心阳虚衰为本,血脉瘀滞、水饮内停、痰浊不化为标。
近年来,心衰的发病率和病死率呈现不断上升的趋势,由此产生的医疗费用和社会负担也迅速增加,2005年美国CHF患者达到550万,直接或间接用于治疗CHF的费用高达279亿美元。我国对35~74岁城乡居民共15518人随机抽样调查的结果:心衰患病率为0.9%,按计算约有400万心衰患者,其中男性为0.7%,女性为1.0%。Framingham研究显示,CHF5年存活率,男性为25%,女性为38%,重症患者一般存活不到1年。据我国部分地区42家医院,在1980、1990、2000年3个全年段,对心衰住院病例共10714例所作的回顾性调查,病因中冠心病由1980年的36.8%上升至2000年的45.6%,居各种病因之首;与我国相邻的日本的一项研究,对8个城市的15所医院共3169名心衰患者进行了调查,发现冠
心病在心衰病因中也占首要地位(33.5%);Framingham研究发现,当年龄到40岁时,人群发生心衰的危险性明显增高(男性:21%,女性:20%),而且这种危险性与心肌缺血及血压水平密切相关。
治疗CHF的传统药物主要包括利尿剂、ACEI(血管紧张素转换酶抑制剂)类、血管扩张剂、强心剂、心肌营养剂、钙通道阻滞剂等,但并未从根本上提高患者的生活质量,也未能有效降低CHF的死亡率,临床应用中也存在诸多问题。随着对CHF发生和发展基本病理机制——心肌重构的认识,传统的强心、利尿、扩血管的常规治疗理念正在逐步淡化,从短期的药理学措施改善症状,逐渐转变为长期的、修复性的策略及对CHF发病机制的干预。
发明内容
根据以上现有技术的不足,本发明提供一种治疗慢性心衰的中药制剂及其制备方法,其疗效显著、安全可靠、无毒副作用,具有广阔的临床应用前景。
本发明所述的一种治疗慢性心衰的中药制剂,按照重量份数计,由以下中药原料配制而成:党参10~15份、丹参15~20份、三七10~15份、黄芪15~20份、红花5~8份、桂枝3~5份、鹿角5~8份、麦冬5~8份、五味子10~15份、白术2~3份、当归3~5份、赤芍3~5份、黄连3~5份、丹皮3~5份和菌陈1~2份。
其中,优选方案如下:
按照重量份数计,由以下中药原料配制而成:党参10份、丹参15份、三七10份、黄芪15份、红花5份、桂枝3份、鹿角5份、麦冬5份、五味子10份、白术2份、当归3份、赤芍3份、黄连3份、丹皮3份和菌陈1份。
也可以按照重量份数计,由以下中药原料配制而成:党参15份、丹参20份、三七15份、黄芪20份、红花8份、桂枝5份、鹿角8份、麦冬8份、五味子15份、白术3份、当归5份、赤芍5份、黄连5份、丹皮5份和菌陈2份。
本发明所述的治疗慢性心衰的中药制剂的制备方法,按照以下方法制备:按照配方称取中药原料并加入到容器中,加水漫过药面浸泡10~20分钟,再加水没过药面3~5厘米,用武火煎煮至沸腾再改用文火煎煮20~30分钟,取滤液;然后再加水没过药面3~5厘米,用武火煎煮至沸腾再改用文火煎煮15~20分钟,然后去渣取滤液,两次滤液合并即可。
使用时,每次200~300ml,早晚各一次。
药理分析:
党参:性平,味甘,具有补中益气、健脾益肺、抗癌、降压、抗缺氧、抗衰老之功效,还可以增强人体免疫力,提高超氧化物歧化酶的活性,增强消除自由基的能力,具有调节胃肠运动、抗溃疡、抑制胃酸分泌、降低胃蛋白酶活性的作用。体质虚弱,气血不足,面色萎黄,以及病后产后体虚者宜食;脾胃气虚,神疲倦怠,四肢乏力,食少便溏,慢性腹泻,肺气不足,咳嗽气促,气虚体弱,易于感冒;气虚血亏者宜食;慢性肾炎蛋白尿者宜食;慢性贫血,萎黄病,白血病,血小板减少性紫癜以及佝偻病患者宜食。
丹参:味苦,性微寒。根入药,含丹参酮,为强壮性通经剂,有祛瘀、生新、活血、调经等效用,为妇科要药,主治子宫出血,月经不调,血瘀,腹痛,经痛,经闭,庙痛。对治疗冠心病有良好效果。此外亦治神经性衰弱失眠,关节痛,贫血,乳腺炎,淋巴腺炎,关节炎,疮疖痛肿,丹毒,急慢性肝炎,肾孟肾炎,跌打损伤,晚期血吸虫病肝脾肿大,癫癎。
三七:甘微苦,温,无毒。入肝、胃,兼入心、大肠。止血,散瘀,消肿,定痛。治吐血,咳血,衄血,便血,血痢,崩漏,症瘕,产后血晕,恶露不下,跌扑瘀血,外伤出血,痈肿疼痛。
黄芪:补气固表,托毒排脓,利尿,生肌。用于气虚乏力、久泻脱肛、自汗、水肿、子官脱垂、慢性肾炎蛋白尿、糖尿病、疮口久不愈合。
红花:辛,温,无毒。入心、肝经。活血通经,去瘀止痛。治经闭,症瘕,难产,死胎,产后恶露不行、瘀血作痛,痈肿,跌扑损伤。
桂枝:性味辛、甘,温,入肺、心、膀胱经,是主治里寒常用的温里药,具有补元阳、通血脉、暖脾胃之功效,常与补肝肾药、补气血药配伍,治疗肾阳不足、命门火衰、肢冷脉微,与其它温里药配伍治疗脘腹冷痛、寒痹腰痛,具有明显的镇痛和抗癌防癌的作用。
鹿角:味咸,微温,无毒。入肝、肾经。行血,消肿,益肾。治疮疡肿毒,瘀血作痛,虚劳内伤,腰脊疼痛。
麦冬:甘,微苦,微寒。归心、肺、胃经。养阴生津,润肺清心。用于肺燥干咳,阴虚痨嗽,喉痹咽痛,津伤口渴,内热消渴,心烦失眠,肠燥便秘。
五味子:敛肺,滋肾,生津,收汗,涩精。治肺虚喘咳,口干作渴,自汗,盗汗,劳伤羸瘦,梦遗滑精,久泻久痢。五味子有扩血管作用。五味子素、五味子丙素、去氧五味子素等对于离体狗肠系膜动脉收缩均有抑制作用,能增加脉鼠离体心脏及麻醉狗冠脉血流量。用北五味子饲喂家兔能调节心肌细胞和心、肾小动脉的能量代谢,提高心肌代谢酶活性,改善心肌的营养和功能。
白术:味苦,甘,性温。归脾、胃经。健脾益气,燥湿利水,止汗,安胎。用于脾虚食少,腹胀泄泻,痰饮眩悸,水肿,自汗,胎动不安。土白术健脾,和胃,安胎。用于脾虚食少,泄泻便溏,胎动不安。
当归:味甘,温。归肝、心、脾经。补血;活血;调经止痛;润燥滑肠。主治血虚诸证:月经不调;经闭;痛经;症瘕结聚;崩漏;虚寒腹痛;痿痹;肌肤麻木;肠燥便难;赤痢后重;痈疽疮疡;跌扑损伤。
赤芍:苦,微寒。归肝经。清热凉血;活血祛瘀。主温毒发斑;吐血衄血;肠风下血;目赤肿痛;痈肿疮疡;闭经;痛经;崩带淋浊;瘀滞胁痛;疝瘕积聚;跌扑损伤。用于温毒发斑,吐血衄血,目赤肿痛,肝郁胁痛,经闭痛经,症瘕腹痛,跌扑损伤,痈肿疮疡。
黄连:泻火,燥湿,解毒,杀虫。治时行热毒,伤寒,热盛心烦,痞满呕逆,菌痢,热泻腹痛,肺结按,吐、衄、下血,消渴,疳积,蛔虫病,百日咳,咽喉肿痛,火眼,口疮,痈疽疮毒,湿疹,汤火烫伤。
丹皮:辛苦;凉;微寒。归心;肝;肾;肺经。清热;活血散瘀。治温热病热入血分;发斑;吐衄;热泪盈眶病菌后期热伏阴分发热;阴虚骨蒸潮热;血滞经闭;痛经;痈肿疮毒;跌扑伤痛;风湿热痹。
菌陈:味微苦;微辛;性微寒。归脾;胃;膀胱经。清热利湿,退黄;主治黄疸尿少,小便不利,湿疮瘙痒,传染性黄疸型肝炎。
本发明的优点在于所使用的中药符合中华人民共和国药典规定,药物安全可靠。诸药相和具有补益心气、活血化瘀、利水化痰等功效,治疗慢性心衰具有疗效确切、见效快、治愈率高、无副作用等特点。
疗效判断:
1、病例选择:
作为治疗慢性心衰的药物,选择慢性心衰患者100例进行临床观察,其中男52例,女性48例,年龄45-85岁,平均年龄55岁,病程1-10年,平均1.2年。
2、治疗用药:
治疗组患者服用本发明药物,每次200~300ml,早晚各一次,7天为一个疗程,根据病情连续服用3-4个疗程,病情严重者遵医嘱酌增用量。
3、疗效判断:
根据美国纽约心脏病学会心衰分级标准(NYHA分级),无心衰者BNP<80ng/L、心功能Ⅰ级者BNP 95~221ng/L、心功能Ⅱ级患者BNP 221~459ng/L、心功能Ⅲ级患者BNP 459~1006ng/L、心功能Ⅳ级患者BNP≥1006ng/L。目前,在国际上把心衰患者BNP的阳性阈值定为100ng/L,当BNP测值在正常值以下时,可以认为能够排除心衰。
显效:临床症状基本消失,试验室检查各相关指标接近正常水平,BNP<100ng/L。
有效:临床症状减轻,试验室检查各相关指标有所改善,NYHA分级向好转方向提升,体征有改善。
无效:临床症状无明显好转或加重。
4、治疗结果:
显效30例,有效62例,无效8例,总有效率为92%。
以下列举一个典型病例:
李某,女,71岁,山东青岛人,慢性心衰20余年,心率85次/分,血压145/85mmHg,心功能NYHAⅢ级,呼吸困难,左心射血分数37%,24h尿量约为620ml,全身水肿,体重78.6kg。治疗前24h呋塞米用量1600mg和安体舒通用量为180mg,血尿素氮27.7mmol/l,血肌酐458.7mmol/l,血电解质常出现紊乱。服用本发明制剂第一个疗程时,心率70次/分,心功能改善为Ⅱ级,左心射血分数48%,体重为70kg,日尿量约为1440ml,血压135/72mmHg,呼吸困难和水肿明显改善。随访至第二疗程,上述指标进一步改善。

实施例1:
一种治疗慢性心衰的中药制剂,按照重量份数计,由以下中药原料配制而成:党参10份、丹参15份、三七10份、黄芪15份、红花5份、桂枝3份、鹿角5份、麦冬5份、五味子10份、白术2份、当归3份、赤芍3份、黄连3份、丹皮3份和菌陈1份。
实施例2:

实施例1所述配方按照以下方法制备治疗慢性心衰的中药制剂:按照配方称取中药原料并加入到容器中,加水漫过药面浸泡10分钟,再加水没过药面3厘米,用武火煎煮至沸腾再改用文火煎煮20分钟,取滤液;然后再加水没过药面3厘米,用武火煎煮至沸腾再改用文火煎煮15分钟,然后去渣取滤液,两次滤液合并即可。

1.一种治疗慢性心衰的中药制剂,其特征在于按照重量份数计,由以下中药原料配制而成:党参10~15份、丹参15~20份、三七10~15份、黄芪15~20份、红花5~8份、桂枝3~5份、鹿角5~8份、麦冬5~8份、五味子10~15份、白术2~3份、当归3~5份、赤芍3~5份、黄连3~5份、丹皮3~5份和菌陈1~2份。
