Abstract
本发明公开了治疗阴虚燥热型糖尿病性视网膜病变的药物及其制备方法,药物中各种原料药为:黑面叶、紫草茸、梵天花根、海参、地蚕、柠条、紫梢花、桑葚子、黄精、女贞子、棒棒草、麦冬、石斛、杉叶藻、枸骨叶、紫珠、铁苋菜、樗根皮、阴香皮、蒲黄、翻白草、蚕沙、豨莶草、秦皮、黑果小檗、绣球防风、骨节草、半枝莲、黄柏、黄芩、黄芪、山药和甘草。本发明的药物具有滋阴清热,止血化瘀的功效,用于治疗阴虚燥热型糖尿病性视网膜病变;且制作工艺简便,毒副作用小且给药方便,药剂易于制造,成本低廉,本发明采用不同药性的中药材,进行了科学配伍,能达到理想的治疗效果,且安全,制备简单,能直达病灶,治愈时间短,治愈后不易复发。

技术领域
本发明涉及含有来源于植物、动物或矿物原料的医用配制品,特别涉及一种治疗糖尿病性视网膜病变的药物及其制备方法。
背景技术
糖尿病性视网膜病变(DR)是糖尿病性微血管病变中最重要的表现,是一种具有特异性改变的眼底病变,是糖尿病的严重并发证之一。临床上根据是否出现视网膜新生血管为标志,将没有视网膜新生血管形成的糖尿病性视网膜病变称为非增殖性糖尿病性视网膜病变(NPDR)(或称单纯型或背景型),而将有视网膜新生血管形成的糖尿病性视网膜病变称为增殖性糖尿病性视网膜病变(PDR)。
在经济发达的国家,糖尿病视网膜病变是一种主要的致盲眼病,一般说来,约有四分之一的糖尿病患者有糖尿病视网膜病变。糖尿病视网膜病变的发生和发展,不仅取决于代谢障碍的程度,并与糖尿病的发病年龄,病程长短、遗传因素和糖尿病控制情况有关。
病因
糖尿病患者主要是胰岛素激素及细胞代谢异常,引起眼组织、神经及血管微循环改变,造成眼的营养和视功能的损坏。微血管是指介于微小动脉和微小静脉之间,管腔小于100~150μm的微小血管及毛细血管网,是组织和血液进行物质交换的场所。由于糖尿病患者血液成分的改变,而引起血管内皮细胞功能异常,使血-视网膜屏障受损。视网膜毛细血管内皮细胞色素上皮细胞间的联合被破坏,造成小血管的渗漏。糖尿病患者微血管病变主要发生在视网膜及肾脏,是致盲、肾功能衰竭及死亡的主要原因。
临床表现
多数糖尿病视网膜病变患者除了有多饮、多尿、多食和疲乏、消瘦等全身代谢障碍的症状外,在糖尿病视网膜病变的初期,一般无眼部自觉症状,视力无影响。随着病变的发展,可引起不同程度的视力障碍。若黄斑区受到侵犯,可有视野中心暗影、中心视力下降、视物变形等表现。视网膜小血管破裂,少量出血进入玻璃体,患者可自觉眼前有黑影飘动。当新生血管大量出血到玻璃体腔,视力可严重丧失,仅存光感。黄斑区以外的血管闭塞,或增殖性视网膜病变导致视网膜脱离,则出现相应部位视野的缺损。
中医病因认为有三:①饮食不节。平日饮食不节,多食酒肉厚味,损伤脾胃,致使脾胃运化不力,并因此酿成内热。日久,使体内津液干涸,所以发生消谷引饮,水液偏渗于膀胱,因而出现多饮、多食与多尿的三多症状。②情志失调。喜、怒、思、忧、恐五志、情志失调,可以化火,内火旺则伤阴耗液致使津液干涸,从而发生消渴。③恣情纵欲,房室不节则伤肾,肾虚阴亏则火旺,所以消耗津液而出现三多症状。根据以上论点,提示糖尿病的发病原因,主要由于阴虚与燥热,而且二者往往互为因果。
本病的主要病机为气阴两虚、痰浊淤血、目络阻滞,目睛失养。故中医认为糖尿病视网膜病变有以下四种常见证型:
一、阴虚燥热证。主证:本病早期,视力减退,眼底所见,视网膜毛细血管静脉扩张,微血管瘤及出血灶,色鲜红。伴燥渴喜饮、消谷善饥,形体消瘦,咽干口燥,便干尿赤。舌红少苔,脉细数。病机分析:本病早期多见阴虚燥热之征,肺胃有热则烦渴喜饮、消谷善饥、便干尿赤,舌红脉数;热盛伤阴,饮津不足则咽干口燥,形体消瘦,苔少、脉细;阴虚火旺,上扰清窍,灼伤脉络,则导致眼底出血,视力下降。
二、气阴两虚证。主证:本病病变发展,视力明显减退,眼前黑影飞舞。眼底:视网膜静脉扩张,微血管瘤,出血斑,硬性渗出斑,絮状白斑,或兼水肿,新生血管,视网膜前出血,玻璃体出血。伴神疲乏力,少气懒言,咽干口燥,五心烦热,自汗盗汗,肢体麻木疼痛。舌红少津,舌体胖大有齿痕,苔薄或花剥。或舌质紫暗,或有瘀斑,或舌下静脉怒张,脉细数无力。病机分析:本病初起多阴虚燥热,燥热不仅伤阴,久则还伤气;阴津亏虚,气无所化,而致气阴两虚,血行无力,引起血瘀及出血。气虚则见神疲乏力,少气懒言,自汗,舌体胖大有齿痕;阴虚则见咽干口燥,五心烦热,盗汗,舌红少津;肢体麻木疼痛,舌质紫暗,或有瘀斑,或舌下静脉怒张,眼底:视网膜静脉扩张,微血管瘤,出血斑等,均为瘀血阻滞,目络瘀阻之征。
三、肾阴亏虚证。主证:病变迁延,自觉视物模糊,眼内干涩不适。眼底:可见微血管瘤、出血大部分吸收,渗出物减少。伴腰酸乏力,咽干口燥,夜尿多。舌红少苔,脉细数。病机分析:肾为先天之本、水火之脏。久病及肾,阴虚日久必致肾水亏虚,肾阴虚则见腰酸乏力,咽干口燥,舌红少苔;阴虚无以制火,虚火上炎,灼伤目络,导致眼底出血而影响视力。
四、阴阳两虚证。主证:本症病变进一步发展,自觉眼前黑影较著,视力显著下降,甚至失明。眼底:微血管瘤、出血,渗出物、新生血管,玻璃体出血,增殖机化条索物。伴咽干口燥,五心烦热,潮热盗汗,畏寒肢冷,面色苍白无华,腰腹怕冷,夜尿频多,阳萎早泄。舌质淡红,舌体胖嫩,边有齿痕,苔薄白,脉沉细无力。病机分析:肾为先天之本、元阴元阳之府,且阴阳互根、相生相成,阴虚日久,阴损及阳,导致阴阳两虚。肾阴虚则咽干口燥,五心烦热,潮热盗汗;肾阳虚则畏寒肢冷,面色苍白无华,腰腹怕冷,夜尿频多,阳萎早泄,舌质淡红,舌体胖嫩,边有齿痕,苔薄白,脉沉细无力。
西医对本病有多种治疗方法。①控制血糖:血糖控制情况与糖网病的进展和视力预后有很大的关系,控制血糖的药物很多,宜在专科医生指导下酌情选用;②激光光凝治疗:黄斑水肿者多采用氩激光作局部格栅光凝治疗;出现增生性改变,可作散在或全视网膜光凝治疗;如果患者屈光介质不清如有白内障,或/和玻璃体出血尚可看清眼底者则作氪红激光光凝治疗;③冷冻治疗:当糖网病患者因白内障,或/和玻璃体出血看不清眼底,不能作光凝治疗时,则可作视网膜冷冻治疗;④玻璃体切除术:玻璃体出血长期不吸收和/或有视网膜脱离则应考虑玻璃体切割术;⑤药物治疗:导升明,口服剂量视病情而定,单纯型每次500毫克,每日2次或3次,增生型每次500毫克,每日3次或4次;可连服3个月,重症者可服1~2年;活血素,每次2~4毫升,每日2次,饭前服用,可连服3个月,重症者可服1~2年;其他药物如口服阿司匹林,肌注安妥碘等,可促进出血吸收。
但是西医手术治疗成本高,病人不易接受,西药治疗毒副作用大,容易复发。然而中药在治疗糖尿病性视网膜病变方面具有辨证论治,针对性强,疗效显著,无耐药性等优势。
发明内容
本发明所要解决的技术问题在于,提供了一种具有滋阴清热,止血化瘀功能治疗阴虚燥热型糖尿病性视网膜病变的药物,治疗阴虚燥热型糖尿病性视网膜病变具有疗程短、效果较好、残留量低,绿色无污染等优点。

为了更好地实现发明目的,本发明还提供一种治疗阴虚燥热型糖尿病性视网膜病变的药物的制备方法,所述药物的剂型为冷敷剂,制备过程为:先将所述原料药粉碎成粗粉,再进一步粉碎,过120目筛,收集通过120目筛的细粉得细粉a;再将不能通过120目筛的粗粉投入多功能提取罐提取两次,第一次加4-6倍量水煎煮1-2小时,第二次加3-5倍量水煎煮1-2小时,合并两次煎液,过滤得滤液;将滤液加热浓缩至糊状,放入烘箱80℃烘干后冷却,研磨成细粉b,将细粉a与细粉b混合后,混匀加入适量甘油,甘油添加量为细粉混合物总质量的2-8倍,调成糊剂, 消毒后装入有色瓶中, 放入冰柜0℃-4℃贮存备用。
为了更好地实现发明目的,本发明还提供一种治疗阴虚燥热型糖尿病性视网膜病变的药物的制备方法,药物的剂型为滴眼剂,制备过程为:
a、取所述原料药加入水提取2次,第一次加4-6倍量水煎煮1-2小时,第二次加3-5倍量水煎煮1-2小时,将2次提取液合并,滤过,减压浓缩得80℃时相对密度为1.04~1.10的浸膏1,药渣备用;
b、将药渣加60-90%乙醇浸泡0.5-1.5小时,提取两次,每次1-2小时,合并提取液,滤过,浓缩,80-160目滤过,6000-10000转/分钟离心,取上清液,经截流分子量为5000-10000的超滤柱超滤,超滤液减压浓缩相对密度为60℃时1.25的浸膏2;
c、将浸膏1和浸膏2混合,加入混合浸膏总质量的0.06%的氯化钠和60-80倍的蒸馏水,并加入浓度为4%氢氧化钠溶液调节PH为6.0-7.0,再加入0.2%活性炭煮沸10分钟,滤过,分装,灭菌,即得滴眼剂。
为了更好地实现发明目的,本发明还提供一种治疗阴虚燥热型糖尿病性视网膜病变的药物的制备方法,当药物的剂型为颗粒剂时,其制备方法包括以下步骤:
步骤一:将所述原料药材按所述比例混合,加相对于混合物4~8倍的醇浓度为85%~95%的乙醇,加热至沸腾回流3~5小时,过滤,采用渗漉法以每分钟1~2ml的速度缓缓渗漉,收集渗漉液,随后在真空度0.06~0.09Mpa下减压浓缩至50~60℃时相对密度为1.04~1.10的膏体,喷雾干燥,喷雾干燥机的进风温度160~175℃、出风温度80~85℃,随后粉碎成粉末,制成干膏粉;
步骤二:在获得的干膏粉中加入相对于干膏粉质量0.2~0.4倍的蔗糖粉和0.1~0.2倍的糊精,制成颗粒,于40~50℃干燥,获得颗粒剂。
本发明还提供治疗阴虚燥热型糖尿病性视网膜病变的药物,药物的剂型为:片剂、糖衣片剂、薄膜衣片剂、肠溶衣片剂、胶囊剂、硬胶囊剂、软胶囊剂、口服液、滴眼剂、口含剂、颗粒剂、冲剂、蜜丸剂、丹剂、溶液剂、注射剂、冷敷剂、栓剂、硬膏剂、糖浆剂、散剂或喷雾剂。
本发明的药物具有滋阴清热,止血化瘀的功效,用于治疗阴虚燥热型糖尿病性视网膜病变;且制作工艺简便,毒副作用小且给药方便,药剂易于制造,成本低廉,本发明采用不同药性的中药材,进行了科学配伍,能达到理想的治疗效果,且安全,制备简单,能直达病灶,治愈时间短,治愈后不易复发。
具体实施方式
中医病因认为有三:①饮食不节。平日饮食不节,多食酒肉厚味,损伤脾胃,致使脾胃运化不力,并因此酿成内热。日久,使体内津液干涸,所以发生消谷引饮,水液偏渗于膀胱,因而出现多饮、多食与多尿的三多症状。②情志失调。喜、怒、思、忧、恐五志、情志失调,可以化火,内火旺则伤阴耗液致使津液干涸,从而发生消渴。③恣情纵欲,房室不节则伤肾,肾虚阴亏则火旺,所以消耗津液而出现三多症状。根据以上论点,提示糖尿病的发病原因,主要由于阴虚与燥热,而且二者往往互为因果。
因此本发明治疗阴虚燥热型糖尿病性视网膜病变以滋阴清热,止血化瘀为主。故选用中药黑面叶、紫草茸、梵天花根、海参、地蚕、柠条、紫梢花、桑葚子、黄精、女贞子、棒棒草、麦冬、石斛、杉叶藻、枸骨叶、紫珠、铁苋菜、樗根皮、阴香皮、蒲黄、翻白草、蚕沙、豨莶草、秦皮、黑果小檗、绣球防风、骨节草、半枝莲、黄柏、黄芩、黄芪、山药和甘草。方中黑面叶、紫草茸、梵天花根活血化瘀;海参、地蚕、柠条、紫梢花、桑葚子、黄精、女贞子补血滋阴,益精填髓;棒棒草、麦冬、石斛、杉叶藻、枸骨叶养阴生津;紫珠、铁苋菜、樗根皮收敛止血;阴香皮、蒲黄、翻白草止血;蚕沙、豨莶草、秦皮、黑果小檗清热,用于眼疾;绣球防风、骨节草明目退翳;半枝莲化瘀止血;黄柏、黄芩清热燥湿;黄芪补气健脾,利尿消肿,托毒生肌;山药健脾益气;甘草又调和诸药。诸药合用,共凑滋阴清热,止血化瘀之功。
其中,上述原料药材的药理如下:
黑面叶:微苦,凉。有小毒。清热解毒,散瘀,止痛,止痒。根:急性胃肠炎,扁桃体炎,支气管炎,尿路结石,产后子宫收缩疼痛,风湿性关节炎。叶:外用治烧烫伤,湿疹,过敏性皮炎,皮肤搔痒,阴道炎。
紫草茸:胶蚧科昆虫紫胶虫在树枝上所分泌的树脂状胶质。苦,寒。消毒,止血,活血化瘀。用于血痨热,肿毒恶疮,瘀血不化。清热,凉血,解毒。用于斑疹不透,疮疡肿毒。
梵天花根:为锦葵科植物梵天花的根。甘苦,温。入脾、胃、心、肾四经。健脾去湿,化瘀活血。治风涅关节炎,劳伤脚弱,水肿,疟疾,痛经,白带,跌打损伤.痈疽肿毒。
海参:咸,温。入心、肾二经。补肾益精,养血淘燥。治精血亏损,虚弱劳怯,阳痿,梦遗,小便频数,肠燥便艰。
地蚕:唇形科水苏属植物地蚕以块茎入药。甘,平。归肺;肾经。益肾润肺,滋阴补血,清热除烦。用于肺结核咳嗽,肺虚气喘,吐血,盗汗,贫血,小儿疳积。
柠条:豆科柠条以根、花、种子入药。本发明取全草和花:甘,温。归肝;肾经。滋补养血。
紫梢花:淡水海绵科动物脆弱骨针淡水海绵或刻盘淡水海绵,以干燥群体入药。甘,温。补肾,益精,助阳。用于阳痿遗精,小便失禁,白带。
桑葚子:甘、平,无毒。补虚益气,去水气。桑叶降血压,清肝明目。嫩桑枝去风湿,治臂痛。桑根白皮利尿、止咳、治浮肿。桑椹中的脂肪酸具有分解脂肪、降低血脂,防止血管硬化等作用。桑椹含有乌发素,能使头发变的黑而亮泽。桑椹有改善皮肤血液供应,营养肌肤,使皮肤白嫩及乌发等作用,并能延缓衰老。桑椹具有免疫促进作用,可以防癌抗癌。桑椹善滋阴养血、生津润燥,适于肝肾阴血不足及津亏消渴,肠燥等症。常食桑椹可以明目,缓解眼睛疲劳干涩的症状。
黄精:味甘,性平,入肺、脾、肾经,具有养气养阴,健脾润肺,益肾填精的功效,用于脾胃气阴亏虚,食少无力,阴虚肺燥,干咳少痰,消渴多饮,脾胃虚弱,脾气虚或脾阴不足,阴虚内热之消渴,肾虚精亏、阳痿遗精、头晕耳鸣、目昏眼花等证。
女贞子:本品为木犀科植物女贞的干燥成熟果实。甘、苦,凉。归肝、肾经。滋补肝肾,明目乌发。用于眩晕耳鸣,腰膝酸软,须发早白,目暗不明。
棒棒草:为石竹科植物短小孩儿参的干燥块根。甘;苦;微温。补肺;健脾;养阴生津。主肺虚咳嗽;脾虚食少;心悸自汗;疲惫乏力;口干
麦冬:性甘、微苦,微寒;归心、肺、胃经。具有养阴生津,润肺清心的功效,可用于喉痹咽痛,津伤口渴,内热消渴,心烦失眠,肠燥便秘等。清养肺胃之阴多去心用,滋阴清心多连心用。
石斛:性微寒,味甘,归胃、肾经,具有益胃生津,滋阴清热的功效。主治阴伤津亏,口干烦渴,食少干呕,病后虚热,目暗不明等证。石斛能显著提高超氧化物歧华酶(SOD)水平,降低过氧化脂质(LPO),调节脑单胺类神经介质水平,抑制类似单胺氧化酶(MAO),起到延缓衰老的作用。石斛含有酯类成分,具有活血化瘀、扩张血管及抗血小板凝结,治疗血栓闭塞脉搏管炎,脑血栓形成,动脉硬化性闭塞等作用。石斛中提取的菲类和联苄类物质,有抗肺癌、卵巢腺癌和前髓细胞白血病的作用。
杉叶藻:微甘、苦,凉。镇咳,舒肝,凉血止血,养阴生津,透骨蒸。主治高热烦渴,结核咳嗽,痨热骨蒸,两胁疼痛,肠胃炎等。茎、叶外用治外伤出血。
枸骨叶:本品为冬青科植物枸骨的干燥叶。苦,凉。归肝、肾经。清热养阴,平肝,益肾。用于肺痨咯血,骨蒸潮热,头晕目眩;高血压。
紫珠:为马鞭草科植物杜虹花的叶。辛、苦,平 。归肝、脾、胃经。散瘀止血,消肿止痛。叶:用于吐血,咯血,衄血,便血;外用治疗外伤出血。根:跌打肿痛,风湿骨痛。《本草拾遗》:“解诸毒物,痈疽,喉痹,毒肿,下痿,蛇虺虫螫,狂犬毒,并煮汁服;亦煮汁洗疮肿,除血长肤。”《中国药植图鉴》:“对食道静脉出血,肠胃溃疡出血,鼻出血,创伤伤出,肺出血以及拔牙出血均有良效。”紫珠草有促进血液凝固的作用,对肠道血管有直接收缩作用和间接通过肠管挛缩压迫堵阻血管而起止血作用。紫珠草对大肠杆菌、福氏痢疾杆菌、金黄色葡萄球菌、溶血性链球菌等均有不同程度的抑制作用。
铁苋菜:苦、涩,凉。清热解毒,利湿,收敛止血。《黑龙江中药》:“用于月经不调,妇女血病。”用于肠炎、痢疾、吐血、衄血、便血、尿血、崩漏、痈疖疮疡、皮肤湿疹。铁苋菜中富含蛋白质、脂肪。糖类及多种维生素和矿物质,其所含的蛋白质比牛奶更能充分被人体吸收,所含胡萝卜素比茄果类高2倍以上,可为人体提供丰富的营养物质,有利于强身健体,提高机体的免疫力,有“长寿菜”之称。
樗根皮:苦、涩,寒;归大肠、胃、肝经。椿皮的别称。《药性论》:“治赤白痢,肠滑,痔疾泻血不注。”《本草拾遗》:“主赤白久痢,疳虫,去疥,主下血。”清热燥湿,收涩止带,止泻,止血。用于赤白带下,湿热泻痢,久泻久痢,便血,崩漏。
阴香皮:阴香皮:为樟科植物阴香的树皮。辛,温。归脾经。温中止痛;祛风散寒;解毒消肿;止血。主主治寒性胃痛;腹痛泄泻;食欲不振;风寒湿痹;腰腿疼痛;跌打损伤;创伤出血;疮疖肿毒。
蒲黄:味甘、微辛、性平;入肝、心、脾经。功用止血,祛瘀,利尿,主治吐血咯血衄血,备痢,便血、崩漏,外伤出血,心腹疼痛,经闭腹痛、产后瘀痛、痛经,跌扑肿痛,血淋涩痛、带下、重舌,口疮、聤耳、阴下湿痒。
翻白草:甘、微苦、平,无毒。《草木便方》:"清利肠胃,除风湿。治赤白久痢成疳,涂恶犬咬伤。"《本草纲目》:"治吐血,下血,崩中,疟疾,痈疮。"清热,解毒,止痢止血。可治妇女赤白带和月经过多症。
蚕沙:为蚕蛾科动物家蚕蛾幼虫的干燥粪便。味辛、甘,性温。《本草纲目》:“治消渴,症结,及妇人血崩,头风,风赤眼,去风除湿。”《本草拾遗》:“炒黄,袋盛浸酒,去风缓诸节不遂,皮肤顽麻痹,腹内宿冷,冷血瘀血,腰脚疼冷。”祛风燥湿、清热活血。治风湿、皮肤不仁,关节不遂,急剧吐泻转筋,筋骨不遂、腰脚痛、腹内瘀血、头风赤眼。
豨莶草:辛、苦,寒。归肝、肾经。祛风湿,利关节,解毒。用于风湿痹痛,筋骨无力,腰膝酸软,四肢麻痹,半身不遂,风疹湿疮。
秦皮:味苦,寒;归肝经、胆经、大肠经。《本草汇言》:“秦皮,味苦性涩而坚,能收敛走散之精气。《本草纲目》:“梣皮,色清气寒,味苦性涩,乃是厥阴肝、少阳胆经药也。故治目病、惊痫,取其平木也;治下痢、崩带,取其收涩也;又能治男子少精、益精有子,皆取其涩而有补也。”《神农本草经》:“除热,目中青翳白膜。”功用清热燥湿,清肝明目,收涩止痢,止带。用于热毒泻痢,带下阴痒,肝热目赤肿痛,目生翳障。
黑果小檗:为小檗科植物黑果小檗的根皮和茎皮。苦;性寒。归肝经。清湿热;泻火解毒。主湿热痢疾;泄泻;目赤肿痛;咽喉肿痛;口疮;湿疹。
绣球防风:唇形科植物绣球防风以根及全草入药。苦、辛,温。破血通经,明目退翳,解毒消肿。治妇女血瘀经闭,小儿雀目,青盲翳障,痈疽肿毒。祛风散寒,健胃消食。根:风寒感冒,风湿关节痛,食欲不振,小儿疳积;全草:外用治疮疡肿毒。
骨节草:为木贼科植物犬问荆的全草。性平,味甘微苦。归肝、肺、胆三经。疏风,明目,活血,舒筋。治迎风流泪,翳膜遮睛,跌打损伤,肠风,血痔。
半枝莲:味辛,苦;性寒。归肺经,肝经,肾经。《泉州本草》:“清热,解毒,法风,散血,行气,利水,通络,破瘀,止痛。内服主血淋,吐血,衄血;外用冶毒蛇咬伤,痈疽,疔疮,无名肿毒。”功用清热解毒;散瘀止血;利尿消肿。可用于热毒痈肿;咽喉疼痛;肺痈;肠痈;瘰疬;毒蛇咬伤;跌打损伤;吐血;衄血;血淋;水肿;腹水及癌症。
黄柏:黄柏:苦,寒。归肾、膀胱经。清热燥湿,泻火除蒸,解毒疗疮。用于湿热泻痢,黄疸,带下,热淋,脚气,骨蒸劳热,盗汗,遗精,疮疡肿毒,湿疹瘙痒。盐黄柏滋阴降火。用于阴虚火旺,盗汗骨蒸。
黄芩:性苦,寒。归肺、胆、脾、大肠、小肠经。《本草汇言》:“清肌退热,柴胡最佳,然无黄芩不能凉肌达表。上焦之火,山桅可降,然舍黄芩不能上清头目。所以方脉科以之清肌退热,疮疡科以之解毒生肌,光明科以之散热明目,妇女科以之安胎理经,此盖诸科半表半里之首剂也。”可清热燥湿,泻火解毒,止血,安胎,降血压。用于湿温、暑温胸闷呕恶,湿热痞满,泻痢,黄疸,肺热咳嗽,高热烦渴,血热吐衄,痈肿疮毒,胎动不安。黄芩有解热、镇静、降血压、利尿、降低毛细血管通透性、抑制肠管蠕动等作用,对葡萄球菌、血溶性链球菌、肺炎双球菌、伤寒杆菌、痢疾球菌、新城疫、绿脓杆菌具有一定的抑制作用,对上呼吸道感染、慢性支气管炎、菌痢等有较好的疗效。
黄芪:甘,微温;归肺、脾、肝、肾经。《本草备要》:“黄芪,生用固表,无汗能发,有汗能止;炙用补中,益元气,壮脾胃。生血,生肌,排脓内托,疮痈圣药。痘症不起,阳虚无热者宜之。”《本草正义》:“黄芪,补益中土,温养脾胃,凡中气不振,脾土虚弱,清气下陷者最宜。其皮直达人之肤表肌肉,固护卫阳,充实表分,是其专长,所以表虚诸病,最为神剂。”功用固表敛汗,利水消肿,托疮排脓,通调水道。
山药:味甘,性平,入肺、脾、肾经,具有健脾补肺、益胃补肾、固肾益精、聪耳明目、助五脏、强筋骨、长志安神、延年益寿的功效,主治脾胃虚弱、倦怠无力、食欲不振、久泄久痢、肺气虚燥、痰喘咳嗽、肾气亏耗、腰膝酸软、下肢痿弱、消渴尿频、遗精早泄、带下白浊、皮肤赤肿、肥胖等证。
甘草:性味甘,平。入脾、胃、肺经。《本经》曰:“主五脏六府寒热邪气,坚筋骨,长肌肉,倍力,金疮肿,解毒”。《药性论》曰:“主腹中冷痛,治惊痫,除腹胀满;补益五脏;制诸药毒;养肾气内伤,令痿;主妇人血沥腰痛;虚而多热;加而用之”。可益气补中,缓急止痛,润肺止咳,泻火解毒,调和诸药。主倦怠食少,肌瘦面黄,心悸气短,腹痛便溏,四肢挛急疼痛,脏躁,咳嗽气喘,咽喉肿痛,痈疮肿痛,小儿胎毒,及药物、食物中毒。用于脾胃虚弱,倦怠乏力,心悸气短,咳嗽痰多,脘腹、四肢挛急疼痛,痈肿疮毒,缓解药物毒性、烈性。
以下采用实施例来详细说明本发明的实施方式,借此对本发明如何应用技术手段来解决技术问题,并达成技术效果的实现过程能充分理解并据以实施。
实施例1 冷敷剂
本发明冷敷剂的制备过程为:先将黑面叶22g、紫草茸26g、梵天花根11g、海参37g、地蚕20g、柠条20g、紫梢花16g、桑葚子22g、黄精24g、女贞子29g、棒棒草18g、麦冬20g、石斛25g、杉叶藻20g、枸骨叶15g、紫珠14g、铁苋菜17g、樗根皮15g、阴香皮12g、蒲黄17g、翻白草12g、蚕沙11g、豨莶草12g、秦皮13g、黑果小檗21g、绣球防风26g、骨节草17g、半枝莲8g、黄柏34g、黄芩32g、黄芪36g、山药23g和甘草14g粉碎成粗粉,再进一步粉碎,过120目筛,收集通过120目筛的细粉得细粉a;再将不能通过120目筛的粗粉投入多功能提取罐提取两次,第一次加5倍量水煎煮1-2小时,第二次加4倍量水煎煮1小时,合并两次煎液,过滤得滤液;将滤液加热浓缩至糊状,放入烘箱80℃烘干后冷却,研磨成细粉b,将细粉a与细粉b混合后,混匀加入适量甘油,甘油添加量为细粉混合物总质量的6倍,调成糊剂, 消毒后装入有色瓶中, 放入冰柜0℃-4℃贮存备用。
实施例2 滴眼剂
本发明滴眼剂的制备过程为:
a、取黑面叶26g、紫草茸25g、梵天花根9g、海参34g、地蚕17g、柠条18g、紫梢花21g、桑葚子17g、黄精21g、女贞子33g、棒棒草15g、麦冬17g、石斛28g、杉叶藻18g、枸骨叶12g、紫珠15g、铁苋菜12g、樗根皮17g、阴香皮16g、蒲黄14g、翻白草17g、蚕沙16g、豨莶草15g、秦皮15g、黑果小檗19g、绣球防风32g、骨节草15g、半枝莲10g、黄柏33g、黄芩35g、黄芪32g、山药25g和甘草11g,加入水提取2次,第一次加5倍量水煎煮1小时,第二次加4倍量水煎煮1小时,将2次提取液合并,滤过,减压浓缩得80℃时相对密度为1.10的浸膏1,药渣备用;
b、将药渣加90%乙醇浸泡1小时,提取两次,每次1小时,合并提取液,滤过,浓缩,100目滤过, 10000转/分钟离心,取上清液,经截流分子量为10000的超滤柱超滤,超滤液减压浓缩相对密度为60℃时1.25的浸膏2;
c、将浸膏1和浸膏2混合,加入混合浸膏总质量的0.06%的氯化钠和80倍的蒸馏水,并加入浓度为4%氢氧化钠溶液调节PH为6.0-7.0,再加入0.2%活性炭煮沸10分钟,滤过,分装,灭菌,即得滴眼剂。
实施例3颗粒剂
取黑面叶25g、紫草茸23g、梵天花根13g、海参32g、地蚕22g、柠条14g、紫梢花19g、桑葚子12g、黄精25g、女贞子27g、棒棒草12g、麦冬11g、石斛30g、杉叶藻12g、枸骨叶17g、紫珠13g、铁苋菜18g、樗根皮13g、阴香皮19g、蒲黄11g、翻白草11g、蚕沙14g、豨莶草18g、秦皮18g、黑果小檗16g、绣球防风29g、骨节草13g、半枝莲12g、黄柏31g、黄芩36g、黄芪34g、山药27g和甘草10g。
其制备方法包括以下步骤:
步骤一:将所述原料药材按所述比例混合,加相对于混合物5倍的醇浓度为95%的乙醇,加热至沸腾回流4小时,过滤,采用渗漉法以每分钟2ml的速度缓缓渗漉,收集渗漉液,随后在真空度0.07Mpa下减压浓缩至60℃时相对密度为1.06的膏体,喷雾干燥,喷雾干燥机的进风温度175℃、出风温度85℃,随后粉碎成粉末,制成干膏粉;
步骤二:在获得的干膏粉中加入相对于干膏粉质量0.3倍的蔗糖粉和0.1倍的糊精,制成颗粒,于50℃干燥,获得颗粒剂。
毒性试验:
1、急性毒性试验报告
试验方法:以本发明实施例2的滴眼剂为对象,选择20只小白鼠进行试验,分为对照组和试验组,各10只,取临床日用量的3倍给试验组的10只小白鼠静脉注射,观察7日。结果20只白鼠均健存,其活动、饮食、毛发、排泄物未发现异常,未见毒性反应。毒性试验证明,该药未有毒性作用和不良反应,临床应用安全可靠。
2、急性毒性试验报告
急性毒性试验:应用NIH小鼠60只,SPF级,雌雄各半,体重17~24g,进行急性毒性试验。小鼠随机分为两组,每组30只,即对照组和给药组,实验前禁食12小时;将本发明的实施例3制备的颗粒剂溶解在水中,(浓度为5.74g生药/ml,最高浓度)灌胃,灌胃容积为5ml/kg(即单次给药剂量为28.7生药/kg),对照组给予等量生理盐水,一天给药2次,给药间隔时间6小时,给药后连续观察14天,并记录小鼠的的毒性反应及死亡数。实验结果表明:与对照组比较,给药后小鼠未见明显差异,实验连续观察14天,小鼠全身状况、饮食、饮水、体重增长均正常。小鼠口服灌胃本发明的颗粒剂LD50>28.7生药/kg,每日最大给药量为57.4生药/kg/日。本发明的中药临床用药量为4.6g生药/日/人,成人体重以60KG计,平均用药剂量为0.077g生药/kg/日。按体重计:小鼠(平均体重以21g计)口服灌胃本发明的中药的耐受量为临床用量的745倍。因此本发明实施例3的颗粒剂急性毒性低,临床用药安全。
3、过敏试验报告
试验方法:以本发明实施例1的冷敷剂作为对象,选择30只生命体征相同的豚鼠进行试验。随机分为5组,一组对照组和四组试验组,分别对试验组的24只豚鼠进行皮下用药,分别用药量为2LD50、4 LD50、6 LD50、8 LD5(LD50约为1.5g/kg),用药进行4周。观察四组试验组和一组对照组的豚鼠,皮肤均未出现任何不适,且行为正常,血液内脏未见病变。
临床资料
病例选择:选择2009年1月-2011年3月期间,门诊接收的阴虚燥热型糖尿病性视网膜病变患者共计180例。其中,180例患者中,男75例,女105例,年龄38-72岁,平均60岁,Ⅰ期156 眼、Ⅱ期122 眼、Ⅲ期80眼。将该180例患者随机分为四组,四组间年龄、病程、分期等构成比无显著性差别(P>0.05),具有可比性。
冷敷剂治疗组:45人。总眼数89眼,使用本发明实施例1制备的冷敷剂,每日3次,将眼部洗净之后,每次将5g的冷敷剂敷于眼部进行20分钟洗去,1个月为一个疗程。
滴眼剂治疗组:45人。总眼数89眼,使用本发明实施例2制备的滴眼剂,每日3次,将浓度为2%的滴眼剂对准眼角进行滴药,每次滴药2-3滴,1个月为一个疗程。
颗粒剂治疗组:45人。总眼数90眼,服用本发明实施例3制备的颗粒剂,每日3次,每次0.8g的剂量,1个月为一个疗程。
西药对照组:45人。总眼数90眼,胰激肽原酶,口服,一次120-240单位,空腹服用;羟苯磺酸钙,每次500毫升,每日2次;阿司匹林,每天服用25~40毫克;1个月为一个疗程。
诊断标准:
1、患者有糖尿病史,血糖尿糖检查高于正常;
2、病例全部用新福林散瞳做详细的眼底检查, 参考《实用眼科学》制订诊断标准;早期无明显视觉症状,随着病程的进展,可引起不同程度的视力下降和视功能障碍。眼底检查:视网膜微血管瘤和点状出血;视网膜深层可有境界清楚,白色或黄色点状硬性渗出斑。眼底荧光血管造影:可见荧光不能灌注的毛细血管闭塞区,多位于后极部,有荧光素渗漏。
3、参照第三届全国眼科学术会议制订的糖尿病性视网膜病变分型分期标准:
分期标准:Ⅰ期:有微动脉瘤或并有小出血点;Ⅱ期:有黄白色“硬性渗出”或并出血;Ⅲ期有白色“软性渗出”或并有出血斑;Ⅳ期:眼底有新生血管或并有玻璃体出血;Ⅴ期:眼底有新生血管和纤维增生;Ⅵ期:眼底有新生血管和纤维增生,并发视网膜脱离。其中,前3期为非增生期,后3期为增生期。
中医诊断标准:早期,视力减退,眼底所见,视网膜毛细血管静脉扩张,微血管瘤及出血灶,色鲜红。伴燥渴喜饮、消谷善饥,便干尿赤,形体消瘦,咽干口燥,便干尿赤。舌红少苔,脉细数。中医辨证为阴虚燥热型。
疗效标准
采用国标准视力表,
显效: 视力提高5行或视力达5.0以上, 眼底出血完全吸收, 渗出与微动脉瘤减少, 观察3年无复发。
有效: 视力提高3~ 4行, 眼底出血部分吸收, 玻璃体混浊减轻, 观察2年内又有新出血。
无效: 视力无提高, 眼底无改善。
治疗效果
参见表1和表2,从表1可以看出,采用本发明的药物治疗阴虚燥热型糖尿病性视网膜病变,相对于西药在治疗效果上,具有显著的改进;从表2可以看出,采用本发明的药物治疗阴虚燥热型糖尿病性视网膜病变,相对于西药,在治疗疗程上显著缩短。

根据上述表格内的临床统计可知,本发明提供的中药具有起效快、有效率高、疗效确切、安全性高、无毒副作用的优势。
典型病例
患者女,58 岁,患糖尿病9年,曾长期服用降糖灵,2011年1月起自觉双眼视力下降,眼前黑影遮挡,4d 后视力急减,仅能分辨眼前指数,伴燥渴喜饮、消谷善饥,形体消瘦,咽干口燥,便干尿赤。舌红少苔,脉细数。确诊为“糖尿病视网膜病变”,中医辨证为阴虚燥热型。检查视力右眼0.1,左眼0.1,右眼玻璃体混浊,机化条索牵拉视网膜,左眼玻璃体轻度混浊,双眼视网膜可见微血管瘤,中心窝反光消失。使用本发明实施例2的滴眼剂,将浓度为2%的滴眼剂对准眼角进行滴药,每次滴药3滴。1个月后视力逐渐提高,出血及渗出机化物部分吸收,血糖下降,视力右眼0.3,左眼0.6,2个月后视力右眼0.5,左眼1.0,双眼玻璃体内混浊吸收,右眼机化条索变淡、变薄,微循环检查可见,微血管瘀血减轻,红血球聚积减少。甲皱微血管出血吸收,瘀血减经,红血球聚积减少,空腹血糖6.3 mmol/L,餐后2h血糖7.1mmol/L出院,三年后,随访无复发。

7.一种根据权利要求1-6任一所述的治疗阴虚燥热型糖尿病性视网膜病变的药物的制备方法,其特征在于,所述药物的剂型为冷敷剂,所述制备过程为:先将所述原料药粉碎成粗粉,再进一步粉碎,过120目筛,收集通过120目筛的细粉得细粉a;再将不能通过120目筛的粗粉投入多功能提取罐提取两次,第一次加4-6倍量水煎煮1-2小时,第二次加3-5倍量水煎煮1-2小时,合并两次煎液,过滤得滤液;将滤液加热浓缩至糊状,放入烘箱80℃烘干后冷却,研磨成细粉b,将细粉a与细粉b混合后,混匀加入适量甘油,甘油添加量为细粉混合物总质量的2-8倍,调成糊剂, 消毒后装入有色瓶中, 放入冰柜0℃-4℃贮存备用。
8.一种根据权利要求1-6任一项所述的治疗阴虚燥热型糖尿病性视网膜病变的药物的制备方法,其特征在于,所述药物的剂型为滴眼剂,所述制备过程为:
a、取所述原料药加入水提取2次,第一次加4-6倍量水煎煮1-2小时,第二次加3-5倍量水煎煮1-2小时,将2次提取液合并,滤过,减压浓缩得80℃时相对密度为1.04~1.10的浸膏1,药渣备用;
b、将药渣加60-90%乙醇浸泡0.5-1.5小时,提取两次,每次1-2小时,合并提取液,滤过,浓缩,80-160目滤过,6000-10000转/分钟离心,取上清液,经截流分子量为5000-10000的超滤柱超滤,超滤液减压浓缩相对密度为60℃时1.25的浸膏2;
c、将浸膏1和浸膏2混合,加入混合浸膏总质量的0.06%的氯化钠和60-80倍的蒸馏水,并加入浓度为4%氢氧化钠溶液调节PH为6.0-7.0,再加入0.2%活性炭煮沸10分钟,滤过,分装,灭菌,即得滴眼剂。
9.根据权利要求1-6任一项所述的治疗阴虚燥热型糖尿病性视网膜病变的药物的制备方法,其特征在于,所述药物的剂型为颗粒剂时,其制备方法包括以下步骤:
步骤一:将所述原料药材按所述比例混合,加相对于混合物4~8倍的醇浓度为85%~95%的乙醇,加热至沸腾回流3~5小时,过滤,采用渗漉法以每分钟1~2ml的速度缓缓渗漉,收集渗漉液,随后在真空度0.06~0.09Mpa下减压浓缩至50~60℃时相对密度为1.04~1.10的膏体,喷雾干燥,喷雾干燥机的进风温度160~175℃、出风温度80~85℃,随后粉碎成粉末,制成干膏粉;
步骤二:在获得的干膏粉中加入相对于干膏粉质量0.2~0.4倍的蔗糖粉和0.1~0.2倍的糊精,制成颗粒,于40~50℃干燥,获得颗粒剂。
10.一种如权利要求1-6中任一项所述的治疗阴虚燥热型糖尿病性视网膜病变的药物,其特征在于,所述药物的剂型为:片剂、糖衣片剂、薄膜衣片剂、肠溶衣片剂、胶囊剂、硬胶囊剂、软胶囊剂、口服液、滴眼剂、口含剂、颗粒剂、冲剂、蜜丸剂、丹剂、溶液剂、注射剂、冷敷剂、栓剂、硬膏剂、糖浆剂、散剂或喷雾剂。
