技术领域
本发明属于中医药领域,涉及一种中药药物组方,该组方具有治疗乳腺增生及早期乳腺癌的功效。
背景技术
乳腺增生是女性最常见的乳房疾病,其发病率占乳腺疾病的首位。近些年来该病发病率呈逐年上升的趋势,年龄也越来越低龄化。它是既非炎症又非肿瘤的一类病,多发于30~50岁女性,发病高峰为35~40岁。乳腺增生症是正常乳腺小叶生理性增生与复旧不全,乳腺正常结构出现紊乱,属于病理性增生。目前该疾病真正的发病原因还不明确,多认为与内分泌失调及精神、环境因素等有关。
乳腺癌是女性最常见的恶性肿瘤之一,发病率占全身各种恶性肿瘤的7~10%,在妇女仅次于子宫癌,已成为威胁妇女健康的主要病因。
乳腺增生病与乳癌在临床表现上都以肿块为主,在发病机制上有相似之处,而且某引起类型的乳腺增生病可以发展成乳癌。
中医学记载此类疾病为“乳癖”范畴之内,分析认为由内伤七情,心理压力过度、生活方式的改变,使得肝气不舒,气滞痰凝、郁结。病理性的乳腺增生,需积极治疗,尤其是囊性增生类型,由于存在癌变的可能,不能掉以轻心。如果发生癌变,切除正常器官,给患者本人、家庭造成不可挽回和无法弥补的心理创伤,产生了巨大的精神压力。
临床上治疗乳腺增生及乳腺癌预防治疗,常用的药物多数是中成药,具有活血化瘀、疏肝理气、软坚散结、调补气血等作用,由于选用药材不同,病症各异,治疗效果差异较大。此外,尚有激素疗法,有人采用雄激素治疗本病,但这种治疗有可能加剧人体激素间失衡,不宜常规应用。仅在症状严重,影响正常工作和生活时,才考虑采用。由于乳腺增生及乳腺癌的病型较多,普通的治疗药物,需要明确针对病型,因此对于诊断的要求极高,误诊也是临床常见情况。
本发明中药组方,根据“乳癖”的辩治分析,立足于祖传秘方,加上自己多年反复的临川实践,认为本病致病原因除思虑伤脾,脾虚水湿不运,聚而成核,或恼怒伤肝,肝失达,气郁为患之外,多与冲任脉络不调有关。治疗此病,多主张肝理气、活血止痛、软坚散结、化痰通络、健脾补胃、排毒荡邪,调理任冲二脉,攻补兼施方可取效。本发明克服了一般药物制剂的弱点,综合了各种药物的优劣,充分考虑了选用药物的药性及配位习性。最终选药以疏肝理气、活血止痛、软坚散结、化痰通络、健脾补胃、排毒荡邪为主,对各种病发乳腺增生及乳腺癌都有疗效,治疗手段让患者树立战胜病痛的坚定信心,专心配合治疗,可达到满意的效果。
技术内容
本发明的目的是提供一种具有疏肝理气、活血止痛、软坚散结、化痰通络、健脾补胃、排毒荡邪功能的中药组方,该组方可用于乳腺增生及乳腺癌的治疗。
本发明的中药组方由如下重量份的中药组成:
柴胡、香附、赤芍、白芍、白术、女贞子、旱莲草、橘核、橘叶、党参8~12份;
生山楂、淫羊藿、菟丝子、紫灵芝、白灵芝、还阳草、阴阳草、郁金13~16份;
血藤、首乌藤、黄皮果、回生草、丹参20~25份。
本发明的中药组方的组成通常按如下重量份制用:
柴胡10份,香附10份,赤芍10份,白芍10份,生山楂15份,党参12份,白术10份,淫羊藿15份,菟丝子15份,女贞子10份,旱莲草10份,血藤25份,橘叶10份,首乌藤20份,橘核10份,紫灵芝15份,白灵芝15份,黄皮果20份,还阳草15份,回生草20份,阴阳草15份,郁金14份,丹参22份。
组方方解:乳络为肝胃所主,因肝郁胃阻,而乳络不畅,日久则痰聚血凝成癖,发为乳腺增生,肝郁气血失畅,累及冲任,故月经不调,女性乳房属于生殖系统,故冲任失调又反之累及乳络,故乳病症状随月经而变化,综上所述,乳癖之病为肝气郁结,冲任失调,而致气滞痰凝血瘀于乳络,结成乳癖,于是取柴胡、香附、赤芍、白芍、菟丝子、橘核、橘叶、女贞子、黄皮果、生山楂、白术、党参疏肝理气和胃,党参、白术、首乌藤、血藤、橘核、橘叶、回生草化痰软坚散结,菟丝子,淫羊藿,女贞子,旱莲草阴阳双补调冲任,柴胡,白芍一疏一敛调气机,使肝气畅达而气血流通,阴阳互济而冲任调和,加上化痰祛瘀,软坚散结而标本兼顾,最终乳癖得消。
本发明中药组方选用药材的药理活性基础为:
柴胡,1.和解少阳:用于外感发热,或邪入半表半里的寒热往来及疟疾寒热等,如(<伤寒论>小柴胡汤)、(柴葛解肌汤)。2.疏肝解郁:用于肝郁气滞,胁肋胀满疼痛,及肝郁血虚,月经不调等,如(<景岳全书>柴胡疏肝散)。
香附,《别录》:味甘,微寒,无毒。理气解郁;调经止痛;安胎。主胁肋胀痛;乳房胀育;疝气疼痛;月经不调;脘腹痞满疼痛;嗳气吞酸,呕恶;经行腹痛;崩漏带下;胎动不安。用于肝郁气滞,胸、胁、脘腹胀痛,消化不良,胸脘痞闷,寒疝腹痛,乳房胀痛,月经不调,经闭痛经。
赤芍,味苦,性微寒。归肝经。清热凉血;活血祛瘀。主温毒发斑;吐血衄血;肠风下血;目赤肿痛;痈肿疮疡;闭经;痛经;崩带淋浊;瘀滞胁痛;疝瘕积聚;跌扑损伤。用于温毒发斑,吐血衄血,目赤肿痛,肝郁胁痛,经闭痛经,症瘕腹痛,跌扑损伤,痈肿疮疡。
白芍,味苦、酸;性微寒。归肝、脾经。具有养肝柔肝、缓急止痛功效。用于头痛眩晕、胸肋疼痛、泻痢腹痛、自汗盗汗、四肢挛急、月经不调、崩漏、通经。
生山楂,山楂具有降血脂、血压、强心、抗心律不齐等作用,同时,山楂也是健脾开胃、消食化滞、活血化痰的良药。对胸膈脾满、疝气、血淤、闭经等症有很好的疗效。山楂内的黄酮类化合物牡荆素,是一种抗癌作用较强的药物,山楂提取物对癌细胞体内生长、增殖和浸润转移均有一定的抑制作用。
党参,具有补中益气,健脾益肺之功效。现代药学研究证实,党参含多种糖类、酚类、甾醇、挥发油、黄芩素葡萄糖甙、皂甙及微量生物碱,具有增强免疫力、扩张血管、降压、改善微循环、增强造血功能等作用。此外对化疗放疗引起的白细胞下降有提升作用。
白术,健脾益气;健胃,燥湿利尿;化痰,止汗;安胎。现代医学研究白术含有的化学成分具有利尿、降血糖、抗凝血预防血栓、抗肿瘤、促进造血功能等作用。白术也是药膳的中药佐料。
淫羊藿,味辛、甘,性温。归肝、肾经。补肾壮阳,祛风除湿。治阳痿不举,偏风不遂,小便淋沥,筋骨挛急,咳嗽,病后青盲,虚火牙痛,半身不遂,腰膝无力,风湿痹痛,四肢不仁。《本经》:主阴痿绝伤,茎中痛。利小便,益气力,强志。《别录》:坚筋骨。消瘰疬、赤痈;下部有疮,洗,出虫。《日华子本草》:治一切冷风劳气,补腰膝,强心力,丈夫绝阳不起,女子绝阴无子,筋骨挛急,四肢不任,老人昏耄,中年健忘。
菟丝子,性温,味甘。入肝、肾经。补肾益精,养肝明目,安胎。用于肝肾亏虚,腰膝酸痛,阳痿遗精,尿频遗尿,两目昏暗,胎动不安等症。
女贞子,味甘、苦,性平。归肝、肾经。补益药。补益肝肾,清虚热,强腰膝,明耳目,乌须发。药(毒)理学证实,女贞子对动物毒性很小,兔1次服新鲜成熟果实75g,无中毒现象。
旱莲草,味甘、酸,性凉。入肝、肾二经,无毒。《日华子本草》中也说,墨旱莲可以排脓,止血,通小肠,敷一切疮并蚕瘑。在临床上,墨旱莲常与女贞子同用,旱莲草和女贞子都有保肝、解毒、降低转氨酶的作用,可以治疗肝肾阴虚,失眠心烦,耳鸣头晕,腰膝酸软等病证。
血藤,性凉,味苦,无毒。入肝、大肠二经。败毒消痈,活血通络,祛风杀虫的作用。治急、慢性阑尾炎,风湿痹痛,赤痢,血淋,月经不调,疳积,虫痛,跌扑损伤。
橘叶,味苦、辛,性平。归肺经。疏肝,行气,化痰,消肿毒。治胁痛,乳痈,肺痈,咳嗽,胸膈痞满,疝气。用于咳嗽痰多、咽喉肿痛、肺痈吐脓、胸满胁痛、痢疾腹痛、口舌生疮、目赤肿痛、小便癃闭。用于咳嗽痰多,胸闷不畅,咽痛,音哑,肺痈吐脓,疮疡脓成不溃。《本草经疏》:橘叶,古今方书不载,能散阳明、厥阴经滞气,妇人妒乳、内外吹、乳岩、乳痈,用之皆效,以诸证皆二经所生之病也。《本草汇言》:橘叶,疏肝、散逆气、定胁痛之药也。按丹溪言。此药共味苦涩,其气辛香,共性温散。凡病血结气结,痰涎火逆,病为胁痛,为乳痈,为肿毒,为胸膈逆气等疾,或捣汁饮,或取渣敷贴,无不应手获效。
首乌藤,味甘、微苦,性平。归心、肝经。具有能养心安神、祛风通络的功效。
橘核,味苦,性平。归肝、肾经。理气,止痛。治疝气,睾丸肿痛,乳痈,腰痛,膀胱气痛。《纲目》:治小肠疝气及阴核肿痛。《本草备要》:行肝气,消肿散毒。由于橘核有散结、止痛的功效,临床常用来治疗睾丸肿痛、乳腺炎性肿痛等症。治疗急性乳腺炎:将橘子仁碾成细末,以25%酒精或一般甜酒、白酒(适当稀释)调湿,均匀铺于纱布上,敷于炎症处。干燥后即须更换。严重病例可另用橘子仁1两,加白酒或甜酒1两,水200毫升,文火煎至100毫升,每天3次,每次口服20毫升。
紫灵芝,又名中国灵芝、仙草。性平,无毒,为上上品。生长于洁净的山坡上,比较珍惜,全国产量有限。主要用于调养糖尿病、高血压、冠心病、失眠等症,长期服用能抗肿瘤、保肝护肝、延缓衰老、扶正固本,对人体有益无害,适合长期保健用。自身免疫功能的低下或失调,是肿瘤之所以会发生并扩展的重要原因。紫灵芝片是最佳的免疫功能调节和激活剂,它可显著提高机体的免疫功能,增强患者自身的抗癌能力。紫灵芝可以通过促进白细胞介素-2的生成,通过促进单核巨噬细胞的吞噬功能、通过提升人体的造血能力尤其是白细胞的指标水平,以及通过其中某些有效成分对癌细胞的抑制作用,成为抗肿瘤、防癌以及癌症辅助治疗的优选药物。紫灵芝对人体几乎没有任何毒副作用。这种无毒性的免疫活化剂的优点,恰恰是许多肿瘤化疗药物和其它免疫促进剂都不具有的。
白灵芝,又名苦白蹄,采自长白山区,菌肉洁白,味奇苦,为灵芝罕见品种。对心血管疾病有特效,祛风除湿,止血、化痰,抗癌。改善睡眠,排毒、美容、提高免疫力及滋补强身等。治疗胃癌、乳腺癌、肺癌等多种癌症。
黄皮果,黄皮果还有较高的药用价值,叶、果和种子等都可入药,具有消食健胃、理气健脾、行气止痛等功效。
还阳草,《陕西草药》:苦涩,凉。清热解毒,散瘀止血。治喉炎,咳嗽,泄泻,痢疾,跌打,创伤。《陕西草药》:收涩,消肿。治痢疾,泄泻,喉炎,跌打损伤,红肿疼痛。
回生草,味辛、涩,性平。入肝经。散瘀止痛、止血生肌、清热息风。用于跌打损伤、筋断骨折、瘀血肿痛、经闭等症。用于胃肠止血、尿血、子宫出血、外伤出血、溃疡久不愈等症。用于小儿高热、小儿急惊风、慢惊风、破伤风等症。
阴阳草,全草有解毒、消炎、清热止泻、利尿之效,可治赤目肿痛、肠炎腹泻、痢疾、肝炎、小儿疳积、肾炎水肿、尿路感染等。
郁金,味辛、苦,性寒。归肝、心、肺经。功效为活血止痛,行气解郁,清心凉血,利胆退黄。用于经闭痛经,胸腹胀痛、刺痛,热病神昏,癫痫发狂,黄疸尿赤。
丹参,味苦,性微寒。归入心、肝经。具有活血散淤、消肿止血、消炎止痛、调经止痛、扩张冠状动脉、改善心肌缺血状况、降低血压、安神静心、降血糖和抗菌等功效。
本发明中药组方,不是简单的功效累加,具有充分的药理学配伍基础及实际临床用药经验,形成具有确切功效的临床验方:
疏肝理气:柴胡、香附、赤芍、白芍、菟丝子、橘核、橘叶、女贞子。
活血止痛:丹参、郁金、淫羊藿、还阳草、回生草。
软坚散结:橘核、橘叶、回生草。
化痰通络:党参、白术、首乌藤、血藤。
健脾补胃:黄皮果、生山楂、白术、党参。
排毒荡邪:柴胡、赤芍、阴阳草、紫灵芝、白灵芝、旱莲草。
本发明治疗乳腺增生及早期乳腺癌的中药组方,优势在于:1、治疗窗口宽,通过系统调节,对于各型乳腺增生及早期乳腺癌均有较好效果。2、对于发病较为严重的乳腺增生,通常治疗效果更为明显。对于早期乳腺癌也有明确的治疗效果。对中重度乳腺癌的治疗,因临床病例较少,尚未做深入研究。
本发明的治疗乳腺增生及早期乳腺癌的中药组方,可以根据需要制成不同的剂型进行使用,给药方式优选口服制剂,其次是局部给药制剂如酊剂、散剂、贴剂、膏剂等。。

动物药理实验
1材料与方法
1.1材料
1.1.1药物和试剂实验药物,按本发明研制;苯甲酸雌二醇注射液,天津金耀氨基酸有限公司,110316;黄体酮注射液,上海通用药业股份有限公司,110725;特茉芬,Rottendorf PharmaGmbH公司,100523。
1.1.2动物,SD大鼠,60只,雌性,体重180~220g,由佳木斯医学实验动物中心提供。
1.1.3仪器,游标卡尺,黑龙江顺腾尺业有限公司;电子天平,北京卡尔多仪器有限公司。
1.2方法取SD雌性未孕大鼠60只,随机分为3组,分别为空白对照组,实验药物组及参比药物组。除空白对照组外,其余各组大鼠按0.5g/kg皮下注射苯甲酸雌二醇,1次/d,连续3周,随后皮下注射黄体酮5mg/kg,1次/d,连续注射1周。空白对照组每天皮下注射生理盐水0.2m l/只,连续注射4周。各给药组按剂量灌胃给药,空自对照组和给予等体积蒸馏水,1次/d,连续8周。末次给药后1h,大鼠麻醉,腹腔静脉取血3ml作雌二醇、孕酮、睾酮测定;用精密游标卡尺测量每只大鼠胸部第2、3对乳房乳头高度;以6mm打孔器取下第2对完整乳房,电子天平称重。
2结果
2.1本发明实验药物对乳腺增生大鼠乳头高度的影响。

由上表1可见,实验组按用药8周后,大鼠第2、3对乳头高度与空白组相比均明显减小(P<0.01)。

由上表2可见,试验药物用药8周可显著减小乳腺增生大鼠第二对乳房重量。且效果优于参比药物组。
临床疗效
1.样本信息
治疗病例均为女性门诊患者。年龄最小者28岁,年龄最大者64岁。患者多数通过医院检查,确诊为乳腺增生或早期乳腺癌病症。
2.治疗方法
选用本发明中药,按比例调配后,于砂锅中加水煎煮40分钟,冷却饮用,每日一次,每服药煎煮2次。6天一个疗程。
3.治疗结果
67例,经2~6个疗程治疗,痊愈35例,显效17例,有效12例,无效3例,总有效率95.5%。对于早期发现的癌症症状,一般5~7个疗程就能痊愈。个别严重者需要更长时间的预防与治疗。
4.典型病例
(1)、王某某:女,38岁,黑龙江省佳木斯市莲江口镇人,2006年检查发现患乳腺增生,在当地人民医院治疗,打针吃药有3年的时间了,一直不见好转,后经人推荐,来我处治疗,根据诊断情况,安排服用本发明中药方,乳块肿痛的症状逐渐好转,次月又服用了四个疗程后,肿块完全消失了,三年回访没有复发。
(2)、李某,女,42岁,双鸭山市太平镇人,在辽宁大连打工,生过孩子后,留下了产后月子病和乳腺增生,在大连附属医院就诊后不见好转,病情时好时坏,在家也不能干重活。来我处就诊时,已经比较严重,服用此配方第五个疗程后,乳块症状全部消失。
(3)、张某,女,53岁,佳木斯市创业乡人,2007年在工厂上班参加体检时,发现乳房有肿块,检查确诊是早期乳腺癌,医院医生说是早期发现,吃药就可以消下去,可在医院吃药有三个月的时间,未有改善,反而症状更严重,乳房变色发紫,疼痛难忍。后经亲戚介绍来我处时,乳腺癌已经开始恶化,服用此配方二个疗程后,病情得到控制,服用五个疗程后,乳房色正常,也不疼痛了,坚持治疗8个疗程,基本恢复正常,后经过3个疗程巩固,至今未反复。
(4)刘某某,女31岁。乳房结块1年病人一年前发现右侧乳房有一结块,核桃大小,每在月经前胀痛,并且结块发硬,月经过后则变软,疼痛减轻,月经先后无定期,在医院被诊断为乳腺增生,乳房结块如鸡蛋大小,在医院服药治疗半年无效,求医于我处。诊断发现病人情志抑郁而肝气不舒,日久则痰聚血凝成癖,发为乳腺增生,肝郁气血失畅,累及冲任,故月经不调,女性乳房属于生殖系统,故冲任失调又反之累及乳络,故乳病症状随月经而变化,综上所述,此病为肝气郁结,冲任失调,而致气滞痰凝血瘀于乳络,结成乳癖。遂用本发明药方,服药5剂后,症状大减,结块变软,继服15剂,痊愈。
具体实施例
将选用的各种组方中药清洗干净,烘干。称取柴胡10克,香附10克,赤芍10克,白芍10克,生山楂15克,党参12克,白术10克,淫羊藿15克,菟丝子15克,女贞子10克,旱莲草10克,血藤25克,橘叶10克,首乌藤20克,橘核10克,紫灵芝15克,白灵芝15克,黄皮果20克,还阳草15克,回生草20克,阴阳草15克,郁金14克,丹参22克,置于砂锅中,加入1.6kg水,先用武火煮沸,后转成文火,持续煎煮40分钟。冷却,滤去药渣,服用。每日一次,每服药煎煮2次,6天一个疗程。
Claims (2)
1.本发明的目的是提供一种治疗乳腺增生及早期乳腺癌的中药组方,该中药组方由如下中药材按比例组成:
柴胡、香附、赤芍、白芍、白术、女贞子、旱莲草、橘核、橘叶、党参8~12份;
生山楂、淫羊藿、菟丝子、紫灵芝、白灵芝、还阳草、阴阳草、郁金13~16份;
血藤、首乌藤、黄皮果、回生草、丹参20~25份。
2.如权利要求1所述治疗乳腺增生及早期乳腺癌的中药组方,其常用重量配比为:
柴胡10份,香附10份,赤芍10份,白芍10份,生山楂15份,党参12份,白术10份,淫羊藿15份,菟丝子15份,女贞子10份,旱莲草10份,血藤25份,橘叶10份,首乌藤20份,橘核10份,紫灵芝15份,白灵芝15份,黄皮果20份,还阳草15份,回生草20份,阴阳草15份,郁金14份,丹参22份。
