Abstract

平口、侧孔气管导管,用于临床麻醉、急救和重症监护病房内病人的气管插管。其前端为圆形平口,带有一侧孔、一个套囊,内径是5-10mm。与其配套使用的导芯最前端是一半球,直径是3-5mm;随后是前圆柱,长为30-50mm,半径等于前端半球的半径;随后是一圆台,前截面的半径等于前圆柱的半径,后截面的半径等于后圆柱的半径,长是10-30mm;随后是后圆柱,直径是5-9mm,长是50-70cm。气管插管时,先依次让导芯的前端半球、前圆柱、圆台、后圆柱通过声门进入气管,再将平口、侧孔气管导管沿着导芯插入气管内即可。本平口、侧孔气管导管和导芯可使插管操作医生的视野免受气管导管的阻挡,将显著提高困难气管插管的成功率,避免气管导管对喉的损伤。

平口、侧孔气管导管和导芯
一、所属技术领域
本发明涉及一种用于临床麻醉、急救和重症监护病房内病人的气管导管,前端为平口,内径为5-10mm,带有一个侧孔和套囊;引导导管进入气管内的是一导芯,导芯最前端是一个半球,半径为3-5mm;随后是前圆柱,长为30-50mm,半径等于前端半球的半径;随后是一段圆台,圆台的前截面的半径等于前圆柱的半径,后截面的半径等于后圆柱的半径,长是10-30mm;随后是后圆柱,直径是5-9mm,长是50-70cm。
二、背景技术
目前,在国内外临床麻醉中广泛采用的气管导管的前端是斜面,带有侧孔,前端斜面是为了适应声门的“1”字形裂隙,使导管更容易通过声门。由于气管导管前端的斜面比较大,往往比较容易受阻于声门、会厌和喉,使得插管困难。反复暴力插管很容易使斜面损伤喉部,导致喉头水肿、通气困难直至病人窒息。这是气管插管困难时病人死亡的最常见原因。在困难气管插管时,无论是使用纤维支气管镜辅助插管还是使用插管探条辅助插管,当纤维支气管镜或探条被成功插入气管后,经常出现的情形是送管困难。原因是当纤维支气管镜或探条的前部与气管导管套入部分成角时,气管导管前段的斜面遇到梨状窝、会厌或声带等,极易受阻,用暴力勉强送管将造成声带、会厌和喉的损伤、纤维支气管镜损伤、插管困难等。为此,本人设计平口、侧孔气管导管和导芯,导管的最前面为一圆形平口,开口处内径为5-10mm,同时带有一侧孔,可加强通气;引导导管进入气管内的是一导芯,导芯最前端是半径为3-5mm的半球,随后是30-50mm的前圆柱,半径与前端半球的半径一样,长为30-50mm,随后是一段圆台,圆台的长是10-30mm,随后是后圆柱,直径是5-9mm,长是50-70cm。本导芯的优点是前端为半球形,更容易通过声门,可减少对声带和喉的损伤,使插管医生的喉镜下视野免受气管导管的阻挡,导芯的前端半球和前圆柱先进入声门,随后渐渐增粗的圆台、后圆柱进入声门,内径比导芯后圆柱的直径稍大的气管导管再被插入。无论是普通气管插管还是困难插管时,平口、侧孔气管导管和导芯可使插管医生的视野更加清晰,可避免损伤声带、会厌和梨状窝等,可显著提高插管的成功率。
三、发明内容
目前临床上普遍使用的气管导管的前端为斜面形,通过声门时一旦受阻容易造成声带、会厌和梨状窝等损伤。本人设计的平口、侧孔气管导管和导芯,前端为圆形平口,由导芯的前端半球、前圆柱、圆台、后圆柱依次过声门,再把导管沿着导芯送入气管,拔出导芯即可。本平口、侧孔气管导管可避免导管对喉头的损伤,选择与纤维支气管镜、插管探条直径相适应的平口、侧孔气管导管辅助插管时,将使送管更加容易,插管的成功率将大大提高。
本发明解决其技术问题所采用的技术方案是:气管导管其他部分与目前常用的普通导管无异,只是导管的前端设计为圆形平口,内径为5-10mm,同时还带有一侧孔,为椭圆形,可加强通气;与之配套使用的是一导芯,最前端是一个半球,半径为3-5mm;随后是前圆柱,长为30-50mm,半径等于前端半球的半径;随后是一段圆台,圆台的前截面的半径等于前圆柱的半径,后截面的半径等于后圆柱的半径,长是10-30mm;随后是后圆柱,直径是5-9mm,长是50-70cm。
本发明的有益效果是,可以减少导管与喉的接触,让导芯较细的前端半球、前圆柱先通过声门进入气管内,因此可使插管医生的视野免受气管导管的阻挡,随后渐渐增粗的圆台、后圆柱、平口、侧孔气管导管依次通过声门进入气管内,将显著提高困难气管插管的成功率,避免气管导管对声带、会厌和梨状窝的损伤。而普通气管导管的前端为一斜面,除非导管前端的斜面直接对准了声门裂隙,否则在插管受阻时插管医生如果用暴力推进导管则极易造成声带和喉的损伤,同时增加插管的难度。

五、具体实施方式
在图1中显示的是平口、侧孔气管导管,导管的前端为圆形平口,平口旁边带有一个侧孔,后带有一套囊,可充气,导管开口的内径是5-10mm。在图2中显示的是导芯,最前端是一半球,直径是3-5mm;随后是前圆柱,长为30-50mm,半径等于前端半球的半径;随后是一圆台,前截面的半径等于前圆柱的半径,后截面的半径等于后圆柱的半径,长是10-30mm;随后是后圆柱,直径是5-9mm,长是50-70cm。
本平口、侧孔气管导管的内径为5-10mm,临床麻醉医生可根据病人的需要,选择相应内径的导管,导芯后圆柱的直径比导管的内径小1-2mm。对估计有困难气管插管的病人,可先将导芯插入气管内,依次让导芯的前端半球、前圆柱、圆台、后圆柱通过声门,再将平口、侧孔气管导管沿着导芯插入气管内,拔出导芯即可;也可先将平口、侧孔气管导管套入导芯到达后圆柱与圆台的交界处,插管医生手握气管导管的末端与导芯的交界处,依次让导芯的半球、前圆柱、圆台、后圆柱和导管通过声门,待导管套囊进入声门后,渐渐后退导芯,把导管插入需要深度后,完全退出导芯,给套囊充气,进一步确认导管在气管内即可完成气管插管。

Claims (3)

1.一种平口、侧孔气管导管,前端是圆形,带有一侧孔和套囊;与之配套使用的导芯由半球、前圆柱、圆台、后圆柱构成。
2.根据权利要求1所述的平口、侧孔气管导管,其特征是前端是圆形平口,带有一侧孔和套;囊,内径为5-10mm。
3.根据权利要求1所述的导芯,由半球、前圆柱、圆台、后圆柱构成,其特征是前端半球的直径是3-5mm;前圆柱长为30-50mm,半径等于前端半球的半径;圆台的前截面的半径等于前圆柱的半径,后截面的半径等于后圆柱的半径,长是10-30mm;后圆柱的直径是5-9mm,长是50-70cm。
