桃仁:性苦、甘,平,归心、肝、大肠经;主治:活血祛瘀,润肠通便,止咳平喘,用于经闭,痛经,癓瘕痞块,跌扑损伤,肠燥便秘。
川芎:性辛,温,归肝、胆、心包经;主治:活血行气,祛风止痛,用于安抚神经,正头风头痛,症瘕腹痛,胸胁刺痛,跌扑肿痛,头痛,风湿痹痛。
柏子仁:性甘,平,归心、肾、大肠经;主治:养心安神,润肠通便,用于虚烦不眠,心悸怔忡,肠燥便秘等症。
葛根:性凉,味甘、辛;主治:解表退热,生津,透疹,升阳止泻,用于外感发热头痛、高血压颈项强痛、口渴、消渴、麻疹不透、热痢、泄泻。
本发明将上述药物组合后,其药效具有协同作用。解:鸡血藤、当归、葛根、良姜、元胡、仙茅、仙灵脾具有活血化瘀、理气止痛,补中益气、温肾壮阳作用,以上七位是本方主药。
党参、麦冬、五味子具有补中益气,生津养血作用,石菖蒲、良姜具有开窍宁神,温中散寒作用,五味子能宁心安神,敛肺滋肾,木香具有舒肝解郁,调节情绪的作用。
本发明的方药集活血化瘀、芳香温痛、宣痹通阳、行气散血、补中益气、温肾壮阳祖国医学治疗胸痹的原则为一体,能达到行气、活血、止痛的目的。经现代药物研究,本方中的药物有扩张冠状动脉血管及增加冠状动脉血流量的作用,其中的鸡血藤、元胡、当归、桃仁较为明显。
方中元胡、五味子、石菖蒲有镇静、催眠、镇痛作用。党参、麦冬、五味子有强心作用。柏子仁、石菖蒲、麦冬能抑制大脑皮层,扩张周围血管,同时能解除血管平滑肌痉挛。
此方剂具有多靶点和双向调节作用,病人服用方便,无副作用,依从性好。长期服用可促进冠脉侧枝循环建立,保护缺血心肌,抑制左室重构。
对冠心病的治疗及预防病变急性发作,稳定延缓病情发展,具有重要的作用和意义,本方治疗冠心病的有效率达75%以上。
下面结合实施例对本发明作进一步说明。
具体实施方式
实施例,自2008年1月至2010年12月,先后对800例冠心病患者进行临床治疗,其中男560例,女240例,年龄36~75岁,病程2天~40年。
1、诊断标准
参照1999年美国心脏病学会(ACC)/美国心脏协会(AHA)/美国医师学会及美国内科学会(ACP-ASIM)联合协定关于《慢性稳定性诊治指南》2002年(ACC/AHA)《不稳定型无ST段抬高心肌梗死诊疗指南》。
2、纳入及排除标准
纳入标准:受试患者须符合上述诊断标准并具备:年龄在36~75岁。
排除标准:
1、严重高血压、糖尿病、恶性肿瘤及肝肾疾病、甲状腺疾病患者;
2、糖尿病性心脏病、甲状腺功能亢进性心脏病、风湿性心脏病等各种心脏病;
3、肝炎、结核等传染病;
4、妊娠或哺乳期妇女;
5、急性心肌梗死患者;
6、过敏体质及对多种药物过敏者。
将纳入标准的800例冠心病患者随机分为治疗I、 II 、III组和对照组,每组200例患者。
治疗I、 II、 III组服用本发明的药物治疗与采用常规治疗的对照组作比较。
治疗Ⅰ组采用以下治疗方案:
药物采用以下重量的原料组成:鸡血藤15g,当归10g,石菖蒲5g,良姜5g,元胡10g,仙茅15g,仙灵脾15g,党参15g,麦冬10g,五味子5g,姜半夏10g,木香5g,桃仁5g,川芎10g,柏子仁10g,葛根10g。
将上述原料全部放入2L的冷水中浸泡30~45分钟。然后用中火煮沸,开锅后继续用文火煮25分钟,滤出药液约200ml,然后再向锅内加入1.5L的冷水,用中火煮沸后继续用文火煮20分钟,滤出药液约150ml。将两次滤出的药液混合均匀,每日一剂,分早晚饭后半小时温服,治疗组均不用硝酸盐类药物。
治疗II组采用以下治疗方案:
药物由以下重量的原料组成:鸡血藤25g,当归20g,石菖蒲15g,良姜15g,元胡25g,仙茅25g,仙灵脾25g,党参25g,麦冬20g,五味子15g,姜半夏20g,木香15g,桃仁15g,川芎20g,柏子仁25g,葛根20g。
将上述原料全部放入3.5L的冷水中浸泡30~45分钟。然后用中火煮沸,开锅后继续用文火煮30分钟,滤出药液约350ml,然后再向锅内加入2L的冷水,用中火煮沸后继续用文火煮20分钟,滤出药液约250ml。将两次滤出的药液混合均匀,每日一剂,分早晚饭后半小时温服,治疗组均不用硝酸盐类药物。
治疗III组采用以下治疗方案:
药物由以下重量的原料组成:鸡血藤20g,当归15g,石菖蒲10g,良姜10g,元胡20g,仙茅20g,仙灵脾20g,党参20g,麦冬15g,五味子10g,姜半夏15g,木香10g,桃仁10g,川芎15g,柏子仁20g,葛根15g。
将上述原料全部放入3L的冷水中浸泡30~45分钟。然后用中火煮沸,开锅后继续用文火煮25分钟,滤出药液约300ml,然后再向锅内加入1.5L的冷水,用中火煮沸后继续用文火煮20分钟,滤出药液约200ml。将两次滤出的药液混合均匀,每日一剂,分早晚饭后半小时温服,治疗组均不用硝酸盐类药物。
对照组采用传统的治疗方案,用硝酸盐类、β受体阻滞剂、阿斯匹林。
所有患者治疗期间,均每天随访一次,包括症状、体征改善情况及心电图常规检查。根据《中华人民共和国中医药行业标准·中医病证诊断疗效标准》制定疗效标准,治疗满一个疗程后,症状消失及心电图好转为有效,未达到上述标准者为无效。
经随访得出,在一个疗程的治疗后,治疗I组200例患者中有150例患者达上述标准,并且临床症状完全消失,50例患者临床症状部分改善或没有改善。总有效率为75%。
治疗II组200例患者中,有169例患者达上述标准,并且临床症状全部消失,31例患者临床症状部分改善或没有改善。总有效率为84.5%。
治疗III组200例患者中有171例达到上述标准,并且临床症状完全消失,29例患者临床症状心电图部分改善或没有改善。总有效率85.5%。
对照组200例患者中141例达到上述标准并且临床症状完全消失,59例患者临床症状及心电图部分改善或没有改善,总有效率70.5%。
综上所述证明:治疗组的治疗效果明显优于对照组,而治疗组三个组中,治疗III组为治疗方案中最优治疗方案,疗效虽然与治疗II组近似,但治疗III组比II组价格低廉,能为病人减轻经济负担。
典型病例
1.王某,女,56岁,阵发性胸痛5年,加重一个月。现心前区闷痛,每天发作10余次,每次持续约5至10分钟,常于活动后发作,伴胸闷心慌,心电图Ⅱ、Ⅲ、avF、V5、V6导联ST段下移1.0~1.5mv。经多方治疗效果不明显,后来我院给予与治疗II组相同的药物进行治疗口服,早晚各一次,治疗三天后,仅偶有轻微胸痛,治疗一周后症状完全消失,瞩患者连服一个月。随访一年未复发。
2.李某,男,72岁,因心前区发作性疼痛伴胸闷头晕10余年,加重2天入院,往有冠心病病史10余年,经常服用消心痛或硝酸甘油等治疗。查体:血压170/120mm/Hg,做心电图检查ST-T倒置、左心室肥厚。结合病史,病症,心电图等诊断为冠心病。入院后给予与治疗I组相同的药物进行口服,早晚各一次,服用后一天即发作次数明显减少,三天后的症状消失,瞩患者连服一个月,随访一年患者未复发。
3.郭某,女,57岁,冠心病5年,近月来胸闷,气短,逐渐加重,胸部紧闷感,烦闷感,压迫感,每天发作2-4次,每次持续1-10分钟,饮食二便正常。有高血压病史25年。心电图显示广泛性心肌缺血,结合病史,病症,心电图等诊断为冠心病。入院后给予与治疗III组相同的药物进行口服,早晚各一次,服用一天后,胸闷及心前区紧束,压迫感均明显减轻,服药三天后的症状消失,心电图好转,瞩患者连服一个月,随访一年未见复发。

1.一种治疗冠心病的中药,该药物由以下重量的原料组成:鸡血藤15~25g,当归10~20g,石菖蒲5~15g,良姜5~15g,元胡10~25 g,仙茅15~25 g,仙灵脾15~25 g,党参15~25 g,麦冬10~20 g,五味子5~15g,姜半夏10~20 g,木香5~15g,桃仁5~15g,川芎10~20g,柏子仁10~25g,葛根10~20g。
2.根据权利要求1所述的一种治疗冠心病的中药,其特征在于:该药物由以下重量的原料组成:鸡血藤15g,当归10g,石菖蒲5g,良姜5g,元胡10g,仙茅15g,仙灵脾15g,党参15g,麦冬10g,五味子5g,姜半夏10 g,木香5g,桃仁5g,川芎10g,柏子仁10g,葛根10g。
