背景技术
医学所指痔疮包括内痔、外痔、混合痔,是肛门直肠底部及肛门粘膜的静脉丛发生曲张而形成的一个或多个柔软的静脉团的一种慢性疾病。据有关普查资料表明,肛门直肠疾病的发病率为59.1%,痔占所有肛肠疾病的87.25%,而其中又以内痔最为常见,占所有肛肠疾病的52.19%。男女均可得病,女性的发病率为67%,男性的发病率为53.9%;任何年龄都可发病,其中20-40岁的人较为多见,并随着年龄的增长而逐渐加重,故有“十人九痔”之说。
痔疮发病原因颇多,久坐、久站、劳累等使人体长时间处于一种固定体位,从而影响血液循环,使盆腔内血流缓慢和腹内脏器充血,引起痔静脉过度充盈、曲张、隆起、静脉壁张力下降而引起痔疮是发病的重要原因之一。若运动不足,肠蠕动减慢,粪便下行迟缓或因习惯性便秘,从而压迫静脉,使局部充血和血液回流障碍,引起痔静脉内压升高,静脉壁抵抗力降低,也可导致痔疮发病率增高。据临床观察及统计普查结果分析,不同职业患者中的患病率有显著差异,临床上机关干部、汽车司机、售货员、教师的患病率明显较高。
历代医学家都有不少关于痔的论述,他们认为,本病的发生,多因饮食不节,过食辛辣,酒色度,湿热内生,下注大肠所致;或因久泻久痢,久坐久立,久忍大便,妇女妊娠而引起阴阳不和,关格壅塞,经脉流溢,渗漏肠间,以致冲发为痔;或因外感风、湿、燥热之邪下冲门所致;或因内伤七情,热毒蕴积,气血壅滞下坠,经络不通而淤滞结聚于肛门,以致冲突为痔。
痔疮最主要的症状是便血和脱出,大便时反复多次地出血,会使体内丢失大量的铁,引起缺铁性贫血。痔疮较严重者会导致或诱发心脑血管疾病,尤其是老年性患者,如患痔疮产生心理压力,不敢上厕所,长此下去会加重便秘,当排便发生困难时,患者用力屏气,可使心跳加快造成脑血管破裂,引起脑出血或脑栓塞;如果出现内痔嵌顿,疼痛还可诱发心绞痛发作;如有血栓形成,可引发肿栓塞。
现有的痔疮治疗方法有如下几种:1、中医药物治疗:马应龙麝香痔疮栓、马应龙麝香痔疮膏、地奥司明片、痔炎消片。此药物疗法可以暂时缓解病症,却无法根治。2、手术治疗:目的是摘除痔核或用缝扎等机械方法使之栓塞或萎陷。电凝或激光照射也很有效。但是要注意术后的保养,稍不谨慎就容易复发,另外手术可能会导致大便失禁等后遗症。3、注射疗法:目前国内外好多采用注射疗法,既将药物注入痔核内治疗痔疮的方法。临床通常使用的注射剂主要有硬化萎缩剂和枯痔坏死脱落剂两种。硬化剂适用于各期内痔,目前临床上比较常用,但药物引起的无菌性炎性反应消失后,纤维化收缩和挤压作用逐渐缓和,痔核易复发。如注射部位过浅,则易引起粘膜溃烂粘膜脓肿,过深易引起荃层组织发生硬化,药量过大,可引起坏死大出血,药液渗入齿线外可引起肛门剧烈疼痛、水肿或血栓形成;坏死剂用量过大常可引起术后坏死性大出血或感染坏死面形成溃疡,特别是广泛的组织坏死或感染,不但治疗时间。4、超低温、超高温疗法:液氮冷冻、激光、微波、电子(包括低频、射频、电容场、电离子透入以及电动振荡)等各种治疗仪器,其治疗原理是利用超低温或超高温产生冷或热效应,振荡电离子透入生物物理效应、电场电容效应等,从而起到使病灶组织蛋白凝固、血管栓塞封闭、电灼电凝止血、切割、组织变性、坏死脱落或硬化萎缩等治疗目的,仍属于硬化和枯痔疗法类型重复再现,同手术疗法结果是相似的。5、结扎、套扎疗法:会使肛门部血液循环受阻,易产生淤血水肿、疼痛、排便困难等。
发明内容
针对上述领域中的不足,本发明提供一种治疗痔疮的中药,见效快,且痔不易复发。
同时还提供该治疗痔疮的中药的制备方法。
一种治疗痔疮的药物,其药效成分为黑木耳菌丝或母种深层发酵后的发酵液中的固体物,其底物为虎杖和地榆的水提取物,所述虎杖和地榆的重量份为虎杖18-25份,地榆3-8份,盐肤木根3-8份。
所述虎杖、地榆和盐肤木根的重量份为虎杖20份,地榆5份,盐肤木根5份。
所述的治疗痔疮的药物,为由药效成分组成的口服粉剂。
治疗痔疮的药物的制备方法,是将黑木耳母种菌丝悬浊液在所述的底物中深层发酵,发酵液浓缩后的固体物即为药效成分。
包括如下顺序步骤:1)制备虎杖、地榆和盐肤木根的水提取液,2)将水提取液灭活,3)加入黑木耳菌丝母种悬浊液深层发酵,4)浓缩发酵液成干粉得药效成分。
所述深层发酵的条件为在28~34℃下,发酵72~144小时,接种量为5-10%。
所述灭活为将水提取液置于高压锅内高温杀菌30分钟。
所述水提取液的制备方法为按重量份称取虎杖、地榆和盐肤木根,加入4-6倍重量份的饮用水微沸1-2小时,滤汁留渣,再加4-6倍重量份的饮用水微沸2-4小时,滤汁,合并滤汁即可。
所述黑木耳菌丝母种悬浊液的制备方法采用如下步骤:A)黑木耳菌丝的斜面培养,B)黑木耳菌丝液体发酵扩繁成悬浊液;所述斜面培养基为:马铃薯20克,葡萄糖20克,磷酸二氢钾2克,蛋白胨10克,牛肉膏5克,酵母膏5克,硫酸镁0.8克,水1000毫升;所述扩繁培养基为:50g磷酸氢二钾、50g磷酸二氢钾、20g硫酸锌、20g天冬酸、20g甘露醇、500g白糖、200g酵母浸膏,加入底物溶液至15kg。
上述药物的使用方法,将30-50g鸡蛋清或40-60g鸡蛋内容物(不宜摄入蛋黄的人群如胆固醇高的去蛋黄使用蛋清)用沸水冲沏成蛋花汤,再加入红糖3-5g,待冷却到口服的温度再加入上述药物,口服。
本发明根据中医理论,将中草药技术与生物发酵技术相结合,以药用植物虎杖和地榆以及盐肤木的水提取物作为液体培养基底物,将药用真菌黑木耳母种菌丝液深层发酵而制得发酵液中的固体物作为治疗的痔疮的有效成分,口服治疗痔疮,有效率达90%以上,能萎缩出血管,盐肤木的独特功效,原痔疮位的不再复发。
虎杖:
【别名】花斑竹、酸筒杆、酸桶笋、酸汤梗、川筋龙、斑庄、斑杖根、大叶蛇总管、黄地榆
【基原】本品为蓼科Polygonaceae植物虎杖Polygonum cuspidatum Sieb.et Zucc.,的干燥根茎和根。
【性味与归经】微苦,微寒。归肝、胆、肺经。
【功能与主治】清热解毒,利胆退黄,祛风利湿,散瘀定痛,止咳化痰。用于关节痹痛,湿热黄疸,经闭,癓瘕,咳嗽痰多,水火烫伤,跌扑损伤,痈肿疮毒。
虎杖含有蓼甙、有机酸、葡萄糖甙、多糖类等。有清热解毒、清凉解署、健胃消食作用。

别名:酸赭、豚榆系、白地榆、鼠尾地榆、西地榆、地芽、野升麻、马连鞍、花椒地榆、水橄榄根、线形地榆、水槟榔、山枣参、蕨苗参、红地榆、岩地芨、血箭草、黄瓜香来源为为蔷薇科地榆属的植物地榆(学名:Radix Sanguisorbae)的干燥根。

功效凉血止血,清热解毒,消肿敛疮
主治吐血,咯血,衄血,尿血,便血,痔血,血痢,崩漏,赤白带下,疮痈肿痛,湿疹,阴痒,水火烫伤,蛇虫咬伤。
地榆根部含有地榆苷I、II、A、B、E等及酚酸类性化合物,尚含少量维生素A。止血主要成分为鞣质。

盐肤木别名:盐霜柏、盐酸木、敷烟树、蒲连盐、老公担盐、五倍子树
来源:漆树科植物盐肤木Rhus chinensis Mill.以根、叶入药。根全年可采,夏秋采叶,晒干。
性味归经:酸、咸,寒。
功能主治:清热解毒,散瘀止血。根:感冒发热,支气管炎,咳嗽咯血,肠炎,痢疾,痔疮出血;根、叶外用治跌打损伤,毒蛇咬伤,漆疮。

黑木耳是一种味道鲜美、营养丰富的食用菌,含有丰富的蛋白质、铁、钙、维生素、粗纤维,其中蛋白质含量和肉类相当,铁比肉类高10倍,钙是肉类的20倍,维生素B 2是蔬菜的10倍以上,黑木耳还含有多种有益氨基酸和微量元素,被称之为“素中之荤”。黑木耳具有益气强身、滋肾养胃、活血等功能,它能抗血凝、抗血栓、降血脂,黑木耳的作用降低血粘,软化血管,使血液流动通畅,减少心血管病发生。黑木耳还有较强的吸附作用,经常食用利于使体内产生的垃圾及时排出体外。黑木耳对胆结石、肾结石也有较好的化解功能,黑木耳的作用因为它所含的植物碱具有促进消化道、泌尿道各种腺体分泌的特性,植物碱能协同这些分泌物催化结石,润滑肠道。黑木耳还含有对人体有益的植物胶质以及一种叫做“多糖体”的物质,这是一种天然的滋补剂,和黑木耳中丰富的纤维素共同作用,能促进胃肠蠕动而防止便秘,有利于体内大便中有毒有害物质及时清除和排出,从而起到预防直肠癌等癌症的作用,还可促使肠道脂肪食物的排泄,减少食物中脂肪的吸收,从而起到防止肥胖和减肥作用。
总之,黑木耳主要是具有润肺补脑,活血止血之功效。
本发明中的虎杖、地榆和盐肤木根的水提取物在黑木耳的深层发酵过程是作为其发酵的培养基的,提供其生物发酵所需的氮源、碳源,采用该培养基,深层发酵物中孢外多糖较多,黑木耳生长较好;同时,在本发明的药物中,虎杖、地榆和盐肤木根的水提取物还是药用成分,由于采用的是发酵液中的固体物作为有效成分,其中还含有菌的代谢产物等。利用虎杖、地榆和盐肤木根解毒清热功效,使热毒排出,消痔去肿;利用虎杖和黑木耳的活血之功效,改膳肛门处的血液微循环,使其不易淤滞结聚;盐肤木根的收敛功能,其他中草药无可比拟,同时还利用黑木耳和地榆的止血功能,使破损的痔疮处迅速止血,促进伤口愈合;同时还利用黑木耳中的植物胶质、多糖体、代谢产物与丰富的纤维素,促进胃肠蠕动,排出体内有害毒素,使整个身体处于正常代谢状态,经络通畅,体质增强,足于抵抗外来风、湿、燥热之邪。由于本发明中虎杖、地榆和盐肤木根与黑木耳的深层发酵物相配合,且地榆用量少,没有出现便秘的现象(杨景宽等临床曾观察发现:“内服地榆,在治疗过程中发生便秘较多”。——摘自《中华内科杂志》,1960(3):248)。
本发明药中加入虎仗、地榆和盐肤木根之后,经深层发酵(《内经》曰醪醴),收敛力特强,在出血状态下的痔疮,12小时后止血,有瘘管的,即萎缩。
本发明的药物使用方法:用100℃100g沸水,将一枚鸡蛋清30-50g或40-60g鸡蛋内容物(不宜摄入蛋黄的人群如胆固醇高的去蛋黄使用蛋清)冲成“蛋花汤”,加入红糖3-5g,熔化后稍凉,加入上述干粉。48小时内,总共使用四~六次。早晚各一次,第一次使用时加干粉10g,第二次至第六次,每次干粉5g,通常总计服用四次可愈,严重者服用六次痊愈。男性患者总有效率100%,女性患者总有效率90%。
服用本药后,注意饮食调理,不食或少食辛辣食,戒烟酒,由于萎缩了出血管所以原痔疮位不再复发。
具体实施方式
实施例1
1、底物制备:
将洗净泥沙并晒干的20kg虎仗加5kg地榆,5kg盐肤木根,置于130kg的饮用水中煮沸后小火烧二小时,滤汁留渣,再加入130kg饮用水,煮沸,文火三小时,滤汁,弃渣。将上述两次药汁合并约得265kg,
2、黑木耳菌丝母种:采集鲜木耳子实体,接种在20毫升的试管内(装有8毫升斜面培养基)进行斜面培养,确认无杂菌后,留以备用。
斜面培养基:马铃薯20克葡萄糖20克磷酸二氢钾2克蛋白胨10克牛肉膏5克酵母膏5克硫酸镁0.8克水1000毫升
3、扩繁成混浊液
3、1.材料:
3.1.1营养琼脂培养基;(上海中药昆虫生物科技开发有限公司康乐培养基厂)
3.1.2.酵母浸膏;(北京双旋微生物培养基制品厂)
3.1.3磷酸氢二钾;(广东汕头西陇化工厂)
3.1.4硫酸锌(分析纯);(上海兴塔化工厂)
3.1.5DL-天冬酸;(天津华北地区特种试剂开发中心)
3.1.6甘露醇;(广州佳德化工有限公司)
3.1.7磷酸二氢钾;(青岛市广汇化工厂)
3.1.8白糖;(超市购买)
3.1.9高锰酸钾;(天津恒兴化学制剂有限公司)
3.1.10福尔马林;(山东济南试剂厂)
3.1.11黑木耳菌丝试管;(中国农科院菌种保存中心)
3.2器具:
3.2.1.500ml玻璃瓶;
3.2.2.手提高压锅,电煤双用;(上海申仪三厂)
3.2.3.摇床:D4-900;(江苏太仑市实验设备厂)
3.2.4.超净工作台;
3.2.5.酒精灯;
3.2.6.勾、耙、针、刀;
3.2.7.火柴;
3.2.8.消毒棉花;
3.2.9.75%乙醇;
3.2.10.橡皮筋。
3.3方法:
3.3.1.在10kg中药溶液底物内分别加入50g磷酸氢二钾、50g磷酸二氢钾、20g硫酸锌、20g天冬酸、20g甘露醇、500g白糖、200g酵母浸膏,搅拌加温至100℃,放入1kg营养琼脂培养基,溶化后再注入5kg中药溶液底物,制成15kg混浊液。
3.3.2.在500ml玻璃瓶内注入400ml混浊液溶液,置入手提高压锅内在0.012MP/cm2,30min,取出后冷却至28℃-34℃,放入超净工作台内。
3.3.3.工具和器具以及灭活后的培养基放入超净工作台内,用30g高锰酸钾置入盛有50%福尔马林30ml的器皿中,灭菌30min。将试管内黑木耳母种,切成三份,每份接入400ml/瓶的混浊液内,静养120min~150min。
3.3.4.在摇床上振荡以20次/min,72~96小时。
4深层发酵
以1∶10的比例扩繁后的混浊液注入盛有并经常压灭活后的底物,在发酵罐内深层发酵72~144小时(28~34℃),放罐再抽真空,罐内液体浓缩,干燥得干粉即可。
该干粉可以制成各种口服的制剂,还可以直接作为口服干粉制剂。
临床统计:
门诊接受痔疮患者共计60例,男42例,女18例。
使用方法:用100℃100g沸水,将一枚鸡蛋清冲成“蛋花汤”,或是1枚整鸡蛋加入红糖5g,熔化后稍凉,加入上述干粉。总共使用四~六次。早晚各一次,第一次使用时加干粉10g,第二次至第六次,每次干粉5g,通常总计服用四次可愈,严重者服用六次痊愈。
痔疮消失,基本治愈,两个月后原痔疮位不复发,为痊愈;排便通畅不再出血为有效。
在服用6次本发明的药物后,男子有效率达100%,女子有效率达90%,不再复发的达80%左右。
典型病例:
陈×强:男,55岁,痔瘘,上海市人,住江苏太仑市,患混合痔15年,出血不止,总计使用六次痊愈。
肖×:男,50岁,珠海人,痔瘘行手术切除一次未愈,总计使用四次痊愈。
肖××,男,60岁,珠海人,痔瘘多年,总计使用四次痊愈。
邹××,女,61岁,湖北人,痔瘘多年,相隔半年治疗两次,同量共使用四次痊愈。
陈××,男,93岁,香港居民(已故),生前使用六次,结束20年病程痊愈。
Claims (10)
1.一种治疗痔疮的药物,其药效成分为黑木耳菌丝母种深层发酵后的发酵液中的固体物,其底物为虎杖、地榆、盐肤木根的水提取物,所述虎杖、地榆和盐肤木根的重量份为虎杖18-25份,地榆3-8份,盐肤木根3-8份。
2.根据权利要求1所述的治疗痔疮的药物,所述虎杖和地榆的重量份为虎杖20份,地榆5份,盐肤木根5份。
3.权利要求1-2任一所述的治疗痔疮的药物,为由药效成分组成的口服粉剂。
4.权利要求3治疗痔疮的药物的制备方法,是将黑木耳母种菌丝悬浊液在所述的底物中深层发酵,发酵液浓缩后的固体物即为药效成分。
5.权利要求4所述的制备方法,包括如下顺序步骤:1)制备虎杖、地榆和盐肤木根的水提取液作为底物溶液,2)将水提取液灭活,3)加入黑木耳菌丝母种悬浊液深层发酵,4)浓缩发酵液成干粉得药效成分。
6.权利要求5所述的制备方法,所述深层发酵的条件为在28~34℃下,发酵72~144小时,接种量为5-10%。
7.权利要求5所述的制备方法,所述灭活为将水提取液置于高压锅炉中高温杀菌30分钟。
8.权利要求5所述的制备方法,所述水提取液的制备方法为按重量份称取虎杖、地榆和盐肤木根,加入4-6倍重量份的饮用水微沸1-2小时,滤汁留渣,再加4-6倍重量份的饮用水微沸2-4小时,滤汁,合并滤汁即可。
9.权利要求5所述的制备方法,所述黑木耳菌丝母种悬浊液的制备方法采用如下步骤:A)黑木耳菌丝的斜面培养,B)黑木耳菌丝液体发酵扩繁成悬浊液;所述斜面培养基为:马铃薯20克,葡萄糖20克,磷酸二氢钾2克,蛋白胨10克,牛肉膏5克,酵母膏5克,硫酸镁0.8克,水1000毫升;所述扩繁培养基为:50g磷酸氢二钾、50g磷酸二氢钾、20g硫酸锌、20g天冬酸、20g甘露醇、500g白糖、200g酵母浸膏,加入底物溶液至15kg。
10.权利要求3所述的药物的使用方法,将30-50g鸡蛋清或40-60g鸡蛋内容物用沸水冲沏成蛋花汤,再加入红糖3-5g,冷却后加入权利要求3所述药物,口服。
